{"id":1200,"date":"2016-07-18T17:24:16","date_gmt":"2016-07-18T16:24:16","guid":{"rendered":"https:\/\/blogs.bmj.com\/sti\/?p=1200"},"modified":"2016-07-18T17:24:16","modified_gmt":"2016-07-18T16:24:16","slug":"uk-national-health-service-nhs-kicks-prep-into-the-long-grass","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/sti\/2016\/07\/18\/uk-national-health-service-nhs-kicks-prep-into-the-long-grass\/","title":{"rendered":"UK National Health Service (NHS) kicks PrEP into the long grass"},"content":{"rendered":"<p>A recent\u00a0<a href=\"http:\/\/www.bmj.com\/content\/bmj\/354\/bmj.i3515.full.pdf\">BMJ editorial<\/a> condemns the NHS position that it will not consider PrEP for direct NHS funding.\u00a0 The decision was first communicated in an <a href=\"https:\/\/www.england.nhs.uk\/2016\/03\/prep\/\">NHS statement<\/a> issued in March, then confirmed by a <a href=\"https:\/\/www.england.nhs.uk\/2016\/05\/prep-provision\/23\/\">review<\/a> on 31<sup>st<\/sup> May, following reconsideration in response to objections raised by interested groups.\u00a0 This brought to an end an eighteen-month process of discussion between Department of Health, doctors and patients groups, Public Health England and other stakeholders.\u00a0 The NHS decision is currently under judicial review \u2013 which no doubt explains the timing of the BMJ editorial.<\/p>\n<p><a href=\"http:\/\/www.nat.org.uk\/Media-and-Blog\/Blog\/Prevention\/PrEP_further_delay_NICE_evidence_review.aspx\">Advocates of PrEP<\/a> argue that the NHS has powers under the Health and Social Care Act (2012) directly to commission services \u2018prioritized for investment\u2019, and that PrEP should qualify for consideration on this basis.\u00a0 But the <a href=\"https:\/\/www.england.nhs.uk\/2016\/05\/prep-provision\/23\/\">conclusion of the NHS<\/a> is that PrEP does not qualify to be so considered because the <a href=\"https:\/\/www.gov.uk\/guidance\/commissioning-regional-and-local-sexual-health-services\">Local Authorities Regulations (2013)<\/a> clearly stipulate that commissioning for sexual health prevention is legally the responsibility of local government.<\/p>\n<p>Technically, then, the argument turns on whether the NHS can commission for PrEP directly, given that PrEP is a form of sexual health prevention. Needless to say, in the eyes of the <a href=\"http:\/\/www.nat.org.uk\/Media-and-Blog\/Blog\/Prevention\/PrEP_further_delay_NICE_evidence_review.aspx\">advocates of PrEP<\/a>, this is a mere technicality invoked by the NHS in order to shirk its responsibilities.\u00a0 The NHS statements also include the proposal to work with local government authorities on exploring how they should go about commissioning PrEP services most effectively, and to dedicate \u00a32m to the establishment of local pilots. \u00a0The problem here is that local authorities, having lately had \u00a3200m shaved off their funding for sexual health services, are presumably not in a hurry to pick up the tab.<\/p>\n<p>So what about the case for PrEP? As regards effectiveness for high-risk MSM populations, PrEP has emerged with flying colours from recent trials, including the UK PROUD\u00a0 study (<a href=\"https:\/\/blogs.bmj.com\/sti\/2015\/10\/22\/prep-highly-effective-against-hiv-in-msm-and-has-limited-impact-on-risk-compensation\/?preview_id=1121&amp;preview_nonce=f28b246a81&amp;preview=true\">PrEP Highly Effective\/STIs\/blog<\/a>; <a href=\"https:\/\/blogs.bmj.com\/sti\/2015\/03\/25\/reported-86-effectiveness-for-msm-prep-by-proud-study-makes-this-intervention-a-viable-option-for-uk-health-services\/\">PROUD\/STIs\/blog<\/a>) and trials with similar populations undertaken in France (<a href=\"http:\/\/i-base.info\/ipergay-prep-study-shows-early-efficacy-in-protecting-gay-men-from-hiv\/\">IPERGAY<\/a>) and California (<a href=\"http:\/\/cid.oxfordjournals.org\/content\/early\/2015\/09\/24\/cid.civ778.abstract\">Volk &amp; Hare<\/a>).\u00a0 Cost effectiveness, however, is another matter \u2013 and here PrEP has failed to make the grade.\u00a0 Recent studies have shown that in the UK PrEP is not even borderline cost-effective without substantial reductions in the cost of the drugs (<a href=\"http:\/\/sti.bmj.com\/content\/91\/Suppl_1\/A1.1.abstract?sid=1d28d81c-bd13-4e04-9b42-abdd42a6bf78\">Cambiano &amp; Phillips\/STIs<\/a>; <a href=\"https:\/\/blogs.bmj.com\/sti\/2015\/10\/22\/prep-highly-effective-against-hiv-in-msm-and-has-limited-impact-on-risk-compensation\/?preview_id=1121&amp;preview_nonce=f28b246a81&amp;preview=true\">PrEP Highly Effective\/STIs\/blog<\/a>; <a href=\"http:\/\/www.aidsmap.com\/Second-UK-cost-effectiveness-study-finds-that-large-PrEP-programmes-may-need-drug-price-cut-to-be-affordable\/page\/3000951\/\">PrEP cost effectiveness study<\/a>).
