{"id":2405,"date":"2023-10-02T15:04:13","date_gmt":"2023-10-02T15:04:13","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmjsrh\/?p=2405"},"modified":"2023-10-03T09:17:53","modified_gmt":"2023-10-03T09:17:53","slug":"mens-duty-to-get-a-vasectomy","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmjsrh\/2023\/10\/02\/mens-duty-to-get-a-vasectomy\/","title":{"rendered":"On Men\u2019s Duty to get a Vasectomy under the Patriarchy"},"content":{"rendered":"<p style=\"text-align: right\"><em>By Ezio Di Nucci<\/em><\/p>\n<p>&nbsp;<\/p>\n<p><span data-contrast=\"auto\">Last night I was running with my public health colleagues (the token philosopher ended up being faster than our Head of Department from Biostat, by the way \u2013 which sure must have a symbolic meaning in the modern academy). <\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">After the race, conversation turned to my forthcoming vasectomy (anecdotally, this might well be because I am told that after a vasectomy you are not allowed to run for a week or so, the patriarchy\u2019s bag of tricks being sure deep).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Then a surprising thing happened: my colleagues \u2013 all women \u2013 started trying to talk me out of getting a vasectomy \u2013 mentioning, for example, evidence pointing to the fact that <\/span><a href=\"https:\/\/en.ssi.dk\/news\/news\/2019\/vasectomy-is-associated-with-increased-risk-of-prostate-cancer\"><span data-contrast=\"auto\">vasectomies increase prostate cancer risk by 15%<\/span><\/a><span data-contrast=\"auto\"> (parochialism disclaimer: the study comes from the very same country where the conversation was taking place, Denmark).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">It might be worth reflecting on the difference between my female colleagues\u2019 arguments and the reaction I have had from male friends and colleagues, which tended to be different versions of the following two considerations:<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<ol>\n<li><span data-contrast=\"auto\">What if you meet someone else and want to have kids with this new partner?<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/li>\n<li><span data-contrast=\"auto\">What if one of your kids dies, you might then want another one.<\/span><\/li>\n<\/ol>\n<p><span data-contrast=\"auto\">Neither of these two considerations made much sense to me, but as they are existential more than philosophical in nature, in what follows I will focus on the argument used by my female colleagues after the race, which throws up a fascinating question at the intersection of reproductive health, feminist bioethics and the philosophy of medicine:\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><i><span data-contrast=\"auto\">given patriarchal structures of oppression, how should we evaluate a 15% increase in the risk of prostate cancer?\u00a0<\/span><\/i><span data-ccp-props=\"{&quot;335559685&quot;:720}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">For the sake of this argument, we will assume the validity of the study (<\/span><a href=\"https:\/\/ebm.bmj.com\/content\/23\/1\/1.abstract\"><span data-contrast=\"auto\">because otherwise there would also be a lot to say about overmedicalization and prostate cancer screenings in particular<\/span><\/a><span data-contrast=\"auto\">).<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The rest of this short comment will try to answer the above question: the basic argument will be that the patriarchy <\/span><i><span data-contrast=\"auto\">silences<\/span><\/i><span data-contrast=\"auto\"> a 15% increase in the risk of prostate cancer (and probably more\u2026 what about 50%, for example?), because the relevant medical considerations here are trumped by feminist ones. And that therefore such data ought to be irrelevant, morally and politically speaking, to my decision-making about getting a vasectomy.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">What is philosophically interesting, here, is the commensurability of, on the one hand, a medical data point \u2013 increase in cancer risk \u2013 and, on the other hand, the ethical and political issue of how the \u201cburdens\u201d of prevention should be distributed within a society still soaked in <\/span><a href=\"https:\/\/blogs.bmj.com\/medical-ethics\/2016\/09\/10\/sharing-motherhood-and-patriarchal-prejudices\/\"><span data-contrast=\"auto\">patriarchal prejudices<\/span><\/a><span data-contrast=\"auto\">, which has recently gone back <\/span><a href=\"https:\/\/blogs.bmj.com\/medical-ethics\/2022\/11\/14\/coney-barretts-juxtaposition-covid-abortion\/\"><span data-contrast=\"auto\">even on abortion<\/span><\/a><span data-contrast=\"auto\">. But before dealing with the complex issue of whether medical considerations are commensurable with feminist ones (social, political, ethical), we mustn\u2019t forget that <\/span><a href=\"https:\/\/www.plannedparenthood.org\/learn\/birth-control\/birth-control-pill\/how-safe-is-the-birth-control-pill%22%20\/l%20%22:~:text=Even%20though%20birth%20control%20pills,they%20can%20lead%20to%20death.\"><span data-contrast=\"none\">contraceptives for women carry their own risks<\/span><\/a><span data-contrast=\"auto\">. <\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The first dilemma, then, is whether or not those medical and ethical\/political issues are commensurable; but that\u2019s an easy dilemma to solve. Given scarcity of resources, we are always weighing medical considerations against non-medical considerations within the healthcare system and society at large (<\/span><a href=\"https:\/\/blogs.bmj.com\/medical-ethics\/2020\/10\/20\/have-enough-people-died\/\"><span data-contrast=\"auto\">just think of COVID-19<\/span><\/a><span data-contrast=\"auto\">), so nothing new here.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The more challenging question is according to what principles should we weigh medical and non-medical (ethical and political) considerations against each other, once we have accepted that those are \u2013 must be \u2013 commensurable.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">And here a 15% increase \u2013 especially in the risk of a kind of cancer that many consider paradigmatic of overdiagnosis anyway &#8211; seems like a small price to pay; and maybe even a price that men <\/span><i><span data-contrast=\"auto\">ought to<\/span><\/i><span data-contrast=\"auto\"> pay. As in, a duty to get a vasectomy in the face of patriarchal structures of oppression. Is that going too far? This might indeed depend on other features of one\u2019s life situation, both locally and globally: already having three kids, for example, can hardly be expected to be irrelevant to such an assessment. As is climate change.\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">A full defence of a duty to get a vasectomy \u2013 under a clearly defined set of circumstances likely to involve both one\u2019s family situation and climate considerations \u2013 is beyond the scope of this short commentary; it is rather a methodological move meant at raising <\/span><a href=\"https:\/\/blogs.bmj.com\/medical-ethics\/2023\/04\/13\/kant-our-prophet\/\"><span data-contrast=\"auto\">the larger issue of whether liberal principles of opportunity and availability are enough to challenge powerful oppressive structures, be that the patriarchy or climate change<\/span><\/a><span data-contrast=\"auto\">. In a slogan: <\/span><i><span data-contrast=\"auto\">is democracy enough?<\/span><\/i><span data-contrast=\"auto\"> But this BIG question is clearly beyond the scope of this short comment. <\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">And this problem \u2013 can I even say it? &#8211; is particularly worrying if it happens to be women themselves who, in \u201ccaring\u201d for their partners or colleagues, appeal to \u201cobjective\u201d medical considerations in preventing change.
