For example, recent changes in the UK allow prescriptions for medical cannabis. <a href=\"https:\/\/bit.ly\/2VntmKd\">https:\/\/bit.ly\/2VntmKd \u00a0<\/a>Medical marijuana prescriptions are legal in more than half of the states and the District of Columbia in the US <a href=\"https:\/\/blogs.bmj.com\/ebn\/files\/2019\/04\/Screen-Shot-2019-04-07-at-7.08.38-PM.png\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-1622 alignleft\" src=\"https:\/\/blogs.bmj.com\/ebn\/files\/2019\/04\/Screen-Shot-2019-04-07-at-7.08.38-PM-300x195.png\" alt=\"\" width=\"300\" height=\"195\" srcset=\"https:\/\/blogs.bmj.com\/ebn\/files\/2019\/04\/Screen-Shot-2019-04-07-at-7.08.38-PM-300x195.png 300w, https:\/\/blogs.bmj.com\/ebn\/files\/2019\/04\/Screen-Shot-2019-04-07-at-7.08.38-PM-768x499.png 768w, https:\/\/blogs.bmj.com\/ebn\/files\/2019\/04\/Screen-Shot-2019-04-07-at-7.08.38-PM-640x416.png 640w, https:\/\/blogs.bmj.com\/ebn\/files\/2019\/04\/Screen-Shot-2019-04-07-at-7.08.38-PM.png 1012w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><a href=\"https:\/\/wb.md\/2Vs6ZTZ\">https:\/\/wb.md\/2Vs6ZTZ<\/a>and \u201cthe FDA has only approved it for treatment of two rare and severe forms of epilepsy, Lennox-Gastaut syndrome and Dravet syndrome.\u201d In Canada, medical cannabis has been available for two decades with a number of revisions the regulation, the most recent in 2016\u00a0<a href=\"https:\/\/bit.ly\/2G4DjqB\">https:\/\/bit.ly\/2G4DjqB<\/a>.<\/p>\n<p>The process for prescribing and dispensing medical marijuana varies. In my province of Ontario, patients typically attend the clinic of a cannabis vendor where they are assessed by, and receive a prescription from, a physician, who more than likely conducts the appointment by videoconference. Patients are prescribed marijuana to smoke, oil to ingest or pills from this clinic and receive their prescription through the mail.<\/p>\n<p>In addition to the prescription of cannabis for medical purposes, in 2018 recreational marijuana was legalized in Canada.\u00a0 This recent change seems to have legitimized cannabis with the general public and, subsequently, prescriptions are on the rise for a wide variety of ailments, including pain, anxiety and glaucoma, for which there is often little or no evidence \u00a0<a href=\"https:\/\/bit.ly\/2OqUbe0\">https:\/\/bit.ly\/2OqUbe0<\/a>.<\/p>\n<p>I work in a family practice and I\u2019ve been alarmed with the increasing number of patients who are taking medical marijuana.\u00a0 They receive prescriptions in the form of oil, or to smoke, for such things as fibromyalgia, insomnia and anxiety.\u00a0 My concerns were augmented the last day I was at the clinic when a patient asked if it was OK to take marijuana with a heart condition.\u00a0There was no information on his chart about his marijuana prescription and I struggled to find information to help him.\u00a0 I finally sent him back to the cannabis clinic for their input.\u00a0 This incident peaked my interest in the overall safety and efficacy of medical cannabis, prompting a deep dive into the available research.<\/p>\n<p>Some physicians echo my concerns about the wide spread use of this drug and have questioned the evidence to support its use <a href=\"https:\/\/bit.ly\/2OqUbe0\">https:\/\/bit.ly\/2OqUbe0<\/a>\u00a0 There is a difference in what is known about the effects and effectiveness of synthetic vs natural cannabis products.\u00a0 Nabilone is a man-made form of cannabis while marijuana is an herbal form (oil, smoke, edibles).\u00a0 There have been studies on nabilone, but there is very little research evidence about marijuana <a href=\"https:\/\/bit.ly\/2LaWdfa\">https:\/\/bit.ly\/2LaWdfa<\/a><\/p>\n<p>The Canadian Agency for Drugs and Technologies in Health (CADTH) is a not-for-profit, national organization that reviews evidence about medical treatments for use by health care providers. CADTH has compiled evidence about the use of nabilone for a variety of health issues. Their reports reveal that there is evidence to support the use of nabilone in very few conditions. Some of their reports demonstrate the following:<\/p>\n<ul>\n<li>In the analysis of one systematic review of systematic reviews and three RCTs, findings revealed that for nausea and vomiting related to chemotherapy, radiation therapy or post surgical, nabilone \u201cwas <strong>not <\/strong>found to be more effective than conventional antiemetics or placebo\u201d As such, it is listed as a third line therapy. <a href=\"https:\/\/bit.ly\/2Ih6vwc\">https:\/\/bit.ly\/2Ih6vwc<\/a>\u00a0In addition, medical cannabinoids are contraindicated for general nausea and vomiting in primary care and in pregnancy.<\/li>\n<li>There are no relevant evidence-based guidelines for the use of medical cannabinoids for PTSD <a href=\"https:\/\/bit.ly\/2VqP9Ri\">https:\/\/bit.ly\/2VqP9Ri<\/a>\u00a0 There is a lack of data related to medical cannabinoids for behavioural symptoms in adults with dementia <a href=\"https:\/\/bit.ly\/2IhUeaI\">https:\/\/bit.ly\/2IhUeaI<\/a><\/li>\n<li>Buccal spray medical marijuana is indicated as third line treatment for neuropathic pain in one guideline. <a href=\"https:\/\/bit.ly\/2VpkrYx\">https:\/\/bit.ly\/2VpkrYx<\/a><\/li>\n<li>Only one systematic review and two primary studies were found in a search for studies investigating the use of nabilone for chronic cancer and non-cancer pain.\u00a0 Some benefits were identified and it was acknowledged that potential harms of nabilone are less than those of other pain medications, particularly opioids. However, limitations of the studies indicate that the findings should be viewed with caution. <a href=\"https:\/\/bit.ly\/2D0CFIN\">https:\/\/bit.ly\/2D0CFIN<\/a><\/li>\n<\/ul>\n<p>There is a dearth of information about drug interactions with medical marijuana.\u00a0Additionally, the potential harms are not front and centre of marketing for medical marijuana.\u00a0 Things like the link between daily use of high-potency cannabis to developing psychosis <a href=\"https:\/\/bit.ly\/2OqUbe0\">https:\/\/bit.ly\/2OqUbe0<\/a>.\u00a0 My concerns of the side effects of marijuana extend to frail elderly, or compromised patients.\u00a0How many falls and injuries will be perpetuated by medical marijuana use?<\/p>\n<p>The <em>Canadian Family Physician<\/em>published a clinical practice guideline for the indications related to prescribing medical cannabinoids. Due to the high level of bias in studies and the lack of evidence from quality studies, medical cannabinoids are recommended for very few conditions. The guideline identifies that there is enough evidence to support prescribing medical cannabinoids for chronic pain, nausea and vomiting and spasicity.\u00a0 Even with this, medical cannabinoids are recommended as third line and there is a strong recommendation against the use of medical marijuana (smoked, oils or edibles) due to the lack of research. <a href=\"https:\/\/bit.ly\/2LaWdfa\">https:\/\/bit.ly\/2LaWdfa<\/a><\/p>\n<p>The evidence shows that medical marijuana has a place in health care.\u00a0However, the circumstances are few and with many limitations.\u00a0 Yet, in my country, medical marijuana is easy to obtain, with seemingly little monitoring, for a wide variety of conditions. The current Canadian experience should serve as a warning to health care providers in other countries in the trend toward more lenient regulation around the use of marijuana in general.\u00a0 Patients should understand that marijuana, in any form, is a drug and should be treated as such.<\/p>\n<p>A Canadian physician, Dr. Sarah Giles, expressed her concerns and they serve as an appropriate final statement: \u201cMarijuana is the next OxyContin, where everyone\u2019s like, \u2018Oh, it\u2019s not addictive, it\u2019s harmless, it\u2019s good,\u2019 and people are kind of using it willy-nilly because you can get it prescribed and not prescribed,\u201d she said. \u201cI think we will look back at this time and just be like, \u2018What the hell was going on?&#8217;\u201d <a href=\"https:\/\/bit.ly\/2OqUbe0\">https:\/\/bit.ly\/2OqUbe0<\/a><!--TrendMD v2.4.8--><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Roberta Heale, Associate Editor of EBN, @robertaheale, @EBNursingBMJ Changes to regulation in many countries have opened up prescribing of medical marijuana.
