Their choice of device, however, was mostly limited to the clunky tank systems supplied by small manufacturers willing to sell on the grey market. Advertising for vaping products was seen only in the broadcasts and media that spilled over from the United States.<\/p>\n<p>After market liberalization, Canadians could buy their (not yet) regulated devices and liquids in almost every corner store. Their choice of device was expanded to include the sleek and inexpensive systems sold by large multinational manufacturers, such as JUUL, BAT, Japan Tobacco, and Imperial Brands. Pod systems selling high concentrations of nicotine salts became the norm (<a href=\"https:\/\/www.cbc.ca\/news\/canada\/toronto\/juul-opens-first-store-toronto-1.5229322\" target=\"_blank\" rel=\"noopener noreferrer\">JUUL boasts of a 79% market share<\/a>). In many regions, advertising for vaping products has become pervasive: the cigarette power walls that were removed only a decade ago have now been replaced with vaping advertising.<\/p>\n<figure id=\"attachment_1421\" aria-describedby=\"caption-attachment-1421\" style=\"width: 300px\" class=\"wp-caption alignleft\"><a href=\"https:\/\/blogs.bmj.com\/tc\/files\/2019\/09\/Canada-vaping-display.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-1421\" src=\"https:\/\/blogs.bmj.com\/tc\/files\/2019\/09\/Canada-vaping-display-300x225.jpg\" alt=\"\" width=\"300\" height=\"225\" srcset=\"https:\/\/blogs.bmj.com\/tc\/files\/2019\/09\/Canada-vaping-display-300x225.jpg 300w, https:\/\/blogs.bmj.com\/tc\/files\/2019\/09\/Canada-vaping-display.jpg 618w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><figcaption id=\"caption-attachment-1421\" class=\"wp-caption-text\">Vaping promotions at an Esso gas station near Ottawa, August 2019<\/figcaption><\/figure>\n<h5>A balancing act of competing policy objectives<\/h5>\n<p>These changes to the vaping market resulted from the government\u2019s decision in 2017 to adopt a <a href=\"https:\/\/www.canada.ca\/en\/health-canada\/services\/publications\/healthy-living\/canada-tobacco-strategy\/overview-canada-tobacco-strategy.html\" target=\"_blank\" rel=\"noopener noreferrer\">harm reduction approach<\/a> to smoking. The new law was structured to \u201cstrike a balance between protecting youth from inducement to nicotine and tobacco use, while <a href=\"https:\/\/sencanada.ca\/en\/Content\/Sen\/Committee\/421\/SOCI\/53253-e\" target=\"_blank\" rel=\"noopener noreferrer\">allowing adult smokers to legally access vaping products as a less harmful alternative to tobacco<\/a>.\u201d<\/p>\n<p>With this decision, and the adoption of the <a href=\"https:\/\/www.canada.ca\/en\/health-canada\/services\/health-concerns\/tobacco\/legislation\/federal-laws\/tobacco-act.html\" target=\"_blank\" rel=\"noopener noreferrer\"><em>Tobacco and Vaping Products Act<\/em><\/a>, Canadians were abruptly enrolled in a natural experiment on the impact of a liberalized nicotine market on their health behaviours. Canadians <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6182113\/\" target=\"_blank\" rel=\"noopener noreferrer\">have mixed views towards novel tobacco control measures<\/a>, but many had supported greatly expanding access to vaping products.<\/p>\n<p>Assessing the results of this experiment has been challenged by the absence of early data on the impact on nicotine use. The health surveillance systems in place for the first 18 months did not include any indicators on vaping: <a href=\"http:\/\/www23.statcan.gc.ca\/imdb\/p2SV.pl?Function=getSurvey&amp;SDDS=4440\" target=\"_blank\" rel=\"noopener noreferrer\">Canadian Tobacco, Alcohol and Drugs Survey<\/a> had been suspended in 2017 and its replacement will not be in the field until November 2019.<\/p>\n<h5>Assessing the impact<\/h5>\n<p>To address the information gap, Health Canada issued contracts totalling more than USD 1 million to polling firms and other consumer researchers, tasking them with surveying Canadians about vaping. These studies, listed below, probed for information on a wide range of behaviours and attitudes related to nicotine use \u2013 why people start, which products they use, what they think of the harms, the flavours they use, whether their friends and families approve, etc.<\/p>\n<p>These studies do not allow us to conclude whether the Canadian vaping experiment has resulted in a net reduction of harm from nicotine use. They do, however, give insight into the minds and motivations of users. When the surveillance system gets up and running, this consumer research can help guide policy adjustments if it appears that the harm reduction balance needs recalibrating.<\/p>\n<p>Had these studies been conducted by University-based researchers, the results would normally find their way into the academic press. Canadian governments do not take this approach, but instead make the findings and data tables available through the <a href=\"https:\/\/www.bac-lac.gc.ca\/eng\/porr\/Pages\/porr.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">federal government\u2019s library of public opinion research<\/a>. In preparation for updating tobacco package warning labels, consumer research was commissioned to test concepts and visuals. One of the studies looked at reactions to warnings printed directly on cigarettes &#8211; \u2013 an <a href=\"https:\/\/www.canada.ca\/content\/dam\/hc-sc\/documents\/programs\/consultation-tobacco-labelling\/27-18-2150-Tobacco-Consultation-Labels-EN-12.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">idea Health Canada floated last year.<\/a><\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Recent Health Canada consumer research:<\/strong><\/p>\n<p><strong>On vaping<\/strong><\/p>\n<p><a href=\"http:\/\/epe.lac-bac.gc.ca\/100\/200\/301\/pwgsc-tpsgc\/por-ef\/health\/2019\/037-18-e\/index.html\" target=\"_blank\" rel=\"noopener noreferrer\">POR 037-18: Vaping Prevention Concepts Testing <\/a><br \/>\n16 in-person focus groups with 13 to 18 year olds to assess response to creative concepts for youth prevention campaigns.<\/p>\n<p><a href=\"http:\/\/epe.lac-bac.gc.ca\/100\/200\/301\/pwgsc-tpsgc\/por-ef\/health\/2018\/099-17-e\/index.html\" target=\"_blank\" rel=\"noopener noreferrer\">POR: 099-17: Evaluation of Possible Labelling Elements for Vaping Products \u2013 Phase I and Phase II<\/a><br \/>\n12 in-person focus groups with adults (18+) to develop and assess response to labelling requirements.<\/p>\n<p><a href=\"http:\/\/epe.lac-bac.gc.ca\/100\/200\/301\/pwgsc-tpsgc\/por-ef\/health\/2017\/088-16-e\/index.html\" target=\"_blank\" rel=\"noopener noreferrer\">POR 088-16: Qualitative\/Quantitative Research on E-cigarette Flavours and Risk Perception<\/a><br \/>\n12 in-person focus groups and an on-line survey of 1509 Canadians who were between 15 and 24 years old.<\/p>\n<p><a href=\"http:\/\/epe.lac-bac.gc.ca\/100\/200\/301\/pwgsc-tpsgc\/por-ef\/health\/2018\/047-17-e\/index.html\" target=\"_blank\" rel=\"noopener noreferrer\">POR 047-17: Longitudinal Vaper Panel Survey to Measure Attitudes and Behaviours regarding Vaping Products<\/a><br \/>\nTwo waves of on-line surveys with vapers 15 years of age or older.
