{"id":136,"date":"2010-09-06T19:40:45","date_gmt":"2010-09-06T19:40:45","guid":{"rendered":"https:\/\/blogs.bmj.com\/sti\/?p=136"},"modified":"2010-09-06T19:50:31","modified_gmt":"2010-09-06T19:50:31","slug":"new-sti-surveillance-data-in-england","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/sti\/2010\/09\/06\/new-sti-surveillance-data-in-england\/","title":{"rendered":"New STI surveillance data in England"},"content":{"rendered":"<p>STI surveillance in England has taken a major step forward with the first publication of STI data based on GUMCAD (Genitourinary Medicine Clinic Activity Dataset). <a href=\"http:\/\/www.hpa.org.uk\/web\/HPAweb&amp;Page&amp;HPAwebAutoListName\/Page\/1201094610372#Trends_in_STI_diagnoses\">\u00a0 For the first time, the genitourinary medicine(GUM) \u00a0clinic data are based on a disaggregate dataset, using pseudonymised individual level data, rather than an aggregate summary.\u00a0 As a consequence, we now have the ability at local, regional and national level to interrogate the dataset for rates of co-infection, re-infection rates and attendance data.\u00a0 While this dataset is still confined to the specialist clinics, it is a huge step forward in surveillance.\u00a0 It remains to be seen how we will develop surveillance spanning the primary care\/specialist interface.\u00a0 Disaggregate datasets in primary care (such as the General Practice Research Database) have been around for a long time, and have been used to explore STI and HIV diagnoses and management.\u00a0 Although they have been linked to hospital and cancer registry data, a link to specialist GUM services is unlikely to happen in the foreseeable future, so we will continue to rely on a complex admixture of GUMCAD, chlamydia screening, laboratory and <em>ad hoc <\/em>datasets.\u00a0 Compared to many other countries, this is pretty good \u2013 but a real assessment of what is delivered by our National Health Service will require a bit more \u201cjoining the dots\u201d.<\/p>\n<p>Inevitably, diagnoses of chlamydia have increased with the National Chlamydia Screening Programme.\u00a0 \u00a0\u00a0This is a success story, indicating increased reach of services.\u00a0 However, the publication of data by primary care trust of residence presents traps for the unwary.\u00a0 Chlamydia diagnoses really do relate to increased testing, are measurable and positivity rates are thought to have limited variation between populations of a given age.\u00a0 However it is much harder to determine the \u201creach\u201d of the testing needed for higher risk individuals \u2013 men who have sex with men, who also need to be proactively offered testing for HIV, hepatitis B, syphilis and gonorrhoea; and urban ethnically mixed populations.<\/p>\n<p>Interestingly, epidemiological treatment of suspected gonorrhoea has risen over the past decade \u2013 suggesting either better documentation ofpartner notification, or truly improved partner notifications.\u00a0 Given that the rate of partner change needed to maintain gonorrhoea in a<a title=\"HPA STI data 2010\" href=\"http:\/\/www.hpa.org.uk\/web\/HPAweb&amp;Page&amp;HPAwebAutoListName\/Page\/1201094610372#Trends_in_STI_diagnoses\" target=\"_blank\"> <\/a>population with developed services and effective antibiotics, this is an important finding.\u00a0 It may be part of the explanation for declining gonorrhoea diagnoses, since a 2002 peak.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>STI surveillance in England has taken a major step forward with the first publication of STI data based on GUMCAD (Genitourinary Medicine Clinic Activity Dataset). \u00a0 For the first time, the genitourinary medicine(GUM) \u00a0clinic data are based on a disaggregate dataset, using pseudonymised individual level data, rather than an aggregate summary.\u00a0 As a consequence, we [&#8230;]<\/p>\n<p><a class=\"btn btn-secondary understrap-read-more-link\" href=\"https:\/\/blogs.bmj.com\/sti\/2010\/09\/06\/new-sti-surveillance-data-in-england\/\">Read More&#8230;<\/a><\/p>\n","protected":false},"author":79,"featured_media":0,"comment_status":"open","ping_status":"closed","sticky":false,"template":"","format":"standard","meta":{"footnotes":""},"categories":[1694,1],"tags":[1756],"class_list":["post-136","post","type-post","status-publish","format-standard","hentry","category-sti-prevention-and-surveillance","category-uncategorized","tag-sti-surveillance"],"yoast_head":"<!-- This site is optimized with the Yoast SEO plugin v27.4 -
