{"id":9312,"date":"2011-06-09T15:02:50","date_gmt":"2011-06-09T14:02:50","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=9312"},"modified":"2011-06-09T16:48:53","modified_gmt":"2011-06-09T15:48:53","slug":"tony-delamothe-falling-sperm-count-saga-an-update","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2011\/06\/09\/tony-delamothe-falling-sperm-count-saga-an-update\/","title":{"rendered":"Tony Delamothe &#8211; Falling sperm count saga: an update"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft\" style=\"float: left\" src=\"http:\/\/www.bmj.com\/site\/blog\/icons\/td.jpg\" alt=\"Tony Delamothe\" width=\"160\" height=\"110\" \/>Very few BMJ original research articles are cited by 1600 other publications, so it\u2019s dispiriting to discover that the message of one of our citation classics may have been wrong.<\/p>\n<p><a href=\"http:\/\/www.bmj.com\/content\/305\/6854\/609.full.pdf\">A\u00a0meta-analysis by Neils Skakkebaek and colleagues<\/a> reported in 1992 that sperm counts had halved over the preceding 50 years. It became big news, no doubt chiming with contemporary anxieties about \u201cthe crisis of masculinity\u201d and the ill effects of endocrine disrupters (eg the Everglades overrun by hermaphrodite \u2018gators\u2019). Who knows:\u00a0 the resulting concern about falling sperm counts might have contributed to the shift from tighty-whities to boxer shorts.<!--more--><\/p>\n<p>Now the same research team have come up with a much stronger study design. Semen samples from 5000 18 year old Danish military recruits were analysed over a 15 year period.\u00a0 Their reassuring findings:\u00a0 whichever way you tilt your head at the graph of the average sperm counts against year it all seems remarkably stable. So maybe falling sperm counts\u00a0is one less thing we should be frightened of.<\/p>\n<p>To allow you to make up your own mind, I would normally be directing you to the research paper in a peer reviewed journal, (and logically the authors might have considered the BMJ) but the findings appear in a <a href=\"http:\/\/journals.lww.com\/epidem\/Citation\/publishahead\/Trends_in_Sperm_Counts__The_Saga_Continues.99558.pdf\">commentary in <em>Epidemiology<\/em><\/a>.\u00a0<\/p>\n<p><a href=\"http:\/\/healthland.time.com\/2011\/06\/07\/no-decline-in-sperm-counts-after-all-danish-data-show\/\">According to Time magazine<\/a> the process went something like this: \u201c The data wasn&#8217;t released by Dr. Niels Erik Skakkebaek of the University of Copenhagen, who initiated the study; he hasn&#8217;t commented yet on the results, pending publication in a scientific journal. Instead, the research group&#8217;s current leader, Niels Jorgensen, sent the data to the Danish Ministry of Health, which helped fund the study. The journal <em>Epidemiology<\/em> then got the data off the health ministry&#8217;s website and published it in a commentary.&#8221;<\/p>\n<p>Whatever the editor\u2019s justification for his actions it looks like sharp practice to me. In his accompanying editorial, Allen Wilcox asks who owns data generated with public funds, while admitting\u00a0 that the researchers didn\u2019t want to release their findings before they had at least 20 years of results. \u201cThe presentation of a few raw data on a Web site\u2014 or in a commentary\u2014is hardly the preferred way to advance science,\u201d he writes.\u00a0 \u201cBut neither is it acceptable for valuable data to be held in storage.\u201d<\/p>\n<p>You could argue, with some justification,\u00a0 that the bald graph reproduced in <em>Epidemiology<\/em> advances science not one jot. It\u2019s hard to imagine what would have been lost if the researchers who\u2019d been collecting the data published them in their own good time and in sufficient detail to allow proper interpretation.<\/p>\n<p>Meanwhile, spare a thought for the subjects who volunteered \u201ca specimen.\u201d If I understand <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pubmed\/17588567\">reference 6<\/a> to the <em>Epidemiology<\/em> commentary correctly, the 18 year olds also had\u00a0their testicular volumes\u00a0measured and completed a questionnaire on self rated health.<\/p>\n<p>I can\u2019t quite see military recruits in the UK or US readily agreeing to this level of intrusion in the interests of science, but then <a href=\"http:\/\/www.bmj.com\/content\/333\/7582\/1289.full\">Danes are the happiest people on earth<\/a> and its young men were presumably \u201ccool\u201d about it.<\/p>\n<p><em><strong>Tony Delamothe<\/strong> is deputy editor, BMJ.<\/em><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Very few BMJ original research articles are cited by 1600 other publications, so it\u2019s dispiriting to discover that the message of one of our citation classics may have been wrong.
