{"id":1318,"date":"2017-08-02T17:00:29","date_gmt":"2017-08-02T16:00:29","guid":{"rendered":"https:\/\/blogs.bmj.com\/sti\/?p=1318"},"modified":"2017-11-01T15:22:32","modified_gmt":"2017-11-01T15:22:32","slug":"why-sti-induced-infertility-can-also-be-a-male-problem","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/sti\/2017\/08\/02\/why-sti-induced-infertility-can-also-be-a-male-problem\/","title":{"rendered":"Why STI-induced infertility can also be a male problem."},"content":{"rendered":"<p>Among long-term <em>sequelae<\/em> of STI that are asymptomatic and may consequently remain untreated are a number that affect men.\u00a0 The condition of Chronic Pelvic Pain Syndrome (CPPS) has recently been discussed by <a href=\"http:\/\/sti.bmj.com\/content\/90\/7\/572\">Kenyon &amp; Horner (STI)<\/a>, along with the feasibility of its treatment in the STI clinic (<a href=\"http:\/\/sti.bmj.com\/content\/90\/5\/370\">Crofts &amp; Horner (STIs)<\/a>; <a href=\"http:\/\/sti.bmj.com\/content\/90\/5\/373\">Crofts &amp; Horner (II) (STIs)<\/a>).\u00a0 But another important, and much dreaded, consequence of untreated STI in heterosexual men is infertility or sub-fertility.\u00a0 It should not be forgotten that this is potentially a <em>male <\/em>problem \u2013 not just in the sense that it affects men, but because, in many cases, it is STI-induced damage to the genital tract of the <em>male<\/em> rather than the female partner that gives rise to the condition.<\/p>\n<p>This male aspect of STI-induced infertility constitutes the topic of a recent review paper by <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5470348\/\">Schuppe &amp; Weidner<\/a> (S&amp;W). \u00a0A key finding cited (<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/18336456\">Schuppe &amp; Hardt<\/a>), apparently originating from work by same team of authors, is that a remarkable 25% of testicular biopsies obtained from infertile men reveal focal inflammatory reactions.\u00a0 Of these, only 2% reported any previous episode of orchitis.\u00a0 Yet, when it comes to determining the <em>pathogens responsible<\/em> for male infertility (which could be uropathogens as well as STI), current evidence allows us to draw few conclusions.\u00a0 The same is true regarding the mechanisms by which these pathogens have an impact on male fertility.\u00a0 Finally, it is not possible to \u00a0establish with any certainty the proportion of total cases of male infertility for which STI are responsible. Of course, where acute epididymitis is involved, there is evidence for azoospermia (c.10%) or oligospermia (c.30%). \u00a0Here the authors are reassuring as to the non-persistence of these effects (<a href=\"http:\/\/www.ejradiology.com\/article\/S0720-048X(13)00476-2\/pdf\">Pilatz &amp; Linn<\/a>). But where cases of STI are asymptomatic, they do not, of course, figure in information for \u2018accessory gland infection\u2019 as defined by WHO criteria.<\/p>\n<p>Regarding the pathogens implicated, the investigations of the role of Mycoplasma genitalium (see <a href=\"http:\/\/sti.bmj.com\/content\/91\/8\/589\">Idahl &amp; Fredland (STI)<\/a>) and HPV viruses have produced conflicting results; Trichomas vaginalis has no significant effect.\u00a0 As regards the mechanism, S&amp;W consider evidence relating to reduced sperm motility, oxidative damage and immune-mediated infertility, but reach no very firm conclusions.
