{"id":1804,"date":"2018-09-10T14:58:03","date_gmt":"2018-09-10T14:58:03","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmjebmspotlight\/?p=1804"},"modified":"2018-09-12T14:35:29","modified_gmt":"2018-09-12T14:35:29","slug":"first-do-no-harm-surgical-mesh","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmjebmspotlight\/2018\/09\/10\/first-do-no-harm-surgical-mesh\/","title":{"rendered":"First do no harm: surgical mesh"},"content":{"rendered":"<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400\"><img loading=\"lazy\" decoding=\"async\" class=\"size-thumbnail wp-image-1324 alignleft\" src=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2017\/06\/Carl-Heneghan-150x150.png\" alt=\"\" width=\"150\" height=\"150\" \/><\/span><\/p>\n<p>Last week, I learnt with great sadness of the <u><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-scotland-edinburgh-east-fife-45430625\">death of Eileen Baxter,<\/a><\/u> who died in an Edinburgh hospital in August. Mrs Baxter&#8217;s cause of death was multiple organ failure. Sacrocolpopexy mesh repair is named as an underlying cause on her death certificate, meaning the mesh triggered the chain of events leading to her death.<\/p>\n<p>Nicola Sturgeon, Scotland\u2019s First Minister, responding to a question raised in the Scottish Parliament said: &#8220;the use of mesh, other than in exceptional circumstances, remains under suspension in NHS Scotland\u2026..&#8221;We&#8217;ve seen the number of operations fall dramatically. In the six months to March this year there were 33 operations carried out. That compares to over 1,100 in a similar period in 2013\/14.&#8221;<\/p>\n<p>What Mrs Sturgeon didn&#8217;t understand was that she was acknowledging in 2013\/14 that at least 1000 women a year were undergoing unnecessary operations &#8211;\u00a0 the year Mrs Baxter had the mesh inserted &#8211;\u00a0 If you don&#8217;t need it now why would you need it then? You don\u2019t stop doing surgery if the benefits outweigh the harms, do you?<\/p>\n<p>The evidence base for surgical mesh is poor &#8211; actually, it\u2019s mostly missing &#8211; and there is a shortage of evidence about harms. I wrote about it <u><a href=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/2018\/07\/13\/uk-bans-transvaginal-mesh-heres-5-reasons-why\/\">here<\/a><\/u>: it\u2019s been 13 years since NICE stressed the need to inform women of the lack of long-term outcomes, and `<u><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/16034866\">Cochrane recommended<\/a><\/u> the need for studies with longer term follow up.<\/p>\n<p><a href=\"https:\/\/www.cebm.net\/2017\/12\/transvaginal-mesh-timeline\/\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-1805 aligncenter\" src=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/09\/mesh-timeline.jpg\" alt=\"\" width=\"500\" height=\"360\" srcset=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/09\/mesh-timeline.jpg 899w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/09\/mesh-timeline-300x216.jpg 300w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/09\/mesh-timeline-768x554.jpg 768w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/09\/mesh-timeline-640x461.jpg 640w\" sizes=\"auto, (max-width: 500px) 100vw, 500px\" \/><\/a><\/p>\n<p style=\"text-align: center\"><span style=\"font-weight: 400\">\u00a0<\/span><small><a href=\"https:\/\/www.cebm.net\/2017\/12\/transvaginal-mesh-timeline\/\">Transvaginal Mesh Timeline &#8211; CEBM<\/a><\/small><\/p>\n<p>In July 2011, the FDA released an <u><a href=\"https:\/\/www.fda.gov\/downloads\/medicaldevices\/safety\/alertsandnotices\/ucm262760.pdf\">Update on the Safety and Effectiveness of Transvaginal Placement for Pelvic Organ Prolapse\u00a0<\/a><\/u>and identified that serious concerns and adverse events were NOT rare, contrary to a previous 2008 FDA public health notice. Therefore, 2011 should have been the year Scotland &#8211; along with the rest of the world &#8211; took the harms of mesh seriously, began an inquiry and removed it from the market.<\/p>\n<p>However, this has not been the case. The lack of evidence has meant that significant numbers of women have been harmed, some of them with devastating consequences before action has been taken. The lack of trials and the lack of a mesh implant registry has meant the accumulating evidence of harm has come too late for many.<\/p>\n<p><em>Primum non nocere<\/em> means &#8220;first, to do no harm&#8221; &#8211; it is a philosophy ingrained in medical students but seemingly lost in the real world of clinical practice. It is better to do nothing than risk causing more harm than good. This is particularly relevant for mesh, given the absence of evidence pointed out by NICE, Cochrane and the FDA, along with the degradation and infection risks with polypropylene outlined by Donald Ostergard in a 2011 review in <u><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3112322\/\">the International Urogynaecology Journal <\/a><\/u>\u00a0(all of which had accumulated before the FDA decided to clear the majority of mesh products).<\/p>\n<p><b>Year: \u00a0Problem<\/b><\/p>\n<p><span style=\"font-weight: 400\">1987: <\/span> <span style=\"font-weight: 400\">At the time of mesh insertion there is a race to the mesh surface between bacteria and your defense cells.<\/span><\/p>\n<p><span style=\"font-weight: 400\">1991: <\/span> <span style=\"font-weight: 400\">Bacteria stick to the surfaces of the mesh and produce a film which protects them from the human immune system and antibiotics.<\/span><\/p>\n<p><span style=\"font-weight: 400\">1993: <\/span> <span style=\"font-weight: 400\">Multifilament mesh promotes infection.<\/span><\/p>\n<p><span style=\"font-weight: 400\">1996: \u00a0<\/span> <span style=\"font-weight: 400\">Bacteria move along synthetic polymeric fibers in mesh.<\/span><\/p>\n<p><span style=\"font-weight: 400\">1998: <\/span> <span style=\"font-weight: 400\">Polypropylene mesh shrinks 30-50% after 4 weeks.<\/span><\/p>\n<p><span style=\"font-weight: 400\">1999: <\/span> <span style=\"font-weight: 400\">Surface roughness promotes wicking of bacteria. Ten bacterial colony forming units are sufficient to infect up to 15% of the mesh.<\/span><\/p>\n<p><span style=\"font-weight: 400\">2000: <\/span> <span style=\"font-weight: 400\">Bacterial colonization is found in one third of surgically removed meshes .<\/span><\/p>\n<p><span style=\"font-weight: 400\">2001: <\/span> <span style=\"font-weight: 400\">smaller pores prevent vascularisation.<\/span><\/p>\n<p><span style=\"font-weight: 400\">2002: <\/span> <span style=\"font-weight: 400\">The extent the bacterial stick to the mesh depends on the mesh surface area. The bigger the are the more they stick. Multifilament meshes have twice the surface area compared to monofilament meshes.<\/span><\/p>\n<p><small><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/21512830\"><span style=\"font-weight: 400\">Degradation, infection and heat effects on polypropylene mesh for pelvic implantation: what was known and when it was known.<\/span><\/a><span style=\"font-weight: 400\"> Ostergard DR. Int Urogynecol J. 2011 Jul;22(7):771-4. doi: 10.1007\/s00192-011-1399-y.<\/span><\/small><\/p>\n<hr \/>\n<p>Given the issues outlined you would consider the bar for approval of mesh, the evidence needed for its use and the post-marketing surveillance requirements would have all been set at a very high bar &#8211; they weren\u2019t.
