I blogged about its use <\/span><a href=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/2017\/11\/28\/oxygen-heart-attack-practice-caught-evidence\/\"><span style=\"font-weight: 400\">here<\/span><\/a><span style=\"font-weight: 400\"> in MI, and we published in BMJ EBM in<\/span><a href=\"http:\/\/ebm.bmj.com\/content\/21\/1\/21\"><span style=\"font-weight: 400\">\u00a0patients without hypoxia in ST elevation MI oxygen increases myocardial injury and infarct size<\/span><\/a><span style=\"font-weight: 400\">. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Low oxygen saturations are common after stroke and its use has been shown to be associated with worse outcomes. Therefore, you\u2019d think that giving oxygen would help and prevent further brain damage in those with stroke.<\/span><\/p>\n<p><span style=\"font-weight: 400\">A <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK488051\/\"><span style=\"font-weight: 400\">UK randomised trial <\/span><\/a><span style=\"font-weight: 400\">assessed whether low-dose oxygen in 8,003 acute stroke patients recently admitted to hospital, who were not severely hypoxic, found that while supplementation did increase oxygen saturations, it made no difference in terms of the primary outcome of disability between those given continuous, nocturnal or no oxygen. <\/span><\/p>\n<p><span style=\"font-weight: 400\">In 1999, <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/10512903\"><span style=\"font-weight: 400\">a quasi-randomised trial<\/span><\/a><span style=\"font-weight: 400\"> design in 292 stroke patients in Norway concluded that supplemental oxygen should not routinely be given to non-hypoxic stroke victims with minor or moderate strokes. Why, then, \u00a0has it taken nearly 20 years to answer this question?<\/span><\/p>\n<p><span style=\"font-weight: 400\">The lack of effect also shows why we need many more randomised trials for essential practices where there is uncertainty, and why you shouldn\u2019t use observational studies to inform practice: <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20458088\"><span style=\"font-weight: 400\">A 2010 retrospective cohort<\/span><\/a><span style=\"font-weight: 400\"> study among 1,487 stroke patients identified three processes of care that were independently associated with improvements in outcomes: one of these three was treating all episodes of hypoxia with supplemental oxygen.<\/span><\/p>\n<p>We now know\u00a0not to use oxygen in those with heart attack or stroke who are not hypoxic (I didn&#8217;t\u00a0know this when I left medical school).\u00a0 What next then for oxygen and too much medicine?<\/p>\n<p><strong>References <\/strong><\/p>\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK488051\/\"><span style=\"font-weight: 400\">Routine low-dose continuous or nocturnal oxygen for people with acute stroke: Three-arm stroke oxygen supplementation RCT<\/span><\/a><span style=\"font-weight: 400\">.
