This sophisticated Taiwanese laboratory <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/11\">study <\/a>identifies a five-gene signature which predicts good outcome in <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/11\">non-small-cell lung cancer<\/a>. Good discussion in the accompanying <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/76\">editorial<\/a>.<!--more--><\/p>\n<p>A couple of years ago in Minnesota, there was an outbreak of multidrug resistant <em><a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/21\">Salmonella enterica<\/a> <\/em>serotype Typhimurium. The source turned out to be <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/21\">pet hamsters<\/a> (and one mouse) bought from a certain pet shop which, like many, feeds antibiotics to its rodents. When you next visit family or friends in Minnesota, do not volunteer to clean out the hamster cage, and regard tame mice with suspicion.<\/p>\n<p>The ergot-derived dopamine agonists <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/29\">pergolide <\/a>and <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/29\">cabergoline <\/a>are quite widely used to treat Parkinson\u2019s disease and occasionally restless legs syndrome. This <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/29\">study <\/a>uses the UK GP Research Database to explore a possible connection between these drugs and <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/29\">cardiac valve regurgitation<\/a>. The association is definite, and judging from the Italian echocardiographic study which follows on p.39, affects about a quarter of those taking these drugs.<\/p>\n<p>A nice <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/47\">summary <\/a>of all you need to know about primary <a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/47\">PCI for S-T elevation myocardial infarction<\/a>, with a video clip too on the website.<\/p>\n<p><a href=\"http:\/\/content.nejm.org\/cgi\/content\/full\/356\/1\/55\">Whipple <\/a>first described his disease in 1907, and there have only been 1,000 cases reported since. As it\u2019s a very odd bacterial disease which affects the gut, lungs, joints, heart and CNS, it\u2019s quite possible that we miss it now and again, which would be a pity, since <em>Tropheryma whipplei <\/em>responds to antibiotics; otherwise, the patient dies.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Gene signatures in cancer cells are beginning to tell us which patients are most likely to survive.
