{"id":924,"date":"2009-12-10T14:52:57","date_gmt":"2009-12-10T13:52:57","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=924"},"modified":"2009-12-10T14:52:57","modified_gmt":"2009-12-10T13:52:57","slug":"olivia-roberts-on-the-migration-merry-go-round-debate","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2009\/12\/10\/olivia-roberts-on-the-migration-merry-go-round-debate\/","title":{"rendered":"Olivia Roberts on the migration merry go round debate"},"content":{"rendered":"<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft\" style=\"float: left\" src=\"http:\/\/www.bmj.com\/columns\/icons\/olivia_roberts.jpg\" alt=\"\" width=\"160\" height=\"110\" \/> Last week the <a title=\"BMA\" href=\"http:\/\/www.bma.org.uk\/\" target=\"_blank\">British Medical Association<\/a> (BMA) published an updated <a title=\"BMA statement\" href=\"http:\/\/www.bma.org.uk\/international\/international_development\/migration\/migrationstatement.jsp?page=1\" target=\"_blank\">statement<\/a> on the international migration of health workers. In the BMA\u2019s view individuals have the right to migrate for personal and professional reasons. As <a title=\"Michael Clemens\" href=\"http:\/\/www.cgdev.org\/content\/publications\/detail\/1422684\" target=\"_blank\">Michael Clemens<\/a> writes in his paper for the Center for Global Development from August 2009, health workers, educated and trained at the public expenses or through their own means, are not a &#8220;statist property right.&#8221; They are individuals with the right to seek to improve their own lives, professionally and personally.<!--more--><\/p>\n<p>Data on developing country migration is sorely lacking, but what we do have doesn\u2019t show a strong correlation between number of migrants leaving a country and health worker numbers, health care system quality or health outcomes. Countries have poor health outcomes for a myriad of reasons, not solely due to migration. What is also absent from the debate is discussion of migration as a livelihood strategy for the <a title=\"mobilisation\" href=\"http:\/\/www.migrationdrc.org\/research\/projects\/project4a.html\" target=\"_blank\">poor<\/a>\u00a0 \u2013 it is possible that stopping people moving to countries with the aim of improving prospects for themselves and their families will have a starker impact on development and health outcomes. Remittances are also a vital source of income for many developing countries, and <a title=\"research\" href=\"http:\/\/www.un.org\/esa\/population\/meetings\/EGM_Ittmig_Arab\/P01_Adams.pdf\" target=\"_blank\">research<\/a> shows that these are spent on capital-intensive \u2018extras\u2019 such as education and healthcare .<\/p>\n<p>The arguments about the costs of migration are showing their age and &#8220;brain drain \u2013 it\u2019s a bad thing for health systems&#8221; has proved too simple when it comes to developing policy solutions. The BMA feels that policy solutions should focus on countries with shortages of health workers.<\/p>\n<p>The UK has had a <a title=\"DH code of conduct\" href=\"http:\/\/www.dh.gov.uk\/en\/Publicationsandstatistics\/Pressreleases\/DH_4011389\" target=\"_blank\">code of conduct<\/a> (albeit voluntary and only applicable to the public sector) in place since 2001. But now the <a title=\"WHO\" href=\"http:\/\/www.who.int\/en\/\" target=\"_blank\">World Health Organization<\/a> is developing its <a title=\"WHO code\" href=\"http:\/\/apps.who.int\/gb\/ebwha\/pdf_files\/EB124\/B124_13-en.pdf\" target=\"_blank\">own code\u00a0<\/a> and the global discussions are taking an interesting turn. Issues under debate include compensation (despite being impractical, unquantifiable and tough to implement), or visas which are only granted if workers promise to leave after a few years and not to return. But where they go from there is, rightly, up to the individual to decide \u2013 no code can, or should, aim to regulate individual choice in this way.<\/p>\n<p>The BMA feels more should be done to support health workers here who chose to migrate. It is a win-win-win argument: the individual benefits by receiving support and being able to access decent educational and career opportunities. The health system benefits by helping its staff\u00a0 make their transition into their new workplace without problems. And patients benefit by having staff who are up to speed on NHS policies and practices.<\/p>\n<p>Let\u2019s accept the fact that people do migrate \u2013 within regions or to neighbouring countries most of all. The BMA feels that energy should be spent supporting the health professionals who migrate and the health systems with shortages, rather than trying to stop the movement or only on terms that benefit developed countries. Supply will inevitably respond to demand.
