Calibri\"><span style=\"font-size: small\"> expecting lean times ahead.\u00a0\u00a0 Services for sexually transmitted infections<\/span><\/span> <span style=\"font-family: Calibri\"><span style=\"font-size: small\">(STI) <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">are unlikely to <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">get major billing in party manifestoes<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">, <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">and political support tends to be driven by committed individuals rather than public demand. <\/span><\/span><\/p>\n<p style=\"margin: 0pt 0pt 10pt\"><span style=\"font-family: Calibri\"><span style=\"font-size: small\">T<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">hese are <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">particularly <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">interesting times for England\u2019s National Chlamydia Screening Programme (NCSP)<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">.\u00a0 The programme<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> has been the subject of a report by the National Audit Office, followed by a hearing of <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">Parliament\u2019s <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">Public Accounts Committee<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> before the Christmas break<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">. <\/span><\/span><\/p>\n<p style=\"margin: 0pt 0pt 10pt\"><span style=\"font-family: Calibri\"><span style=\"font-size: small\">The NCSP was <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">announced in 2003, and differed from pilot studies in several respects.\u00a0 Both <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">English <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">pilots<\/span><\/span><span style=\"font-family: Calibri\"><sup><span style=\"font-size: xx-small\">(1,2,3)<\/span><\/sup><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> had achieved<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> high rates of coverage within their<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> single year<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> of operatio<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">n<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">, with general practice <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">a<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> predominant setting,<\/span><\/span> <span style=\"font-family: Calibri\"><span style=\"font-size: small\">and <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">using some form <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">of payment<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> for general practitioners who participated, <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">while<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> only one<\/span><\/span><span style=\"font-family: Calibri\"><sup><span style=\"font-size: xx-small\">(<\/span><\/sup><\/span><span style=\"font-family: Calibri\"><sup><span style=\"font-size: xx-small\">3<\/span><\/sup><\/span><span style=\"font-family: Calibri\"><sup><span style=\"font-size: xx-small\">)<\/span><\/sup><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> had included males in the target group. <\/span><\/span><\/p>\n<p style=\"margin: 0pt 0pt 10pt\"><span style=\"font-family: Calibri\"><span style=\"font-size: small\">During<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> the financial year April 2008<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">-2009 an estimated 15.9% of <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">England\u2019s<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> 6.7M 15-24 year old population had been tested <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">for Chlamydia <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">outside specialist genitourinary medicine clinics \u2013 still far short of the estimated one third which was achieved in the pilots and thought to be needed to achieve a real impact on incidence.\u00a0\u00a0\u00a0 However, <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">whatever happen<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">s<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> now <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">the programme <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">wil<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">l<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> continue to have a major impact on the organisation of sexual health services.\u00a0 The dissemination of testing into family planning (contraception) clinics, other young people\u2019s services and increasingly into general practice <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">has <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">already <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">mainstreamed<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> awareness of STIs<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> among the public and professionals<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">. <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">The next few months will be crucial in defining <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">public <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">policy <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">on<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> the <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">balance and <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">relationship between the NCSP (simple service, high throughput) and specialist STI services <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">(complex and expensive, and <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">focussing on the needs of individuals of higher than average risk behaviour or worse than average luck<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">)<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">. <\/span><\/span><\/p>\n<p style=\"margin: 0pt 0pt 10pt\"><span style=\"font-family: Calibri\"><span style=\"font-size: small\">T<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">he NCSP <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">was<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> criticised by the National Audit Office<\/span><\/span><span style=\"font-family: Calibri\"><sup><span style=\"font-size: xx-small\">(<\/span><\/sup><\/span><span style=\"font-family: Calibri\"><sup><span style=\"font-size: xx-small\">4<\/span><\/sup><\/span> <span style=\"font-family: Calibri\"><sup><span style=\"font-size: xx-small\">)<\/span><\/sup><\/span> <span style=\"font-family: Calibri\"><span style=\"font-size: small\">for <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">multiple and weak branding, disorganised and cost-<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">in<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">efficient commissioning, and highly variable partner notification (and even treatment) rates. <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">The report is definitely worth a read, <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">along with its<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> sister publication \u2013 a report on the NCSP to the Department of Health by Dr Ruth Hussey. <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">The <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">NCSP<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> was impleme<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">nted in a period of increasing<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> devolution of a wide range of <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">healthcare resource <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">decisions to local areas, with pressure applied <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">where needed <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">by <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">blunt instruments<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> such as <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">the<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> \u201cVital Signs Indicator\u201d which last year set a standard <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">of 17% coverage for<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> the NCSP .\u00a0 In this respect, its difficulties\u00a0 are <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">likely to be <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">a wider sign of the times <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">&#8211; <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">as suggested by the Chair<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> of the Public Accounts Committee<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">, who <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">remarked<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> in closing: \u201c<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">What went wrong? You ploughed ahead with local, fragmented implementation, the programme has been inefficient, it has wasted public funds and each programme has been buying its own kit, devising its own marketing and websites.<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">\u201d <\/span><\/span><\/p>\n<p style=\"margin: 0pt 0pt 10pt\"><span style=\"font-family: Calibri\"><span style=\"font-size: small\">Although <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">the <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">Public Accounts Committee<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">\u2019s recommendations are<\/span><\/span> <span style=\"font-family: Calibri\"><span style=\"font-size: small\">not yet published<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">, a flavour of what <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">we can<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> expect can be inferred from a webcast of the hearing at <\/span><\/span><a title=\"Public Accounts Committee Webcast\" href=\"http:\/\/www.parliamentlive.tv\/Main\/Player.aspx?meetingId=5227\" target=\"_blank\"><span style=\"font-family: Calibri;color: #0000ff\"><span style=\"text-decoration: underline\"><span style=\"font-size: small\">http:\/\/www.parliamentlive.tv\/Main\/Player.aspx?meetingId=5227<\/span><\/span><\/span><\/a><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> or, if you prefer the written word, at <\/span><\/span><a title=\"Public Accounts Committee transcript\" href=\"http:\/\/www.publications.parliament.uk\/pa\/cm\/cmpubacc.htm#uncorr\" target=\"_blank\"><span style=\"font-family: Calibri;color: #0000ff\"><span style=\"text-decoration: underline\"><span style=\"font-size: small\">http:\/\/www.publications.parliament.uk\/pa\/cm\/cmpubacc.htm#uncorr<\/span><\/span><\/span><\/a><\/p>\n<p style=\"margin: 0pt 0pt 10pt\"><span style=\"font-family: Calibri\"><span style=\"font-size: small\">A more coherent branding and commissioning of the NCSP will have implications for the branding, and prioritisation, of more specialist STI services. <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">Clinicians <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">and providers will need to think <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">and advocate <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">long and hard <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">for a locally effective<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> the future balance between <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">the<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> NCSP<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> (whatever form it may take)<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">, and the broader picture <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">of services for STIs<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">, including specialist services<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">. <\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\">Who<\/span><\/span><span style=\"font-family: Calibri\"><span style=\"font-size: small\"> and what will they be for, if everyone offers a yearly chlamydia test?
