{"id":1018,"date":"2014-08-20T20:12:21","date_gmt":"2014-08-20T20:12:21","guid":{"rendered":"https:\/\/blogs.bmj.com\/sti\/?p=1018"},"modified":"2014-08-17T20:12:43","modified_gmt":"2014-08-17T20:12:43","slug":"unaids-gap-report-the-beginning-of-the-end-of-the-aids-epidemic","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/sti\/2014\/08\/20\/unaids-gap-report-the-beginning-of-the-end-of-the-aids-epidemic\/","title":{"rendered":"UNAIDS Gap Report: \u201cThe beginning of the end of the AIDS epidemic"},"content":{"rendered":"<p>In advance of the 20<sup>th<\/sup> International AIDS Conference in Melbourne 20<sup>th<\/sup>-24<sup>th<\/sup> July, UNAIDS has published its Report entitled <a href=\"http:\/\/www.unaids.org\/en\/media\/unaids\/contentassets\/documents\/unaidspublication\/2014\/UNAIDS_Gap_report_en.pdf\">The Gap<\/a>.\u00a0 This offers a panoramic survey of progress and challenges to date, graphically presented.<\/p>\n<p>The general sense of a tide having at last turned in the global battle against HIV is borne out by the Report\u2019s account of the increased concentration of the epidemic on certain key fronts where the struggle will henceforth need to be carried on.\u00a0 Overall, it points to a global decline &#8211; and a swiftly accelerating decline &#8211; in HIV incidence and AIDS related mortality globally.\u00a0 New infections have fallen <strong>38%<\/strong> from 2001, <strong>13%<\/strong> over the last three years; mortality has declined <strong>35%<\/strong> since 2005, <strong>19%<\/strong> over the last three years. \u00a0The Middle East and North Africa (see STIs\/blog; \u00a0<a href=\"http:\/\/sti.bmj.com\/content\/89\/Suppl_3\/iii5.abstract?sid=b5eddd0f-e5c4-4b2c-8c26-b78dd2be664a\">STIs\/Saba &amp; Abu-Raddad<\/a>; <a href=\"http:\/\/sti.bmj.com\/content\/89\/Suppl_3\/iii1.extract?sid=b5eddd0f-e5c4-4b2c-8c26-b78dd2be664a\">Abu-Raddad &amp; Riedner<\/a>), and Eastern Europe and Russia constitute the sole exceptions to this pattern (with increases in mortality of 66%, and 5% respectively).<\/p>\n<p>The \u201cflip-side\u201d is an increasingly visible concentration of the epidemic.\u00a0 Fifteen countries (with Nigeria and S. Africa generally topping the list) account for <strong>75%<\/strong> of the global HIV burden, <strong>76%<\/strong> of last year\u2019s (2013) new infections, and <strong>74%<\/strong> of last year\u2019s AIDS-related mortality.\u00a0 In view of this, it is no surprise that the major part of the Gap report consists in a series of profiles: first, of the principal world regions, and second key populations (prisoners, transgender etc.).\u00a0 These recurrently pose the question: \u201c<strong>Why am I being left behind<\/strong>?\u201d<\/p>\n<p>Three aspects of the situation as described by the report strike this reader as particularly telling:<\/p>\n<p>&#8211;\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 the proportion of those living with HIV who are still not accessing ART: three in every five.\u00a0 In Nigeria <strong>80%<\/strong> have no access to treatment.<\/p>\n<p>&#8211;\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 the vulnerability of adult women in sub-Saharan Africa, who alone account for <strong>80%<\/strong> of the 16 million women aged 15 yrs and older who live with HIV.<\/p>\n<p>&#8211;\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 the appallingly low proportion of children living with HIV who receive ART: a mere <strong>24%<\/strong>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>In advance of the 20th International AIDS Conference in Melbourne 20th-24th July, UNAIDS has published its Report entitled The Gap.\u00a0 This offers a panoramic survey of progress and challenges to date, graphically presented.
