{"id":287,"date":"2010-07-30T09:03:44","date_gmt":"2010-07-30T08:03:44","guid":{"rendered":"https:\/\/blogs.bmj.com\/heart-journalscan\/?p=287"},"modified":"2015-11-12T14:04:03","modified_gmt":"2015-11-12T13:04:03","slug":"allopurinol-anti-ischaemic-in-stable-angina","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/heart\/2010\/07\/30\/allopurinol-anti-ischaemic-in-stable-angina\/","title":{"rendered":"Allopurinol anti-ischaemic in stable angina"},"content":{"rendered":"<p>Allopurinol is a xanthine oxidase inhbitor which has previously been shown to reduce myocardial oxygen consumption in patients with heart failure.\u00a0 As such, as the authors of this paper decided to investigate the potential benefit of high-dose allopurinol in patients with chronic stable angina.<!--more--><br \/>\nThe double-blinded, randomised, placebo controlled study recruited 65 patients with angiographically documented coronary artery disease, a positive exercise test, and a two month history of stable angina.\u00a0 Patients were assigned to either 600mg of allopurinol a day or placebo for 6 weeks prior to crossover.\u00a0 The primary endpoint was time to ST depression, the secondary endpoints were total exercise time and time to chest pain.<br \/>\nAlthough placebo treatment was also associated with an increase in the median time to ST depression, allopurinol was seen to be more beneficial (249 secs vs. 298 secs; p&lt;0.0002).\u00a0 In addition, allopurinol increased the median total exercise time to 393secs from a baseline of 301secs, while placebo increased it to 307 secs (p=0.0003).\u00a0 The time to chest pain was significantly improved by allopurinol to 304secs from a baseline of 234secs (p=0.001, see figure).<\/p>\n<p>Conclusion:<\/p>\n<p>In this small study, Allopurinol treatment acted as a safe anti-ischaemic agent in patients with stable angina.\u00a0 However, exactly how the drug should be used in combination with other available anti-anginals remains to be clarified.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-288\" title=\"Journal Scan Sept 1\" src=\"https:\/\/blogs.bmj.com\/heart-journalscan\/files\/2010\/07\/Journal-Scan-Sept-1.jpg\" alt=\"Figure\" width=\"551\" height=\"352\" \/><\/p>\n<p>Figure: Change in time from chest pain to symptoms after treatment with allopurinol or placebo.<\/p>\n<p>\u2022\u00a0Effect of high-dose allopurinol on exercise in patients with chronic stable angina: a randomised, placebo controlled crossover trial.\u00a0 Noman A, Ang DSC, Ogston S et al.\u00a0 Lancet 2010;375:2161-2167.<!--TrendMD v2.4.8--><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Allopurinol is a xanthine oxidase inhbitor which has previously been shown to reduce myocardial oxygen consumption in patients with heart failure.\u00a0 As such, as the authors of this paper decided to investigate the potential benefit of high-dose allopurinol in patients with chronic stable angina.
