{"id":51083,"date":"2021-10-05T16:29:40","date_gmt":"2021-10-05T15:29:40","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=51083"},"modified":"2021-10-05T16:29:40","modified_gmt":"2021-10-05T15:29:40","slug":"unpacking-vaccine-hesitancy-the-spectrum-of-vaccine-acceptance","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/10\/05\/unpacking-vaccine-hesitancy-the-spectrum-of-vaccine-acceptance\/","title":{"rendered":"Unpacking vaccine \u201chesitancy:\u201d the spectrum of vaccine acceptance"},"content":{"rendered":"<p><a href=\"https:\/\/www.bloomberg.com\/news\/articles\/2021-07-02\/u-k-finds-greater-covid-19-vaccine-hesitancy-in-young-people\"><span style=\"font-weight: 400\">Media reports<\/span><\/a><span style=\"font-weight: 400\"> investigating the challenges that lead to relatively low covid-19 vaccine uptake rates in younger age groups highlight a need to consider \u201cvaccine hesitancy\u201d not as a single phenomenon, but as <\/span><a href=\"https:\/\/www.researchgate.net\/publication\/353324967_THE_SPECTRUM_OF_VACCINE_ACCEPTANCE\"><span style=\"font-weight: 400\">part of a spectrum<\/span><\/a><span style=\"font-weight: 400\">. It can range from <\/span><i><span style=\"font-weight: 400\">vaccine apathy<\/span><\/i><span style=\"font-weight: 400\"> at one end\u2014driven by lack of interest in a vaccine that mitigates a risk one perceives to be low\u2014to full-blown <\/span><i><span style=\"font-weight: 400\">vaccine conspiracy<\/span><\/i><span style=\"font-weight: 400\"> at the other. Most people are happy to observe vaccine <\/span><i><span style=\"font-weight: 400\">acceptance<\/span><\/i><span style=\"font-weight: 400\">, driven by a rational desire to mitigate risks from covid-19, by accepting a vaccine that evidence makes clear we should trust. But not everyone trusts healthcare systems to have developed vaccines safely, often because of past historical <\/span><span style=\"font-weight: 400\">transgressions<\/span><span style=\"font-weight: 400\"> or because they are concerned about <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6784763\/\"><span style=\"font-weight: 400\">pharmaceutical companies that have<\/span><\/a><span style=\"font-weight: 400\"> lied in the past. They may be worried about vaccine ingredients or side effects. Some pharmaceutical companies might be trusted more or less simply on the basis of their location, history, and commitment to making profits or not. Some people may just want to see what happens to others first, indicating a vaccine <\/span><i><span style=\"font-weight: 400\">questioning <\/span><\/i><span style=\"font-weight: 400\">or <\/span><i><span style=\"font-weight: 400\">hesitancy<\/span><\/i><span style=\"font-weight: 400\"> that falls short of vaccine <\/span><i><span style=\"font-weight: 400\">refusal<\/span><\/i><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Not everyone who is not ready to have a vaccine is an \u201canti-vaxxer\u201d and only by acknowledging this, working with diverse communities and understanding the range of people\u2019s concerns are we likely to win them over to the side of public health interventions.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Our research\u2014which ranges from the streets of <\/span><a href=\"https:\/\/pure.royalholloway.ac.uk\/portal\/files\/42084849\/COVID_19_in_Slough_Final_Report_V2.pdf\"><span style=\"font-weight: 400\">Slough<\/span><\/a><span style=\"font-weight: 400\">, just a few miles from Royal Holloway\u2019s Egham campus, to <\/span><a href=\"https:\/\/medrxiv.org\/cgi\/content\/short\/2021.06.15.21258948v1\"><span style=\"font-weight: 400\">Armenia<\/span><\/a><span style=\"font-weight: 400\">, Uganda, and the digital spaces of <\/span><a href=\"https:\/\/dl.acm.org\/doi\/abs\/10.1145\/3411763.3450396\"><span style=\"font-weight: 400\">reddit<\/span><\/a><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">suggests that not only are attitudes to vaccines context-specific, but also, as <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0140673611606788?casa_token=IoTo0Awr2cgAAAAA:YWrb2jtc5IcFC9i6FeBZTDTcXFlXldKrACbSrZ-MkuZ88-FsPArcqpKM1FIDvzsWyoJ30G1lFNI\"><span style=\"font-weight: 400\">other studies<\/span><\/a><span style=\"font-weight: 400\"> have suggested, acutely heterogeneous: a vaccine may be accepted for one disease, but rejected for another; attitudes may differ between demographic groups within the same population; and may change at different points in time. Rather than seeing vaccine hesitancy as a single challenge, we need to better understand the broad range of reasons why people may be cautious, so that we can best suggest targeted interventions that speak to their specific concerns and circumstances. We need to see attitudes to vaccines as a <\/span><a href=\"https:\/\/www.researchgate.net\/publication\/353324967_THE_SPECTRUM_OF_VACCINE_ACCEPTANCE\"><span style=\"font-weight: 400\">broad spectrum<\/span><\/a><span style=\"font-weight: 400\"> that ranges from apathy, through acceptance, questioning, hesitancy, refusal and conspiracy, each of which needs to be tackled independently.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Hesitancy to new vaccines that have been developed quickly, rather than to vaccines <\/span><i><span style=\"font-weight: 400\">per se, <\/span><\/i><span style=\"font-weight: 400\">is not an irrational concern. Communities may be concerned over the ingredients they may contain, as a recent article in <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41591-021-01454-y\"><span style=\"font-weight: 400\">Nature<\/span><\/a><span style=\"font-weight: 400\"> recognised. The vaccines themselves, and their delivery, may be infused with a gamut of <\/span><a href=\"https:\/\/pure.qub.ac.uk\/en\/publications\/interventions-in-critical-health-geopolitics-borders-rights-and-c\"><span style=\"font-weight: 400\">geopolitical and post-colonial legacies<\/span><\/a><span style=\"font-weight: 400\">, particularly where old rivals compete in a world where there is, in fact, more than enough space for many victors. If <\/span><a href=\"https:\/\/rusi.org\/explore-our-research\/publications\/rusi-newsbrief\/the-coronavirus-pandemic-global-challenges-require-global-cooperation\"><span style=\"font-weight: 400\">Russia and the USA,<\/span><\/a><span style=\"font-weight: 400\"> for example, were more willing to see one another as allies and collaborators in the fight against covid-19, they might be less likely to sow seeds of distrust about the efficacy of one another\u2019s vaccines. Such an approach does little other than destroy confidence in either\u2019s product in countries such as Armenia and Georgia, caught in the middle of the old East and Western rivalries. The European Union\u2019s relationship to the UK and the Oxford\/Astra-Zeneca vaccine has been, at times, <\/span><a href=\"https:\/\/www.politico.eu\/article\/astrazeneca-pascal-soriot-uk-priority-access-coronavirus-vaccine\/\"><span style=\"font-weight: 400\">fraught with accusations<\/span><\/a><span style=\"font-weight: 400\"> traded about supply and demand, provenance and efficacy.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Resisting the temptation to frame the vaccine hesitant as either ignorant or susceptible to conspiracy theories would allow us to better understand other factors that may be relevant, such as vaccines containing religiously prohibited ingredients (most don\u2019t, which is easily explained). It could also be the case that vaccination centres are too geographically distant for those who do not own their own car (in which case, community healthcare workers could vaccinate the vulnerable in their own homes). Finally, vaccination progress will be harder to access for those who are not registered, and may not be able to easily register, with a GP surgery. Such concerns\u2014all of which were raised by citizens we surveyed in <\/span><a href=\"https:\/\/pure.royalholloway.ac.uk\/portal\/files\/42084849\/COVID_19_in_Slough_Final_Report_V2.pdf\"><span style=\"font-weight: 400\">Slough<\/span><\/a><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">highlight challenges not only with the vaccines themselves, but with the medical-public health systems through which they are delivered and which may need to be reconfigured to meet the needs of the local context.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In <\/span><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2021.06.15.21258948v1\"><span style=\"font-weight: 400\">Armenia<\/span><\/a><span style=\"font-weight: 400\">, hesitancy varied not only from vaccine to vaccine (physicians who readily promote childhood vaccinations for measles, polio, and other diseases, showed hesitancy towards both the recently introduced Gardasil vaccine for HPV, as well as the covid-19 vaccines), but also between different age groups, and at different times. Those who showed concerns about newness\u2014a concern also highlighted in the recent <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41591-021-01454-y\"><span style=\"font-weight: 400\">Nature<\/span><\/a><span style=\"font-weight: 400\"> report\u2014may become less hesitant over time, as their fears are not realised. Such concerns can and should be addressed directly by promoting experiences of those who have already received the vaccine safely, as a <\/span><a href=\"https:\/\/www.reddit.com\/r\/COVID19_support\/comments\/jw587x\/im_a_participant_in_the_pfizer_vaccine_trial_ama\/\"><span style=\"font-weight: 400\">reddit forum we moderate<\/span><\/a><span style=\"font-weight: 400\"> did, by running a question-and-answer session in which vaccine trial volunteers discussed the relatively mild side-effects they had experienced before the vaccine was rolled out to the wider public. Giving voices to healthcare workers, who tend to be both <\/span><a href=\"https:\/\/opendocs.ids.ac.uk\/opendocs\/bitstream\/handle\/20.500.12413\/16580\/Data%20Synthesis_Covid19%20Vaccine%20Perceptions%20Africa_Social_Behavioural%20Science%20Data%20Mar%202020-Mar%202021.pdf?sequence=3&amp;isAllowed=y\"><span style=\"font-weight: 400\">early recipients of in-country vaccine supplies<\/span><\/a><span style=\"font-weight: 400\"> and also highly trusted by their communities, provides opportunities to bolster community confidence. <\/span><a href=\"https:\/\/www.jmir.org\/2015\/5\/e133\/\"><span style=\"font-weight: 400\">Evidence shows<\/span><\/a><span style=\"font-weight: 400\"> this is a much more effective response than pushing the vaccine hesitant away. Concerns over the vaccines for covid-19 are no more universal than the reasons behind enduring resistance to <\/span><a href=\"https:\/\/www.georgetownjournalofinternationalaffairs.org\/online-edition\/the-failure-of-polio-eradication-blame-geopolitics-not-religion\"><span style=\"font-weight: 400\">polio eradication<\/span><\/a><span style=\"font-weight: 400\">, a war that may <\/span><a href=\"https:\/\/polioeradication.org\/polio-today\/polio-now\/this-week\/\"><span style=\"font-weight: 400\">have been won<\/span><\/a><span style=\"font-weight: 400\"> while the world\u2019s attention has been distracted by a different pandemic.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The policy implications for adopting a more nuanced approach to \u201cvaccine hesitancy\u201d might in itself reveal further potential options for making a difference to vaccination levels. In <\/span><a href=\"https:\/\/www.timesofisrael.com\/care-for-a-shot-with-your-shot-tel-aviv-sets-up-vaccine-station-in-local-bar\/\"><span style=\"font-weight: 400\">Israel<\/span><\/a><span style=\"font-weight: 400\"> progress was accelerated by vaccinating the young and healthy in bars, in exchange for a free soft drink, as did offering lottery tickets alongside vaccination in the <\/span><a href=\"https:\/\/www.scientificamerican.com\/article\/from-1-million-lotteries-to-free-beer-do-covid-vaccination-incentives-work1\/\"><span style=\"font-weight: 400\">US<\/span><\/a><span style=\"font-weight: 400\">). Covid-19 continues to provide a chance to study, unpack, and understand how and why vaccine hesitancy arises and resonates in some individuals and communities. It will help us understand how to frame messages around other urgent policy priorities such as the c<\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/pmc7856368\/\"><span style=\"font-weight: 400\">limate <\/span><\/a><span style=\"font-weight: 400\">emergency<\/span><span style=\"font-weight: 400\">, which is predicted to make such pandemics <\/span><a href=\"https:\/\/esajournals.onlinelibrary.wiley.com\/doi\/full\/10.1890\/08-0079.1?casa_token=LBMbT5RXm9UAAAAA%3AeSIPdpEE37nB1YpDkUEgXfLsZN8mUuP_17D3x5IuLEi_Xjx6Z1UpVp4cDquRZ4okZPQf9Lf-a8JN6EeB\"><span style=\"font-weight: 400\">more common in the future<\/span><\/a><span style=\"font-weight: 400\"> as well as being a challenge in its own right.
