{"id":18,"date":"2009-10-13T12:09:34","date_gmt":"2009-10-13T11:09:34","guid":{"rendered":"https:\/\/blogs.bmj.com\/ebn-confessions\/?p=18"},"modified":"2009-10-13T12:09:34","modified_gmt":"2009-10-13T11:09:34","slug":"venous-leg-ulcers","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/ebn-confessions\/2009\/10\/13\/venous-leg-ulcers\/","title":{"rendered":"Venous Leg ulcers"},"content":{"rendered":"<p class=\"MsoNormal\" style=\"margin: 0cm 0cm 0pt\"><span style=\"font-size: small;font-family: Arial\">It would be easy to conclude, as I come to an end of my community placement, that the majority of people over the age of 75 have at least one venous leg ulcer.<span>\u00a0 <\/span>Mr O\u2019Leary was no exception \u2013 in fact his was particularly painful and had so far resisted all attempts to heal.<span>\u00a0 <\/span>Like so many of the older folks in the community, he suffered the pain stoically.<span>\u00a0 <\/span>As he squelched his way into his living room for the daily bandage change, I glanced at the exudate stained leather of his shoes and the soggy mass of<span>\u00a0 <\/span>bandaging and was reminded of the embarrassment endured by these patients.<span>\u00a0 <\/span>Mr O\u2019Leary smiled warmly, gently rolling his fingers in Parkinsonian rhythm as he enquired after my health in a soft Irish lilt.<span>\u00a0 <\/span>I desperately wanted to find a way to promote healing, and started searching for any hints in the research literature.<span>\u00a0 <\/span>It surprised me to find that until the 1990s, although the UK Department of Health knew that a major proportion of community health expenditure went into treating leg ulcers, it was not known how much that was, neither was the prevalence of leg ulcers known, nor what treatments were being used and whether they worked.<span>\u00a0 <\/span>In fact little was known at all.<span>\u00a0 <\/span>A systematic review of available research was commissioned for the first time to try to answer these and other questions about leg ulcers.<span>\u00a0 <\/span>The results were not encouraging.<span>\u00a0 <\/span>The majority of the questions remained unanswered or where there were answers those answers were unclear.<span>\u00a0 <\/span>I realised that nursing research has a place in identifying further research needs as well as answering clinical questions.<span>\u00a0 <\/span>Fortunately what has been described as an \u2018evidence desert\u2019 spurred the NHS into funding a suite of systematic reviews of wound care in 1997.<span>\u00a0 <\/span>It is now widely accepted that compression bandaging is the main treatment for venous leg ulcers (Cullum et al 2001), and that no conclusive evidence favouring any particular dressing beneath the compression bandaging has emerged (Palfreyman et al 2006).<span>\u00a0\u00a0 <\/span>Unfortunately Mr O\u2019Leary was not able to tolerate significant compression, and seldom remembered to elevate his legs to aid venous return.<span>\u00a0 <\/span>I had no Panacea to help him, and it was clear that we still need further research into this area.<span>\u00a0 <\/span>\u2018I\u2019ll see you at the same time tomorrow Mr O\u2019Leary\u2019 I said having completed the dressing.<span>\u00a0 <\/span>What was my next visit?<span>\u00a0 <\/span>Oh yes, another venous leg ulcer.<\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin: 0cm 0cm 0pt\"><span style=\"font-size: small\"><span style=\"font-family: Arial\">\u00a0<\/span><\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin: 0cm 0cm 0pt\"><span style=\"font-size: 9pt;color: #333333\"><span style=\"font-family: Arial\">\u00a0<\/span><\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin: 0cm 0cm 0pt\"><span style=\"font-size: 10pt;color: black\"><span style=\"font-family: Arial\">Cullum N, Nelson EA, Fletcher AW, <em>et al.<\/em>.
