The Latin word was manus, from which we get words such as maintain, manacle, manage, man\u00e8ge, manicure, manipulate, manner, manoeuvre, mansuetude, manual, manufacture, [manu]factory, manumit, and manuscript. Manqu\u00e9, an adjective that refers to an unfulfilled ambition (e.g. a scientist manqu\u00e9), is from the Latin mancus, having a useless hand. To emancipate is to take someone by the hand (Latin capere, to take). An amanuensis (short for servus a manu plus the suffix -ensis, belonging to) writes things out by hand at another\u2019s dictation, as Tolstoy\u2019s wife, Sofya Andreevna, did for him and as Eric Fenby did for the composer Frederick Delius. The manubrium sterni is like a handle, comparing the body of the sternum (Greek \u03c3\u03c4\u03ad\u03c1\u03bd\u03bf\u03bd the chest) to the blade of a sword, and the xiphoid process (\u03be\u03af\u03d5\u03bf\u03c2 a sword) to its tip.<\/p>\n<p>Add Latin manus to [in]festus, hostile or troublesome, and you get manifestus, caught in the act, red-handed, and therefore obvious, manifest. As Dryden wrote in <em>Absalom and Achitophel<\/em>, \u201cNow, manifest of Crimes, contriv&#8217;d long since, He stood a bold Defiance with his Prince\u201d. A manifest thus became a piece of evidence, such as the list of a ship&#8217;s cargo or any inventory, and a manifesto, from the Italian form of the word, then became evidence of intention: \u201ca printed declaration, explanation, or justification of policy issued by a head of state, government, or political party or candidate.\u201d<\/p>\n<p>The earliest reference in English to a manifesto of this sort is to be found in Nathaniel Brent&#8217;s translation of Paolo Sarpi&#8217;s <em>History of the Council of Trent<\/em> (1620). Theodore Alois Buckley\u2019s history of 1852 makes it clear that manifestos were commonly compiled at the time of the Council, which was an<a href=\"https:\/\/en.wikipedia.org\/wiki\/Ecumenical_council\"> ecumenical council<\/a> of the<a href=\"https:\/\/en.wikipedia.org\/wiki\/Catholic_Church\"> Roman Catholic Church<\/a> held between 1545 and 1563. When, for example, the Emperor Charles V declared war on the Protestants in 1546, the elector of Saxony and the Landgrave of Hesse published a manifesto, declaring that \u201cthe war was really undertaken in the cause of religion\u201d. This laid at least some of the blame on the Council and the then Pope, Paul III (Alexander Farnese), with whom the Emperor had a treaty, although the Emperor had declared it to be a secular campaign, not wanting his wishes to be associated with those of the Pope.<\/p>\n<p><a href=\"http:\/\/evidencelive.org\/2017-registration-open\/\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1585\" src=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/Evidence-live-2-date.jpg\" alt=\"\" width=\"1928\" height=\"479\" srcset=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/Evidence-live-2-date.jpg 1928w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/Evidence-live-2-date-300x75.jpg 300w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/Evidence-live-2-date-768x191.jpg 768w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/Evidence-live-2-date-1024x254.jpg 1024w\" sizes=\"auto, (max-width: 1928px) 100vw, 1928px\" \/><\/a><\/p>\n<p>A manifesto is also \u201ca book or other work by a private individual supporting a cause\u201d, like Atul Gawande\u2019s<a href=\"https:\/\/www.goodreads.com\/book\/show\/6667514-the-checklist-manifesto\"> <em>Checklist Manifesto<\/em><\/a> (2009) and my 2010<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/20642541\"> manifesto for UK clinical pharmacology<\/a>. The 1848 political pamphlet <em>Manifest der Kommunistischen Partei<\/em> (<em>The Communist Manifesto<\/em>), by Karl Marx and Friedrich Engels, is perhaps the most famous example. Tsar Nicholas II issued the<a href=\"http:\/\/alphahistory.com\/russianrevolution\/october-manifesto\/\"> October Manifesto<\/a> in 1905, promising political reforms and aborting the first Russian Revolution. Artists are fond of publishing manifestos describing their creeds; well-known examples include the Russian<a href=\"https:\/\/helenbledsoe.com\/?p=238\"> Futurists\u2019 Manifesto<\/a>, <em>A Slap in the Face of Public Taste<\/em> (1912), and the<a href=\"http:\/\/poetsofmodernity.xyz\/POMBR\/French\/Manifesto.htm\"> Surrealists\u2019 Manifestos<\/a> (1924) by Andr\u00e9 Breton. There are<a href=\"http:\/\/www.1000manifestos.com\/list\/\"> many other examples<\/a> of varying seriousness.<\/p>\n<p>Searching PubMed for the term \u201cmanifesto\u201d in the titles of papers, I found nearly 250. The earliest all dealt with alcohol. The<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2511775\/pdf\/brmedj08205-0046.pdf\"> first<\/a>, published anonymously in the <em>British Medical Journal<\/em> in 1902, was titled \u201cA medical temperance manifesto\u201d and reported that the Council of the British Medical Temperance Association in conjunction with the American Medical Temperance Association and the Association of German-speaking Medical Men, had agreed to issue an international manifesto on abstinence from alcoholic beverages. Their manifesto was again<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2512596\/pdf\/brmedj08230-0047a.pdf\"> advertised<\/a> in the journal in 1903 and later that year in the<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1649059\/pdf\/calstatejmed00247-0038.pdf\"> California State Medical Journal<\/a>. The database lists no further manifestos until 1935, when the subject was again<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2461433\/pdf\/brmedj07586-0020b.pdf\"> alcohol<\/a>, again in the <em>British Medical Journal<\/em>, \u201cThe Brewers\u2019 Attack upon Youth\u201d, advertised in an article titled \u201cThe Beer Drinking Habit: Doctors\u2019 Manifesto\u201d; signed by \u201ca thousand medical men\u201d. This manifesto inveighed against a campaign by the Brewers\u2019 Society \u201cto get the Beer Drinking Habit instilled into Thousands, almost Millions, of Young Men who do not at present Know the Taste of Beer\u201d (punctuation in the original). The<a href=\"http:\/\/www.legislation.gov.uk\/asp\/2012\/4\/contents\"> 2012 legislation<\/a> in Scotland on minimum unit pricing,<a href=\"https:\/\/www.bma.org.uk\/collective-voice\/policy-and-research\/public-and-population-health\/alcohol\/minimum-unit-pricing?utm_source=The%20British%20Medical%20Association&amp;utm_medium=email&amp;utm_campaign=8921964_NEW11Z1%20MAIN%20ENEWSLETTER%2027.11.17&amp;utm_content=MUP%20heading&amp;dm_i=JVX,5B88C,7WMJK3,KHYHK,1\"> delayed<\/a> because of challenges from the Scottish Whisky Association, but now to be implemented from May 2018, could be considered the latest in this series of manifestos.This puts pressure on the rest of the UK to do likewise. The Welsh Parliament is already considering<a href=\"https:\/\/www.bma.org.uk\/news\/2017\/july\/bma-welcomes-minimum-alcohol-unit-pricing\"> legislation<\/a>.<\/p>\n<p>Since 1935, papers about manifestos have appeared sporadically. PubMed lists only 25 in the 84 years from 1902 to 1985. Since then the numbers have been variable (Figure 1), with a marked increase since 2010, although the numbers are still small. The main topics include public health, women\u2019s health, the UK\u2019s National Health Service, and various aspects of research and policy, including evidence-based medicine.<\/p>\n<p><a href=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/manifesto-nos-of-publications.png\"><img loading=\"lazy\" decoding=\"async\" class=\"size-full wp-image-1582 aligncenter\" src=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/manifesto-nos-of-publications.png\" alt=\"\" width=\"915\" height=\"541\" srcset=\"https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/manifesto-nos-of-publications.png 915w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/manifesto-nos-of-publications-300x177.png 300w, https:\/\/blogs.bmj.com\/bmjebmspotlight\/files\/2018\/01\/manifesto-nos-of-publications-768x454.png 768w\" sizes=\"auto, (max-width: 915px) 100vw, 915px\" \/><\/a><\/p>\n<p><strong>Figure 1.<\/strong> The numbers of papers containing the word \u201cmanifesto\u201d in titles since 1986 (source PubMed); the 15 papers published in 1997 included a series of 12 titled \u201cOsteopathic manifesto.\u201d<\/p>\n<p>The purpose of the EBM manifesto is to develop, disseminate, and implement better evidence for better healthcare in response to systematic bias, wastage, error, and fraud in research underpinning patient care. Its nine principles are to:<\/p>\n<ul>\n<li>expand the roles of patients, health professionals, and policymakers in research;<\/li>\n<li>increase the systematic use of existing evidence;<\/li>\n<li>make research evidence relevant, replicable, and accessible to end users;<\/li>\n<li>reduce questionable research practices, bias, and conflicts of interests;<\/li>\n<li>ensure that drug and device regulation is robust, transparent, and independent;<\/li>\n<li>produce better usable clinical guidelines;<\/li>\n<li>support innovation, quality improvement, and safety through the better use of real-world data;<\/li>\n<li>educate professionals, policymakers, and the public in evidence-based healthcare, so that they can make informed choices;<\/li>\n<li>encourage the next generation of leaders in evidence-based medicine.<\/li>\n<\/ul>\n<p>During elections candidates or their parties may issue manifestos in which they outline their plans, which, if they are elected, they have a<a href=\"https:\/\/blogs.bmj.com\/bmj\/2016\/05\/13\/jeff-aronson-mandates-and-manifestos\"> mandate<\/a> to carry out. If they do not do so, or if they do things the manifestos did not include, they can be accused of abusing their mandate. Few people, I suspect, read political manifestos, which often contain<a href=\"https:\/\/blogs.bmj.com\/bmj\/2016\/04\/29\/jeffrey-aronson-when-i-use-a-word-weasel-words\"> weasel words<\/a> and vague, platitudinous promises.
