{"id":1419,"date":"2019-09-26T02:58:28","date_gmt":"2019-09-26T02:58:28","guid":{"rendered":"https:\/\/blogs.bmj.com\/tc\/?p=1419"},"modified":"2019-09-26T03:00:41","modified_gmt":"2019-09-26T03:00:41","slug":"world-pmi-funded-foundation-for-a-smoke-free-world-adding-insult-to-injury","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/tc\/2019\/09\/26\/world-pmi-funded-foundation-for-a-smoke-free-world-adding-insult-to-injury\/","title":{"rendered":"World: PMI-funded Foundation for a Smoke-Free World adding insult to injury"},"content":{"rendered":"<p><strong>Jennifer Fang and Kelley Lee<\/strong><\/p>\n<p>Despite 7 million deaths annually and counting, tobacco control worldwide continues to be <a href=\"http:\/\/www.ipsnews.net\/2019\/09\/governments-must-prioritise-sustained-tobacco-control-investment-low-middle-income-nations\/\">seriously underfunded as a public health priority<\/a>. This funding gap is especially acute in low-income countries and among certain historically disadvantaged populations.\u00a0 It is into this space that the Philip Morris International (PMI) financed Foundation for a Smokefree World (FSFW) has stepped in to flash the cash in a search for allies. In 2018, the Foundation provided funds to create the <a href=\"https:\/\/www.newshub.co.nz\/home\/politics\/2019\/08\/smoking-researcher-marewa-glover-s-organisation-funded-by-tobacco-giant-philip-morris.html\">Centre for Research Excellence: Indigenous Sovereignty and Smoking in New Zealand<\/a>. \u00a0Indigenous peoples in settler countries, such as the Maori population in New Zealand, have <a href=\"https:\/\/www.smokefree.org.nz\/smoking-its-effects\/facts-figures\">higher smoking rates<\/a> and, consequently, disproportionate levels of tobacco-related disease and deaths. The appropriateness of accepting tobacco industry funding, to address a problem created and sustained by that same industry, has been <a href=\"https:\/\/www.rnz.co.nz\/news\/in-depth\/397224\/gloves-off-smoking-researcher-shunned-over-philip-morris-funding\">deeply controversial<\/a>.<\/p>\n<p>It is in a similar context that the Foundation has gone knocking in Malawi.\u00a0 The country\u2019s economy is the <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/03056244.2018.1431213\">most tobacco leaf-dependent<\/a> in the world. While the government accepts the need to diversify, <a href=\"https:\/\/untobaccocontrol.org\/impldb\/wp-content\/uploads\/resources\/UN%20agencies%20and%20tobacco%20industry%20interference.pdf\">many development agencies have declined<\/a> to engage with farmers due to their association with the tobacco industry. This presents an opportunity for the <a href=\"https:\/\/www.smokefreeworld.org\/agriculture-livelihoods\">FSFW to step in with funding<\/a> to \u201caddress the agricultural impact of smoking cessation.\u201d \u00a0Its engagement in Malawi <a href=\"https:\/\/www.smokefreeworld.org\/sites\/default\/files\/uploads\/board\/2018-sep_6_-_fsfw_-_minutes_of_a_special_meeting_of_the_board_of_directors.pdf\">includes plans<\/a> to establish a Center for Agricultural Transformation and a \u201csignature\u201d regional conference to \u201cinform a strategy and action plan to drive economic diversification.\u201d In November 2018, one of the authors conducted interviews with policy makers, farmers unions and ex-tobacco industry employees in Malawi to understand the role of China in Malawi\u2019s tobacco industry. We found positive interest in Malawi in the planned centre\u2019s capacity to \u201cassist the country to diversify\u201d. Knowledge of its PMI was not mentioned by interviewees. The centre has the potential to embed FSFW (and through it, PMI) within policy making networks, and create a position of ongoing influence, in the country.<\/p>\n<p>A further strategic target of the FSFW\u2019s largesse is China, where the official annual number of tobacco-related deaths has now <a href=\"http:\/\/www.wpro.who.int\/china\/mediacentre\/factsheets\/tobacco\/en\/\">reached one million<\/a>.\u00a0 We conducted interviews with tobacco control experts in China in July 2016 to understand local perceptions of the tobacco industry\u2019s corporate social responsibility projects and found the longstanding shortfall in tobacco control resources was gravely apparent, with funding largely coming from external organizations..\u00a0 In 2019, the FSFW Board Governance Committee <a href=\"https:\/\/www.smokefreeworld.org\/sites\/default\/files\/uploads\/board\/2019-feb-12_-_fsfw_-_minutes_of_a_special_meeting_of_the_board_of_directors2.pdf\">recommended the appointment<\/a> of \u201ca director with expertise in China.\u201d \u00a0Anticipating challenges in navigating China\u2019s complex policy landscape, a <a href=\"https:\/\/www.smokefreeworld.org\/sites\/default\/files\/uploads\/board\/2019-May-24%20--%20FSFW%20-%20Minutes%20of%20a%20Special%20Meeting%20of%20the%20Board%20of%20Directors.pdf\">US$1.3 million budget was allocated<\/a> to engage APCO Worldwide to \u201cestablish the necessary framework for the Foundation, including governance, infrastructure, staffing and support pieces.\u201d\u00a0 It enhances the industry\u2019s presence in Asia, where the vast majority of the world\u2019s smokers reside. In the absence of institutional public statements against engaging with the PMI-funded Foundation in China, the entry of a controversial funder with deep pockets would be a major public health setback.<\/p>\n<p>The three cases above suggest a pattern to how the FSFW is seeking footholds with underserved populations. Recipients welcome new resources to address longstanding needs.\u00a0 For PMI, this engagement strategy casts its FSFW in hero mode, undermining global tobacco control solidarity against the industry while sustaining its <a href=\"https:\/\/www.pmi.com\/who-we-are\/designing-a-smoke-free-future\">business plan to globally dominate<\/a> both the traditional and alternative nicotine product markets.<\/p>\n<p><em><strong>Jennifer Fang is a Research Fellow at the Faculty of Health Sciences, Simon Fraser University, Canada.
