{"id":40025,"date":"2025-01-12T03:05:33","date_gmt":"2025-01-12T03:05:33","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmjgh\/?p=40025"},"modified":"2025-01-12T03:05:33","modified_gmt":"2025-01-12T03:05:33","slug":"health-is-the-new-global-plastics-treaty-battleground","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmjgh\/2025\/01\/12\/health-is-the-new-global-plastics-treaty-battleground\/","title":{"rendered":"Health is the new Global Plastics Treaty Battleground"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p>In November, international delegates to<a href=\"https:\/\/www.unep.org\/inc-plastic-pollution\/session-5\"> INC-5<\/a>, the international negotiating committee for a Global Plastics Treaty (GPT) held in Busan, South Korea, failed to agree on a legally binding United Nations treaty on plastic pollution. The main fault lines lie between countries supporting an ambitious treaty, including a cap on plastic production, and a minority seeking to focus the treaty provisions on waste management and voluntary measures. At INC-5, health emerged as a key battleground for disputes over the scope and power of the Treaty.<\/p>\n<p>The draft text discussed in Busan included a dedicated article focused on the health impact of plastic pollution: Article 19.<\/p>\n<p>Several countries propose a strengthening of obligations in Article19 to prevent and mitigate health risks from plastic pollution, for example by setting targets for reducing exposure.<\/p>\n<p>However, others proposed weak language that \u2018encourages\u2019 states to prevent health risks rather than supporting binding commitments, or oppose the inclusion of a dedicated article on health altogether. In their statement opposing its inclusion, the Saudi Arabia delegation claimed that including health issues in the treaty will \u2018<a href=\"https:\/\/resolutions.unep.org\/incres\/uploads\/cg_4_-_art.19_health_saudi_arabia_0.pdf\">divert attention from the core objective we have at hand of\u2014addressing plastic pollution<\/a>\u2019.<\/p>\n<p>The protection of health is one of the main reasons why an international, legally binding treaty on plastic pollution is needed. As the <a href=\"https:\/\/apps.who.int\/gb\/mspi\/pdf_files\/2024\/11\/Item2_01-11.pdf\">WHO submission<\/a> to INC-5 states \u2018the plastic crisis is also a health crisis\u2019.\u00a0 Microplastics have been detected in human blood and multiple human organs, and <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/38446676\">correlations between microplastics in arterial plaques and fatal outcomes are emerging<\/a>.\u00a0 Among over <a href=\"https:\/\/plastchem-project.org\/\">15,000 chemicals used in plastics, 3,600 are unregulated chemicals of concern<\/a>.\u00a0 Fewer than 6% of the total are subject to international regulation, and we lack essential hazard information on 62%, making it impossible to be sure that they are safe.\u00a0 Many of the health burdens of plastic pollution disproportionately fall on economically disadvantaged, minority and indigenous populations who live and work at sites of plastic production and discard.<\/p>\n<p>Article 19 is crucial because regulatory authorities have failed to respond to mounting evidence of the toxic qualities of plastic-related chemicals.\u00a0 <a href=\"https:\/\/ehp.niehs.nih.gov\/doi\/pdf\/10.1289\/ehp.00108579\">We have known that vinyl chloride, used to make PVC, is a carcinogen for 50 years<\/a>, yet <a href=\"https:\/\/cris.unibo.it\/bitstream\/11585\/840319\/4\/12_347_Bottausci_21.pdf\">tens of millions of tonnes<\/a> are manufactured globally each year; many countries have restricted the use of chemicals called <a href=\"https:\/\/ec.europa.eu\/health\/opinions\/en\/phthalates-school-supplies\/glossary\/def\/directive-phthalate-containing-toys-childcare-articles.htm#:~:text=Since%201999%20the%20European%20Commission,long%2Dterm%20impact%20on%20health.\">phthalates in children\u2019s toys<\/a> because of their toxicity, yet they remain in <a href=\"https:\/\/pubs.acs.org\/doi\/pdf\/10.1021\/acs.est.2c09182\">intraveous<\/a> and <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6538481\/\">respiratory equipment<\/a> that releases them directly into patients\u2019 bodies.\u00a0 These are just two examples of many.<\/p>\n<p>Naysayers at INC-5 were supported by <a href=\"https:\/\/www.theguardian.com\/environment\/2024\/nov\/27\/plastic-lobbyists-biggest-group-un-treaty-talks-busan-korea\">hundreds of industry lobbyists<\/a>, some of whom are <a href=\"https:\/\/www.source-material.org\/plastics-science-merchants-doubt-oil-tobacco\/\">accused of intimidating environmental scientists<\/a>. One reason these interest groups oppose the inclusion of health issues is that health impacts from plastic pollution occur at every stage of the plastic lifecycle, from production to use and disposal. Binding provisions to protect health would therefore help to bolster the case for an ambitious Treaty that addresses the entire lifecycle of plastics.<\/p>\n<p>Until now, these lobby groups have been largely successful in sidelining health from the Treaty negotiations. Gaps in scientific evidence have been misrepresented to emphasise scientific uncertainty and justify inaction. At INC-5, a 2019 WHO report on <a href=\"https:\/\/www.who.int\/publications\/i\/item\/9789241516198\">microplastics in drinking water<\/a> which suggested the risks to human health are low was widely touted. Four days into the meeting, WHO issued a statement to clarify that the report should not be represented as suggesting that health risks from microplastics are low in general.<\/p>\n<p>One sign that the health risks of plastics are being taken more seriously is that a proposed <a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(24)02254-2\/fulltext\">blanket exemption for medical plastics<\/a> from the Treaty provisions has now been removed from the draft text.<\/p>\n<p>But more still needs to be done to strengthen the Treaty\u2019s ability to protect human health, including a strong and binding Article 19. Calls for scientific certainty from those profiting from chemicals that they have placed on the market without testing must not distract us. We already know the risks. We must act now to protect us all from plastic chemicals that are increasingly ubiquitous across our workplaces, homes and natural environments.
