The \u2018correct\u2019 answers were determined by six professors of medical ethics who were also medical doctors.<\/p>\n<p>To give a flavour of the test, I included an example of a scenario which was ultimately discarded from the final version:<\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Insurance-question.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-4671\" src=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Insurance-question-300x160.png\" alt=\"\" width=\"544\" height=\"290\" srcset=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Insurance-question-300x160.png 300w, https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Insurance-question-768x410.png 768w, https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Insurance-question-640x342.png 640w, https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Insurance-question.png 903w\" sizes=\"auto, (max-width: 544px) 100vw, 544px\" \/><\/a><\/p>\n<p>The best answer, according to the panel of learned professors, is 3 and the worst is 1.<\/p>\n<p>As there are two answers per scenario, the maximum score in the honesty test is 44.<\/p>\n<p><strong>Previous testing<\/strong><\/p>\n<p>In July 2023, I administered the test to 19 medico-legal advisers and paralegals at a medical defence organisation. The median score was 40 and the mean was 39.4.<\/p>\n<p>In November 2023, 139 4<sup>th<\/sup> year medical students at the University of Birmingham took the test under supervision. The median score was 41 and the mean 39.8.<\/p>\n<p>Since then, hundreds of clinicians on my remedial ethics course have taken the test. Most score between 39 and 42.<\/p>\n<p><strong>Testing ChatGPT<\/strong><\/p>\n<p>On 22<sup>nd<\/sup> May 2025, I asked ChatGPT, version o3, to complete the test, using the prompt below:<\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Prompt.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-4672\" src=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Prompt-300x37.png\" alt=\"\" width=\"527\" height=\"65\" srcset=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Prompt-300x37.png 300w, https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Prompt.png 602w\" sizes=\"auto, (max-width: 527px) 100vw, 527px\" \/><\/a><\/p>\n<p><em><u>ChatGPT is a principlist, with a twist<\/u><\/em><\/p>\n<p>The first surprise was that ChatGPT purportedly used Beauchamp and Childress\u2019 four principles of medical ethics. It made the common error of calling one of the principles \u2018autonomy\u2019 rather than \u2018respect for autonomy\u2019.<\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Approach.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-4673\" src=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Approach-300x62.png\" alt=\"\" width=\"518\" height=\"107\" srcset=\"https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Approach-300x62.png 300w, https:\/\/blogs.bmj.com\/medical-ethics\/files\/2025\/05\/Approach.png 602w\" sizes=\"auto, (max-width: 518px) 100vw, 518px\" \/><\/a><\/p>\n<p>ChatGPT appeared to supplement a principlist analysis by reference to other values such as integrity, professionalism and prudence.<\/p>\n<p>&nbsp;<\/p>\n<p><em><u>The score<\/u><\/em><\/p>\n<p>The second surprise was its test result: 43\/44. This exceeds the vast majority of medical students, medico-legal advisers and clinicians who have taken the test to date.<\/p>\n<p>In its reasoning, ChatGPT referenced:<\/p>\n<ul>\n<li>Ethical notions, such as integrity, truthfulness and consent<\/li>\n<li>Legal concepts, such as theft, fraud and duty of candour<\/li>\n<li>Regulatory guidance of the General Medical Council<\/li>\n<li>Expectations set by COPE (Committee on Publication Ethics)<\/li>\n<li>Guidelines from the International Committee of Medical Journal Editors (ICMJE)<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><em><u>The error<\/u><\/em><\/p>\n<p>ChatGPT\u2019s sole error resulted from a failure to recognise the mandatory nature of a requirement.<\/p>\n<p>When I pointed this out to ChatGPT, it responded \u2018<em>Thank you for pushing on the nuance here\u2019<\/em> and reached the right answer.<\/p>\n<p><strong>An example of ChatGPT\u2019s ethical reasoning<\/strong><\/p>\n<p>To preserve the validity of the honesty test, I am unable to disclose its scenarios or ChatGPT\u2019s comments on them. However, I have asked ChatGPT to answer the insurance scenario above, using the same prompt as before. The full response, which appears below, sheds light on ChatGPT\u2019s methodical approach to such ethical problems.<\/p>\n<p>&nbsp;<\/p>\n<p><em>\u2018<strong>Best answer \u2192 Option 3: \u201cDo not treat the patient and recommend suitable colleagues who could perform the procedure.\u201d<\/strong><\/em><\/p>\n<table>\n<thead>\n<tr>\n<td><strong><em>Why this is the most ethical course<\/em><\/strong><\/td>\n<td><strong><em>Key principles it satisfies<\/em><\/strong><\/td>\n<\/tr>\n<\/thead>\n<tbody>\n<tr>\n<td><strong><em>Meets statutory &amp; professional duties.<\/em><\/strong><em> The General Medical Council (GMC) requires every doctor who practises in the UK to have \u201cadequate and appropriate insurance or indemnity\u201d in place at all times; practising without it can trigger fitness-to-practise action and loss of licence. (<\/em><a href=\"https:\/\/www.gmc-uk.org\/registration-and-licensing\/managing-your-registration\/information-for-doctors-on-the-register\/insurance-indemnity-and-medico-legal-support?utm_source=chatgpt.com\"><em>GMC UK<\/em><\/a><em>, <\/em><a href=\"https:\/\/www.themdu.com\/guidance-and-advice\/guides\/gmc-guidance-on-professional-indemnity?utm_source=chatgpt.com\"><em>The MDU<\/em><\/a><em>, <\/em><a href=\"https:\/\/www.bmj.com\/content\/349\/bmj.g6296?utm_source=chatgpt.com\"><em>BMJ<\/em><\/a><em>)<\/em><\/td>\n<td><em>Professional integrity, Justice<\/em><\/td>\n<\/tr>\n<tr>\n<td><strong><em>Protects the patient\u2019s right to compensation.<\/em><\/strong><em> If something goes wrong, the patient retains a clear route to redress because the colleague you refer to will (or should) be properly indemnified.<\/em><\/td>\n<td><em>Non-maleficence, Justice<\/em><\/td>\n<\/tr>\n<tr>\n<td><strong><em>Maintains timely care.<\/em><\/strong><em> A prompt referral honours the patient\u2019s clinically semi-urgent need while avoiding unlawful practice on your part.<\/em><\/td>\n<td><em>Beneficence<\/em><\/td>\n<\/tr>\n<tr>\n<td><strong><em>Is transparent and honest.<\/em><\/strong><em> You openly acknowledge your current limitation and arrange an alternative rather than obscure or down-play it.<\/em><\/td>\n<td><em>Autonomy (truth-telling)<\/em><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><strong><em>Worst answer \u2192 Option 1: \u201cOperate on the patient without insurance.\u201d<\/em><\/strong><\/p>\n<table>\n<thead>\n<tr>\n<td><strong><em>Why this is the poorest course<\/em><\/strong><\/td>\n<td><strong><em>Key principles it breaches<\/em><\/strong><\/td>\n<\/tr>\n<\/thead>\n<tbody>\n<tr>\n<td><strong><em>Contravenes GMC rules &amp; the law.<\/em><\/strong><em> Performing private work with no indemnity flagrantly violates GMC Good Medical Practice and statutory obligations. (<\/em><a href=\"https:\/\/www.bmj.com\/content\/349\/bmj.g6296?utm_source=chatgpt.com\"><em>BMJ<\/em><\/a><em>)<\/em><\/td>\n<td><em>Prudence (self-care), Professional integrity<\/em><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><strong><em>Brief comments on the remaining options<\/em><\/strong><\/p>\n<ul>\n<li><strong><em>Option 2 \u2013 Operate without insurance but \u201cexercise particular caution.\u201d<\/em><\/strong><em><br \/>\nTrying to be extra-careful does not cure the core problem: the act remains professionally unlawful and leaves the patient uncompensated. Ethically it is marginally better than Option 1 because at least you show concern for safety, but it is still unacceptable.<\/em><\/li>\n<li><strong><em>Option 4 \u2013 Tell the patient you are \u201cfinalising insurance arrangements\u201d and proceed if they consent.<\/em><\/strong><em><br \/>\nTransparency is an improvement, yet the practice is still prohibited; a patient cannot waive your statutory duty to hold indemnity, and the wording risks misleading them into thinking cover is imminent or already partly in force.<\/em><\/li>\n<\/ul>\n<p><em>\u00a0<\/em><\/p>\n<p><em>Hence, Option 3 is the only answer that simultaneously respects the patient\u2019s welfare and autonomy <strong>and<\/strong> keeps you within professional and legal boundaries, while Option 1 is the clearest violation of both<\/em>.\u2019<\/p>\n<p><u>\u00a0<\/u><\/p>\n<p><strong>Conclusion<\/strong><\/p>\n<p>ChatGPT, version o3, performed impressively &#8211; though not perfectly &#8211; in a situational judgement test focussed on medical ethics. Its answers aligned closely with those of experienced medical ethicists and its reasons were generally sound.<\/p>\n<p>It outperformed most 4<sup>th<\/sup> year medical students and doctors on my remedial ethics course, as well as a small group of medico-legal advisers.<\/p>\n<p>Doctors facing ethical problems should seek advice from friends, colleagues, ethicists or an ethics committee. However, ChatGPT is now a credible ethics adviser, with the added advantages of constant availability and infinite patience. Doctors with a smartphone and internet connection can have their own, on-call medical ethicist at hand.<\/p>\n<p>ChatGPT is nonetheless fallible and, for important matters, a second opinion from an old-fashioned human is advisable.<\/p>\n<p>&nbsp;<\/p>\n<p><em>Daniel Sokol is a medical ethicist and barrister.
