Photo by Phil Mynot<\/small>t<\/figcaption><\/figure>\n<p>When you hear the phrase \u201cdiversity in science\u201d, you typically think about the efforts to create a diverse and inclusive scientific workforce.\u00a0 However, it is also about the research we conduct.\u00a0 For over twenty years [1] <a href=\"https:\/\/doi.org\/10.1186\/s12905-015-0251-9\">papers have been raising an issue that we have a sex bias as the research community<\/a> predominantly study only one sex; <a href=\"10.1016\/j.neubiorev.2010.07.002\">typically males<\/a>. This bias is apparent even when the disease of interest is a <a href=\"https:\/\/doi.org\/10.1016\/j.surg.2014.07.001\">female prevalent disorder<\/a>. Talking to a lay person from outside of science, this situation seems ludicrous.<\/p>\n<p><strong>Is it justifiable to study one sex and generalize? <\/strong><\/p>\n<p>Multiple funding agencies, journals, and professional bodies have initiated programs to encourage us to include both sexes.\u00a0 In 2014, the US <a href=\"https:\/\/www.nih.gov\/\">National Institute Health<\/a>, took the unusual step to mandate change and include both sexes in <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5101948\/\">all experiments they fund<\/a> bar a few circumstances. \u00a0They feel there is sufficient evidence to force change.<\/p>\n<p>In a nut-shell, this is because we now know that women and men may differ in the prevalence, progression and severity of a disease and during treatment, can experience significant differences in the side effects and efficacy. \u00a0Studying only one sex means we are only representing half the population. We talk of personalized medicine being the future, I would argue that considering sex is an easy win to realize this ambition and significantly improve the healthcare provided.<\/p>\n<p><strong>Why do we often study one sex?<\/strong><\/p>\n<p>We have to make hard choices when we design experiments.\u00a0 The scientific process relies on us simplifying a complex world to generate a testing space where we can isolate cause and effect and then we generalize the results. To date, we have assumed sex isn\u2019t a big deal.<\/p>\n<p>The 3R (Replace, Reduce and Refine) framework, which guides our actions, has also encouraged this position. The historic definition of the reduce element has focused on minimizing the absolute number of animals within an experiment and thus encouraging us to study in a narrow testing space and to generalize the results.<\/p>\n<p>So, why males?\u00a0 It was believed that estrogen cycle resulted in female animals being more variable and therefore to meet our ethical obligation and use as few animals as possible we should use males.\u00a0 A <a href=\"https:\/\/doi.org\/10.1016\/j.neubiorev.2014.01.001\">meta-analysis has highlighted this is a flawed belief<\/a>, but the legacy remains.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-101 alignleft\" src=\"https:\/\/blogs.bmj.com\/openscience\/files\/2019\/03\/shutterstock_691546423.jpg\" alt=\"\" width=\"884\" height=\"489\" \/><\/p>\n<p>&nbsp;<\/p>\n<p><strong>Practically, can we study both sexes? <\/strong><\/p>\n<p>\u201cIt is prohibitively expensive; it would double the sample size!\u201d is a common response. This is a misconception. It is recommended that you maintain your original design but <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4535645\/\">split the animals for a treatment between the two sexes<\/a>. The sensitivity to see the treatment effect is maintained because you now have a factorial design and the treatment effect is <a href=\"https:\/\/bpspubs.onlinelibrary.wiley.com\/doi\/full\/10.1111\/bph.14539\">estimated from both sexes simultaneously<\/a>. We are not increasing the sample size because we are not powering the experiment to assess whether the treatment effect depends on sex; but, running both sexes will allow us to see a large difference if it exists.<\/p>\n<p>However, we should not underestimate the challenges of switching to studying both sexes.\u00a0 Experiments with living animals are more complex, ensuring the welfare of the animals are met and negative effects are minimized.\u00a0 Working with two sexes will further add to this complexity. For example, rodents rely heavily on scent marking to communicate.\u00a0 Exposing a rodent \u00a0to <a href=\"https:\/\/nc3rs.org.uk\/sites\/default\/files\/documents\/NC3RsarticleJaneHurst%20making%20sense%20of%20scents.pdf\">scents from unfamiliar animals or the opposite sex<\/a> can lead to anxiety in the animal or changes in the experiment measure.\u00a0 Different labs approach this challenge in different ways, some through cleaning whilst others accept the complex scent environment. We need to share our experiences and the strategies we develop so that we can learn how to manage this new paradigm from each other.<\/p>\n<p>Studying both sexes also makes the analysis more complex. \u00a0This might explain why scientists often pool data when they have used two sexes.\u00a0 This could reflect a skill gap in statistical analysis. We can address this by seeking out expertise, on-going training, and publishing analysis examples.<\/p>\n<p><strong>Driving change <\/strong><\/p>\n<p>For twenty years this issue has been discussed, yet our standard research practices have perpetuated the bias.\u00a0 Lack of awareness of sex as a biological variable is embedded in our research culture.\u00a0 <a href=\"https:\/\/doi.org\/10.1111\/bph.14539\">We don\u2019t always report the sex, we often only collect data on one sex and we frequently ignore sex in the analysis when we have collected both sexes<\/a>. \u00a0Ignoring sex as a biological variable is our cultural norm. \u00a0It is the way it has always been done.<\/p>\n<p>Change is hard. \u00a0For this to change, within our teams, institutes and areas of expertise, we need leadership. We need to build coalitions that build a vision and provide impetus. These coalitions can identify and implement strategies to strengthen the driving forces for change and then consult to identify and find solutions to the blockers that are resisting the change and maintaining this status quo.\u00a0 The issues are multifaceted and the solutions to support this change need to be bespoke for your community.
