Although more than 180 signatories have ratified the FCTC, including the European Union (EU) and all its member states, compliance across the EU is <a href=\"https:\/\/globalizationandhealth.biomedcentral.com\/articles\/10.1186\/s12992-018-0386-1\">incomplete<\/a>. This lack of compliance is illustrated by the recent pattern of partnerships between Phillip Morris International (PMI) and local authorities in three European countries; <a href=\"https:\/\/www.keeptalkinggreece.com\/2018\/10\/08\/astypalaia-smoke-free-island\/\">Greece<\/a>, <a href=\"https:\/\/lagraciosasinhumo.com\/en\/\">Spain<\/a>, and the <a href=\"https:\/\/mmkv.cz\/cs\/aktuality\/karlovy-vary-chteji-byt-prvnim-mestem-bez-koure-v-cr\">Czech Republic<\/a>.<\/p>\n<p>Astypalaia is a small Greek island in the Aegean Sea. In October 2018 Astypalaia\u2019s Mayor announced, in a public event, a collaboration with Papastratos (the Greek subsidiary of PMI) with the aim to make the island \u201csmoke-free\u201d. The Mayor of Astypalaia was <a href=\"https:\/\/www.in.gr\/2018\/10\/08\/health\/health-news\/astypalaia-proto-smoke-free-nisi-pagkosmios\/\">joined<\/a> by the Regional Governor of Southern Aegean (both elected officials) and the Marketing Director of <a href=\"https:\/\/papastratosmazi.gr\/ola-ta-nea-ths-papastratos\/orama-epistimi\/astypalaia-smoke-free\/\">Papastratos<\/a>. They all highlighted the stated aim of the collaboration was to improve the health and wellbeing of citizens and tourists through smoking bans in enclosed spaces and eventually in outdoor <a href=\"https:\/\/www.cnn.gr\/ellada\/story\/149876\/giati-i-astypalaia-einai-to-proto-smoke-free-nisi-ston-kosmo\">spaces<\/a>. Smoking in enclosed public spaces has been banned in Greece since 2008 (law 3730\/2008), although poorly enforced. The initiative described above was not accompanied by enforcement <a href=\"https:\/\/www.keeptalkinggreece.com\/2018\/10\/08\/astypalaia-smoke-free-island\/\">measures<\/a>; locals and visitors were simply informed through videos, volunteers, brochures and posters provided by <a href=\"https:\/\/www.in.gr\/2018\/10\/08\/health\/health-news\/astypalaia-proto-smoke-free-nisi-pagkosmios\/\">Papastratos<\/a> and encouraged to <a href=\"https:\/\/www.cnn.gr\/ellada\/story\/149876\/giati-i-astypalaia-einai-to-proto-smoke-free-nisi-ston-kosmo\">comply<\/a>. The Marketing Director of Papastratos stated <a href=\"https:\/\/papastratosmazi.gr\/ola-ta-nea-ths-papastratos\/orama-epistimi\/astypalaia-smoke-free\/\">publicly<\/a> that \u201c[\u2026] a world without cigarettes is the future\u201d and committed to supporting the Astypalaia initiative through worldwide promotion and acts of corporate social responsibility on the <a href=\"https:\/\/www.cnn.gr\/ellada\/story\/149876\/giati-i-astypalaia-einai-to-proto-smoke-free-nisi-ston-kosmo\">island<\/a>. The Mayor of Astypalaia also suggested that in a smoke-free environment, smokers \u201c[\u2026] could attempt to stop smoking, [\u2026]but we understand that some people won\u2019t quit, so we will inform them of the alternatives <a href=\"https:\/\/www.in.gr\/2018\/10\/08\/health\/health-news\/astypalaia-proto-smoke-free-nisi-pagkosmios\/\">available<\/a>\u201d.<\/p>\n<p>The example of Astypalaia has been followed by <a href=\"https:\/\/lagraciosasinhumo.com\/en\/\">La Graciosa<\/a>, a small Spanish island and by the spa city of <a href=\"https:\/\/mmkv.cz\/cs\/aktuality\/karlovy-vary-chteji-byt-prvnim-mestem-bez-koure-v-cr\">Karlovy Vary<\/a> in the Czech Republic. In all three cases, the pattern is similar. Participating cities partner with PMI and receive a \u2018Smoke-free culture\u2019 certification by <a href=\"https:\/\/www.tuv.at\/en\/solutions\/business-assurance\/management-systems\/smoke-free-culture\">T\u00dcV Austria<\/a> which, among others, requires them to \u201cresponsibly inform the employees of the organization who are cigarette smokers and consciously wish to continue smoking, regarding the potential and scientifically suggested alternative to cigarette choices\u201d. Such references to alternative tobacco products, including heated tobacco products (HTP) are consistently made in official webpages and press releases and constitute a key element of this <a href=\"https:\/\/ash.org\/graciosa\/\">strategy<\/a>.<\/p>\n<p>This approach seems entirely consistent with PMI\u2019s efforts to promote its HTP IQOS through its self-proclaimed commitment to a \u2018<a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(17)32561-8\/fulltext\">smoke-free world<\/a>\u2019. To this end, PMI seeks opportunities to engage with local and national governments, as well as state-owned organisations, such as the Thessaloniki International Fair (TIF), the main international annual trade and consumer exhibition in Greece, which is organised by a state organisation (HELEXPO). TIF was <a href=\"https:\/\/www.thessalonikifair.gr\/en\/certified-smoke-free-tif\">self-declared<\/a> smoke-free in 2018 with the <a href=\"https:\/\/www.businessnews.gr\/ellada\/item\/127677-smoke-free-deth-me-tin-ypostiriksi-tis-papastratos\">support<\/a> of Papastratos. Not surprisingly, in February 2020 the CEO of Papastratos clearly suggested a \u2018better\u2019 regulatory environment in the presence of multiple Greek government <a href=\"https:\/\/www.amna.gr\/en\/article\/432062\/Papastratos-unveils-plan-for-one-million-fewer-smokers-in-Greece-by-end-2021\">officials<\/a>. A few months later, new legislation (law 4715\/2020, article 36) was passed by the Greek government setting up a mechanism through which producers of novel tobacco products can receive approval to communicate claims of reduced harm to consumers.<\/p>\n<p>The case of Astypalaia and the subsequent increased presence of PMI in Greek public affairs highlights the risks of the tobacco industry\u2019s involvement in tobacco control policies. PMI seems determined to continue with this approach despite strong opposition from the public health community which hasn\u2019t been able to <a href=\"https:\/\/ash.org\/graciosa\/\">reverse<\/a> the decision of La Graciosa to partner with the tobacco industry.
