{"id":40039,"date":"2025-02-25T09:06:06","date_gmt":"2025-02-25T09:06:06","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmjgh\/?p=40039"},"modified":"2025-02-25T09:06:06","modified_gmt":"2025-02-25T09:06:06","slug":"global-trends-in-maternal-care-call-for-a-decolonization-approach","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmjgh\/2025\/02\/25\/global-trends-in-maternal-care-call-for-a-decolonization-approach\/","title":{"rendered":"Global trends in maternal care call for a decolonization approach"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p>Self-managed\u00a0abortion, or abortion self-care, is when\u00a0a\u00a0pregnant person obtains\u00a0an\u00a0abortion without\u00a0supervision by\u00a0a\u00a0healthcare professional.<\/p>\n<p><a href=\"https:\/\/www.who.int\/publications\/i\/item\/WHO-SRH-22.1\">Safe abortion self-care<\/a> requires:<\/p>\n<ul>\n<li>Access to accurate information on the dosing, timing, how to manage pain, and when to seek further help;<\/li>\n<li>Access to medicines of reliable quality;<\/li>\n<li>Confirmation of success, and options for contraception.<\/li>\n<\/ul>\n<p>Abortions, miscarriages and births at home have a long history as part of ancestral and Indigenous birthing practices. The process of colonisation has led to the imposition of Western medical models of maternal healthcare globally, and has privileged Western research over Indigenous ways of knowing and being. Through the forced displacement of Native American nations, in-depth connection and knowledge of botanical remedies to assist in commonplace medical needs, developed over centuries, has been lost<\/p>\n<p>\u201cWe knew what kind of plants were available, how to use them, and how to harvest them.\u00a0 We used them in lactation, headaches, indigestion and abortion\u201d says Rachael Lorenzo, who is of the Mescalero Apache, Laguna Pueblo and Xicana tribes and is current Co-Executive Director of <a href=\"https:\/\/www.iwrising.org\/\">Indigenous Women Rising<\/a>, a reproductive sovereignty organization that provides financial assistance to Native abortion seekers in the USA.\u00a0 They explain that Western colonialist frameworks often value a live birth above all else.\u00a0 In contrast, the Indigenous worldview centers the wellbeing of the collective, regardless of birth outcome.\u00a0 The ritual ceremony of burying remains on tribal lands is the same whether the pregnancy loss was due to induced or spontaneous abortion.<\/p>\n<p>Globally, the legal status of abortion self-care varies, with self-management often criminalised even in countries like the UK, where abortion access is assumed. Indigenous practices of abortion, rooted in autonomy and community, stand in stark contrast to these state-imposed controls.<\/p>\n<p>The legal and structural barriers to self-managed abortion perpetuate settler-colonial oppression while denying Native peoples their inherent right to practice traditional birthing. To continue to deny Native people the ability to practice and preserve their languages, cultures and traditions in light of the violent and genocidal history is unconscionable.<\/p>\n<p>Lorenzo shares a photograph from the Mescalero Apache Tribe Museum in New Mexico, showing a mother, father, and two children, a small glimpse into the Apache\u2019s nomadic life. Without permanent settlements and facing threats like warfare and drought, families had to constantly assess the safety of each pregnancy. In this context, abortion was necessary for the wellbeing of the entire family unit, including the pregnant person and the fetus.<\/p>\n<p><a href=\"blank\">\u201cMenstrual regulation\u201d<\/a> is the term used for when a menstrual period is missed, and techniques including use of abortifacients are used to induce menstruation whether or not pregnancy is confirmed.\u00a0 It is a well-documented, commonplace traditional practice in many cultures globally.<\/p>\n<figure id=\"attachment_40040\" aria-describedby=\"caption-attachment-40040\" style=\"width: 660px\" class=\"wp-caption alignnone\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-40040\" src=\"https:\/\/blogs.bmj.com\/bmjgh\/files\/2025\/02\/Sabrina-Das-blog.jpg\" alt=\"\" width=\"660\" height=\"620\" srcset=\"https:\/\/blogs.bmj.com\/bmjgh\/files\/2025\/02\/Sabrina-Das-blog.jpg 660w, https:\/\/blogs.bmj.com\/bmjgh\/files\/2025\/02\/Sabrina-Das-blog-300x282.jpg 300w, https:\/\/blogs.bmj.com\/bmjgh\/files\/2025\/02\/Sabrina-Das-blog-640x601.jpg 640w\" sizes=\"auto, (max-width: 660px) 100vw, 660px\" \/><figcaption id=\"caption-attachment-40040\" class=\"wp-caption-text\">Figure 1: Photograph of an Apache family displayed in the Mescalero Apache Tribe Museum (credit: Rachael Lorenzo)<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p>Mohawk midwife and Indigenous feminist Katsi Cook links the movement for sovereignty of <a href=\"https:\/\/davidsuzuki.org\/what-you-can-do\/what-is-land-back\/\">\u201cLand Back\u201d<\/a> to sovereignty over <a href=\"https:\/\/pressbooks.claremont.edu\/gws183transfemtheories2024\/chapter\/abby-gonzalez-indigenous-reproductive-healthcare-and-politics\/\">Indigenous bodies<\/a>. Indeed, the universal principle of bodily autonomy can be applied as an argument for availability of self-managed abortion for all.\u00a0 \u201cWhat is good for Native people is good for everyone\u201d is a quote attributed to Tecumseh, a Shawnee warrior and chief in the late 1700s and early 1800s. This idea still resonates today, where global healthcare inequalities continue to exist along racial lines. <a href=\"https:\/\/www.npeu.ox.ac.uk\/birthplace\/results\">Homebirth, midwife-led birth<\/a>, <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5538578\/\">doulas<\/a> and use of non-medical therapies such as <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2212958822000581\">massage<\/a>, <a href=\"https:\/\/sigmapubs.onlinelibrary.wiley.com\/doi\/10.1111\/wvn.12747\">peer-support<\/a>, and <a href=\"https:\/\/research.ebsco.com\/linkprocessor\/plink?id=66b0e65c-e1d1-38fa-abe7-5f478b8b6427\">hypnobirthing<\/a> have well documented, evidence-based benefits in maternal care.\u00a0 However due to structures that hegemonizes capitalism, patriarchy and Western models of healthcare delivery, these forms of care are <a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2024\/jun\/insights-us-maternal-mortality-crisis-international-comparison\">often unavailable<\/a> both in high and low-income settings. The global trend we see now is one of medicalization of birth and <a href=\"https:\/\/gh.bmj.com\/content\/6\/6\/e005671\">increase in operative birth<\/a> without a clear and proven benefit.\u00a0 Instead, linked to increased medical interventions and lack of bodily autonomy in birth, we have growing concerns around <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S1871519222002141\">birth trauma<\/a> and poor maternal mental health.<\/p>\n<p>A modern vision for what self-managed abortion may look like, that respects traditional knowledge and cultural safety, as well as incorporates our current evidence-base for safe abortion care, calls for a decolonization approach.
