{"id":1184,"date":"2017-02-06T08:40:36","date_gmt":"2017-02-06T08:40:36","guid":{"rendered":"https:\/\/blogs.bmj.com\/case-reports\/?p=1184"},"modified":"2017-11-01T15:13:29","modified_gmt":"2017-11-01T15:13:29","slug":"back-to-basics-with-female-genital-mutilation","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/case-reports\/2017\/02\/06\/back-to-basics-with-female-genital-mutilation\/","title":{"rendered":"Back to Basics with Female Genital Mutilation"},"content":{"rendered":"<p>By Manasi\u00a0Jiwrajka<\/p>\n<p>The 6<sup>th<\/sup> of February marks the <a href=\"http:\/\/www.un.org\/en\/events\/femalegenitalmutilationday\/index.shtml\" target=\"_blank\" rel=\"noopener\">International Day of Zero Tolerance to Female Genital Mutilation<\/a>.<\/p>\n<p>Last year I had the privilege to listen to Khadija Gbla at the Global Health Conference in Newcastle, Australia organised by the Australian Medical Student Association. Khadija is the voice and face of female genital mutilation (FGM) in Australia and worldwide. She addressed an audience of ambitious future doctors interested in global health, and what struck me was the simple message: take a good history and examine the patient.<\/p>\n<p>As junior doctors and medical students, we often forget that we may be closer to patients than anyone else because there is not that white coat barrier between the patient and us. We forget that although we don\u2019t always make major clinical decisions, the information we relay to our seniors is what determines their clinical decision, and as such a thorough history and examination is crucial.<\/p>\n<p>Patients who have had FGM inflicted on them may present with common symptoms such as \u00a0incontinence, dysmenorrhea, dyspareunia or all of these. <a href=\"http:\/\/casereports.bmj.com\/content\/2013\/bcr-2012-008155.full\" target=\"_blank\" rel=\"noopener\">In this case report<\/a> by Abdulcadir and Dallenbach <sup>1<\/sup>, a 27 -year old female presents with the above symptoms after having consulted a gynaecologist. They write:<\/p>\n<blockquote><p><em>\u201cShe had been living in Europe for more than 5 years and mentioned having consulted a gynaecologist some years ago, but no treatment had been proposed. She added that she had felt uncomfortable because the physician did not seem familiar with FGM\/C. After that experience, she never consulted again in spite of worsening symptoms.\u201d<\/em>(Abdulcadir and D\u00e4llenbach 2013)<\/p><\/blockquote>\n<p>One of the key learning points from the case report, as authors write, is \u201cOveractive bladder post female genital mutilation\/cutting type III is a very distressing condition, probably under-reported due to shame, poor medical reception and cultural barriers.\u201d We in the medical profession can at least make ourselves aware of FGM and reduce the stigma associated with this.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-1185 aligncenter\" src=\"https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/WHO-Classification-500x197.png\" alt=\"\" width=\"500\" height=\"197\" srcset=\"https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/WHO-Classification-500x197.png 500w, https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/WHO-Classification-300x118.png 300w, https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/WHO-Classification.png 606w\" sizes=\"auto, (max-width: 500px) 100vw, 500px\" \/> <img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-1186 aligncenter\" src=\"https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/FGM-315x400.png\" alt=\"\" width=\"315\" height=\"400\" srcset=\"https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/FGM-315x400.png 315w, https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/FGM-300x381.png 300w, https:\/\/blogs.bmj.com\/case-reports\/files\/2017\/02\/FGM.png 521w\" sizes=\"auto, (max-width: 315px) 100vw, 315px\" \/><\/p>\n<p>Khadija\u2019s message to us still holds true.
