HPV vaccination loses the battle for public support: calculating the health implications for Japan"},"content":{"rendered":"<p>A recent brief contribution to <em>The Lancet-Oncology<\/em> (<a href=\"http:\/\/www.thelancet.com\/journals\/lanonc\/article\/PIIS1470-2045(16)00147-9\/abstract\">Tanaka &amp; Ueda<\/a>) uses predictions of the probable health outcomes of the suspension of the Japanese HPV vaccination programme to make the case for an urgent reassessment of the current policy.\u00a0 This intervention is very timely.\u00a0\u00a0The approved age for HPV vaccination\u00a0for Japanese girls\u00a0is a window of four years from 13-16 yrs, and the current year (2016-17) constitutes the fourth since the suspension of the programme (June 2013); so the current year is the last opportunity for a return to the initial HPV vaccination policy before the effects of the suspension (for the oldest in the cohort) become irrevocable.\u00a0\u00a0 From the following year on (2017-8), the authors argue, every additional year of suspension will exclude an additional age group from the protective effects of HPV vaccination \u2013 unless, that is, the eligibility period is extended to include older girls).\u00a0 With a view to maximizing the impact of this message, they\u00a0assess the impact of restarting vaccination in 2020 as against restarting in the current year.\u00a0 They do this on two scenarios depending on whether or not vaccination, when resumed, is given to those who would have missed out in 2013-2017 as well as to those currently eligible.<\/p>\n<p>So how great are these effects?\u00a0\u00a0 On the first scenario (no catch-up for missed years), risk of HPV 16\/18 infection at 20yrs is estimated for the 2013-6 year groups at around a steady <strong>1.0%<\/strong>, given resumption of vaccination 2020, as against the <strong>0.3-0.4%<\/strong> risk, with resumption in 2016; on the second, that steady 1% risk over the four missed years is replaced by an evenly paced decline from 1.0% to previous levels (0.3-0.4%) over the four-year period.<\/p>\n<p>Of course, it is the long-term health impact of these HPV infections\u00a0that constitutes the cost of the current Japanese policy, and, were it recognized,\u00a0the strongest incentive for a resumption of vaccination.\u00a0 Unfortunately, the full impact\u00a0is very long term, and\u00a0hard to\u00a0quantify.\u00a0 But some advanced indication of\u00a0its potential scale\u00a0is\u00a0provided by the recent Finish \u2018FUTURE\u2019 trial that demonstrated an absence of CIN3 and ICC lesions in vaccinated participants (<a href=\"http:\/\/sti.bmj.com\/content\/89\/Suppl_1\/A53.2.abstract?sid=6185db9e-6032-49f4-be19-49d80c8004f3\">Paavonen\/3\/STIs<\/a>), as well as, more indirectly, the enormous (c.90%) declines in genital wart presentations in Australia (<a href=\"http:\/\/sti.bmj.com\/content\/91\/3\/214.abstract?sid=bc1298b4-6963-42f4-8186-547874806e0d\">Chow &amp; Fairley\/STIs<\/a>; <a href=\"http:\/\/sti.bmj.com\/content\/91\/Suppl_2\/A78.1.abstract?sid=e784672f-9f55-47f7-986f-41a014de9089\">Ali &amp; Donovan (STIs)<\/a>) and New\u00a0 Zealand (<a href=\"http:\/\/sti.bmj.com\/content\/90\/2\/103.extract?sid=e784672f-9f55-47f7-986f-41a014de9089\">Wilson &amp; Baker\/STIs<\/a>).\u00a0 The benefits foregone by the unvaccinated will also include protection against head and neck \u2013 especially oropharyngeal \u2013 cancers (<a href=\"http:\/\/sti.bmj.com\/content\/early\/2015\/04\/21\/sextrans-2014-051808.full\">Field &amp; Lechner\/STIs<\/a>; <a href=\"http:\/\/sti.bmj.com\/content\/91\/4\/284.abstract?sid=d6ba6b43-b28d-4a9f-97b7-923219c53dda\">King &amp; Sonnenberg\/STIs<\/a>), and against a small, but significant range of anogenital cancers in women (<a href=\"https:\/\/blogs.bmj.com\/sti\/2016\/10\/14\/prevention-of-anogenital-cancers-in-women-may-be-an-additional-benefit-of-hpv-vaccination\/\">Prevention of anogenital cancers in women\/STIs\/blogs<\/a>).\u00a0 On a more positive note, however, there is some evidence for the benefit of \u2018catch-up\u2019 and incomplete vaccination (<a href=\"https:\/\/blogs.bmj.com\/sti\/category\/hpv\/\">&#8220;Catch-up&#8221; and incomplete vaccination\/STIs\/blogs<\/a>) as against those who have hitherto put this in doubt (<a href=\"http:\/\/sti.bmj.com\/content\/87\/Suppl_1\/A62.1.abstract?sid=be51499e-7a80-4d7a-80ee-8318463934ac\">STs\/Chesson &amp; Markowitz<\/a>).<\/p>\n<p>An important lesson of the Japanese experience for everyone concerned with HPV is the importance of public education.\u00a0 The very poor levels of understanding revealed by a recent systematic review (<a href=\"http:\/\/sti.bmj.com\/content\/92\/6\/474.abstract?sid=612ddc92-79e9-46ca-8592-e1e923bb7229\">Patel &amp; Moss\/STIs<\/a>) amongst European adolescents is a timely warning that uninformed
