{"id":4040,"date":"2020-11-23T17:07:54","date_gmt":"2020-11-23T16:07:54","guid":{"rendered":"https:\/\/blogs.bmj.com\/medical-ethics\/?p=4040"},"modified":"2020-11-24T13:02:24","modified_gmt":"2020-11-24T12:02:24","slug":"the-covid-19-vaccine-informed-consent-and-the-recruitment-of-volunteers","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/medical-ethics\/2020\/11\/23\/the-covid-19-vaccine-informed-consent-and-the-recruitment-of-volunteers\/","title":{"rendered":"The COVID-19 vaccine, informed consent and the recruitment of volunteers"},"content":{"rendered":"<p>By Jennifer O\u2019Neill.<\/p>\n<p>Last week, in an announcement which offered hope in a time of growing despondence, Pfizer declared that their COVID-19 vaccine had <a href=\"https:\/\/www.theguardian.com\/world\/2020\/nov\/12\/scientist-behind-biontech-pfizer-coronavirus-vaccine-says-it-can-end-pandemic\"><em>\u201coutperformed expectations in the crucial phase 3 clinical trials, proving 90% effective in stopping people falling ill.\u201d<\/em><\/a>\u00a0 If approved, Pfizer\u2019s jab will be the first in a new era of vaccines. Instead of using a traditional approach, such as an <a href=\"https:\/\/www.research.ox.ac.uk\/Article\/2020-07-19-the-oxford-covid-19-vaccine\">attenuated, or weakened, viral vector<\/a>, Pfizer\u2019s vaccine uses viral mRNA as <a href=\"https:\/\/www.nature.com\/articles\/d41586-020-03166-8\">\u201cmolecular instructions for human cells to make the coronavirus spike protein\u201d<\/a>.\u00a0 This will potentially stimulate development of anti-SARS-CoV-2 antibodies.\u00a0 <a href=\"https:\/\/www.nature.com\/articles\/d41586-020-03166-8\">Tentative trial data<\/a> appears promising. Of the 44,000 participants, only 94 developed COVID-19 symptoms. \u00a0Given that the <a href=\"https:\/\/www.fda.gov\/news-events\/press-announcements\/coronavirus-covid-19-update-fda-takes-action-help-facilitate-timely-development-safe-effective-covid\">FDA have set the bar<\/a> for licencing at 50%, it seems likely that the vaccine could be made available soon. Likewise, the UK equivalent body &#8211; the <a href=\"https:\/\/www.gov.uk\/government\/organisations\/medicines-and-healthcare-products-regulatory-agency\">Medicines and Healthcare Products Regulatory Authority (MHRA<\/a>) &#8211; are similarly poised to offer an emergency licence if satisfied that its <a href=\"https:\/\/www.gov.uk\/government\/news\/new-measures-to-support-roll-out-of-vaccines-in-the-uk\">safe and effective<\/a>.<\/p>\n<p>As the media rush to hail the new vaccine \u2013 bolstered by the subsequent news <a href=\"https:\/\/www.theguardian.com\/world\/2020\/nov\/16\/what-does-moderna-vaccine-mean-fight-covid\">Moderna<\/a> have a 95% effective vaccine in trials \u2013 it is important to ensure that information communicated to the public be truthful, transparent and accurate. This is best communicated by experienced professionals. Questions remain as to whether the vaccine is <a href=\"https:\/\/www.nature.com\/articles\/d41586-020-03166-8\">effective<\/a> across all populations. Of particular concern is the <a href=\"https:\/\/www.google.com\/search?q=elderly+populations+weaker+immune+response+to+vaccines&amp;oq=elderly+populations+weaker+immune+response+to+vaccines&amp;aqs=chrome..69i57j33i160l2.7680j0j7&amp;sourceid=chrome&amp;ie=UTF-8\">elderly population<\/a> known to elicit weaker immunological responses to vaccines.\u00a0 It is also yet to be seen whether the vaccine will completely block transmission or instead simply reduce symptoms in vaccinated individuals \u2013 vital in terms of easing social distancing measures. \u00a0There is also a lack of longevity in terms of the data.\u00a0 Before the pandemic, it would take<a href=\"https:\/\/www.ifpma.org\/wp-content\/uploads\/2019\/07\/IFPMA-ComplexJourney-2019_FINAL.pdf\"> 10-15 years<\/a> to develop a vaccine from inception to licencing, yet the COVID-19 vaccines data was collected over two months. Nevertheless, the UK Government forges ahead with plans to roll out the vaccine.\u00a0 The military are being enlisted to distribute vaccine vials on dry ice &#8211; in their <a href=\"https:\/\/www.msn.com\/en-gb\/news\/newslondon\/army-faces-biggest-task-since-the-war-in-rolling-out-millions-of-covid-vaccine-doses\/ar-BB1aRZtN\">\u201cbiggest task since the war<\/a>\u201d &#8211; and <a href=\"https:\/\/news.sky.com\/story\/covid-19-volunteers-with-no-medical-background-could-administer-coronavirus-jab-reports-12134400\">St John\u2019s Ambulance Volunteers<\/a> are recruiting volunteers to administer the jab. Yet we must pause to consider how to ensure adequate levels of vaccine acceptance. Arguably, this is best achieved through shared decision-making and informed consent, which can be a <a href=\"https:\/\/jme.bmj.com\/content\/early\/2020\/05\/04\/medethics-2020-106068\">vehicle to addressing vaccine hesitancy.<\/a><\/p>\n<p><a href=\"https:\/\/www.theguardian.com\/society\/2019\/sep\/29\/government-seriously-considering-compulsory-vaccinations-matt-hancock\">Mandatory vaccination<\/a> was proposed as a solution to childhood vaccine hesitancy in 2019 by Health Secretary Matt Hancock. At that time, the House of Lords and the\u00a0<a href=\"https:\/\/www.rcpch.ac.uk\/news-events\/news\/vaccinations-important-highly-effective-says-child-health-expert\">Royal College of Paediatrics and Child Health<\/a>\u00a0(RCPCH) opposed the move, arguing that it would backfire, creating determined vaccine refusers. \u00a0Vaccine mandates may be undermined by high levels of exemption as they fail to address underlying fears.\u00a0 Studies suggest that comparable results may be achieved by strongly recommending vaccines instead.\u00a0 The RCPCH prompted healthcare practitioners to ensure <a href=\"https:\/\/www.rcpch.ac.uk\/news-events\/news\/rcpch-responds-prime-ministers-announcement-immunisation-uptake\">\u201cthat every contact [with a patient]\u2026be a vaccine opportunity\u201d<\/a> and so highlighted the crucial role <a href=\"https:\/\/jme.bmj.com\/content\/early\/2020\/05\/04\/medethics-2020-106068\">informed consent <\/a>\u00a0holds in ensuring the success of vaccination strategies. This is particularly pertinent given the <a href=\"https:\/\/www.independent.co.uk\/news\/uk\/home-news\/coronavirus-conspiracy-theories-bill-gates-hoax-vaccine-5g-qanon-protest-piers-corbyn-b575326.html\">global conspiracy theories<\/a> which surround COVID and in light of limited vaccine development time &#8211; two issues which are likely to contribute to vaccine hesitancy.<\/p>\n<p>The process of shared decision-making is crucial to ensuring legally valid informed consent is obtained. It enables experienced practitioners to <a href=\"https:\/\/jme.bmj.com\/content\/early\/2020\/05\/04\/medethics-2020-106068\">engage in a dialogue<\/a> with patients to address treatment fears, answer questions and provides an opportunity to challenge misinformation. It must be recognised that the general public have questions surrounding the COVID-19 vaccine, and it should not be left to the media to answer them. The use of volunteers raises serious concerns as to whether this key part of a vaccination strategy has been considered.\u00a0 It is particularly important in light of the <a href=\"https:\/\/www.immdsreview.org.uk\/downloads\/IMMDSReview_Web.pdf\">Cumberlege Report<\/a> (First Do No Harm) which exposed a lack of trust in healthcare due to the \u201c..<em>widespread lack of informed consent and a reluctance \u2026by those charged with patient care.. to listen and\u2026act\u2026\u201d<a href=\"#_ftn1\" name=\"_ftnref1\"><strong>[1]<\/strong><\/a>.<\/em><\/p>\n<p>An open dialogue as part of informed consent can be used to prompt consideration as to how autonomy is interpreted. Rather than the individualistic interpretations which stimulate talk of infringed <a href=\"https:\/\/www.theguardian.com\/commentisfree\/2020\/sep\/29\/coronavirus-act-liberties-powers-police-public-health-crisis\">individual liberties<\/a> and <a href=\"https:\/\/www.theguardian.com\/world\/2020\/apr\/29\/coronavirus-pandemic-1918-protests-california\">anti-mask opposition<\/a>, autonomy sits more comfortably as a relational value.\u00a0 The COVID-19 pandemic has demonstrated to great effect that when it comes to disease, we do not live in isolation. Our understanding of autonomy should therefore incorporate <em>some <\/em>social responsibility. Whilst ultimately upholding the patient\u2019s right to decide whether to incur individual risk, information disclosed to patients should be given from both individual and societal perspective. In this way, patients can be <em>fully <\/em>informed.\u00a0 \u00a0The benefits of vaccination should therefore address both benefit to the individual <em>and <\/em>society, such as the potential for individual and herd immunity. \u00a0It should also be disclosed that such immunity may not be long-lasting or equally effective across all age-ranges.\u00a0 Risk should be disclosed in terms of both <em>known<\/em> risks, including common side effects, and potentially <em>unknown<\/em> risk. The risk from <em>not<\/em> vaccinating should also be explained &#8211; both to the individual who may be at greater risk of contracting COVID-19 &#8211; and to society through increased transmission.<\/p>\n<p>Such risk may include both primary infection and subsequent \u201cLong-COVID\u201d. This will affect both the individual and society as a whole through increased <a href=\"https:\/\/www.bma.org.uk\/bma-media-centre\/long-term-impact-of-covid-on-nhs-must-not-be-underestimated\">strain on the NHS<\/a>.\u00a0 Whilst there will still be vaccine refusal, such an approach will engage those who are, as yet, undecided by tackling misinformation and promoting solidarity.<\/p>\n<p>Ultimately, by upholding autonomy, trust in the profession can also be rebuilt, which is vital given the likelihood of future epidemics and pandemics. \u00a0This approach will require additional consultation time, but this should be seen, in the long-term, as an investment.
