{"id":2214,"date":"2019-09-28T17:02:50","date_gmt":"2019-09-28T17:02:50","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmjsrh\/?p=2214"},"modified":"2019-09-28T17:02:50","modified_gmt":"2019-09-28T17:02:50","slug":"neglect-of-stis-and-infertility","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmjsrh\/2019\/09\/28\/neglect-of-stis-and-infertility\/","title":{"rendered":"Why neglect of STIs and infertility could be undermining family planning programmes"},"content":{"rendered":"<p>&nbsp;<\/p>\n<h5>Why do people still not use effective contraceptive methods?<\/h5>\n<p>One reason is that many women and their families in low- and middle-income countries fear the most effective contraceptives can cause infertility.\u00a0 A conventional response to this is that people simply need more information to put them on the right track \u2013 that women who, for instance, are using injectable contraceptives simply need reassurance that any fears of infertility are unfounded.<\/p>\n<p>But what if the risk of infertility in some lower income countries is all too real \u2013 but the real cause is hidden? This, <a href=\"https:\/\/srh.bmj.com\/content\/early\/2019\/09\/25\/bmjsrh-2018-200270\">our paper<\/a> argues, may well be happening with infertility caused by undiagnosed, asymptomatic STIs.<\/p>\n<p>Chlamydial infection in particular is an important cause of tubal factor infertility. So infertility caused by undetected, asymptomatic chlamydia may well be widespread in areas where it is prevalent \u2013 including among women who have recently stopped using contraceptives.<\/p>\n<p>All of this may contribute to a sense that it is the contraceptives causing the infertility \u2013 simply because their use coincides with less or no condom use, and undetected STI infection.<\/p>\n<p>An indicator of the scale of the threat STIs pose to women\u2019s fertility comes from recent work (<a href=\"https:\/\/journals.plos.org\/plosmedicine\/article?id=10.1371\/journal.pmed.1002512\">here<\/a> and <a href=\"https:\/\/journals.plos.org\/plosmedicine\/article?id=10.1371\/journal.pmed.1002511\">here<\/a>) in South Africa, showing a major, uncontrolled chlamydia epidemic among young people. It seems reasonable to assume that this problem, and any resulting infertility, is not confined to one country.<\/p>\n<p>We hope our commentary will encourage more research into the global prevalence of STIs and the degree to which the associated infertility could be undermining people\u2019s trust in family planning programmes. Counterintuitively it is possible that by addressing fertility problems in programme settings, fertility may even be reduced as more people trust that they can use contraception without experiencing devastating infertility.<\/p>\n<p>More broadly, we call for research and programmes that address women\u2019s own priorities as a starting point to develop sexual and reproductive health programmes that are more attractive and win people\u2019s trust. To address complex problems, programmes and research must be co-produced <a href=\"https:\/\/www.who.int\/bulletin\/volumes\/94\/5\/15-168492\/en\/\">with the communities concerned<\/a>, and break down disciplinary boundaries such as those between family planning and STI prevention and treatment.<\/p>\n<p>Crucially, programmes must address women\u2019s right and desire to control their fertility in the fullest sense \u2013 in other words having children when they want them as well as avoiding births when they do not. In high fertility settings, infertility \u2013 so often personally devastating and socially stigmatising for the people affected \u2013 may not even be considered a problem by funders.
