{"id":50979,"date":"2021-09-15T16:53:02","date_gmt":"2021-09-15T15:53:02","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=50979"},"modified":"2021-09-29T11:13:31","modified_gmt":"2021-09-29T10:13:31","slug":"learning-from-covid-19-to-improve-health-information-for-south-asian-communities","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/09\/15\/learning-from-covid-19-to-improve-health-information-for-south-asian-communities\/","title":{"rendered":"Learning from covid-19 to improve health information for South Asian communities"},"content":{"rendered":"<p><span style=\"font-weight: 400\">Covid-19 has had a disproportionate impact on ethnic minority populations, including South Asian communities in the UK, and highlighted the urgent need for <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1548\"><span style=\"font-weight: 400\">culturally appropriate health information<\/span><\/a><span style=\"font-weight: 400\">. Written and multimedia information was produced at pace during the pandemic, amid an absence of guidance on optimal ways of developing culturally sensitive health messaging for covid-19. Short videos in a range of South Asian languages were produced on numerous topics and disseminated on social media, on WhatsApp, and via community organisations. Much of this work was led by <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m2282\/rr-6\"><span style=\"font-weight: 400\">South Asian healthcare professionals and organisations<\/span><\/a><span style=\"font-weight: 400\">. We must learn from this and develop culturally relevant health information for all conditions that disproportionately affect South Asians, including hypertension, type 2 diabetes, asthma, and depression. This would help reduce inequalities in health outcomes seen before the pandemic and exacerbated during it.\u00a0<\/span><\/p>\n<p><i><span style=\"font-weight: 400\">Lessons from covid-19 health messaging<\/span><\/i><\/p>\n<p><span style=\"font-weight: 400\">R<\/span><span style=\"font-weight: 400\">esearch into the best ways of developing culturally relevant health information resources is lacking. <\/span><a href=\"https:\/\/europepmc.org\/article\/PPR\/PPR321080\"><span style=\"font-weight: 400\">Recommendations for developing<\/span><span style=\"font-weight: 400\"> a tailored health communication strategy<\/span><\/a><span style=\"font-weight: 400\"> for ethnic minority communities during the pandemic included co-producing and pre-testing health messages that personalise experiences and target specific community beliefs; delivering messages by credible, trusted sources; and using language that retains cultural context and meaning to increase understanding. <\/span><span style=\"font-weight: 400\">Recommendations also included using <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7380298\/\"><span style=\"font-weight: 400\">community engagement<\/span><\/a><span style=\"font-weight: 400\"> to understand the needs of the target community. Guidance specifically about covid<\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/factors-influencing-covid-19-vaccine-uptake-among-minority-ethnic-groups-17-december-2020\"><span style=\"font-weight: 400\">-19 vaccine uptake<\/span><\/a> <span style=\"font-weight: 400\">highlighted the importance of <\/span><span style=\"font-weight: 400\">multilingual, non-stigmatising communications with ethnic minority communities, and using educational videos in multiple languages to increase awareness, sharing these with individuals within family and community networks. <\/span><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2020.12.22.20248259v1.full\"><span style=\"font-weight: 400\">Misinformation can cause anxiety<\/span><\/a><span style=\"font-weight: 400\">, highlighting the need for clear, honest, and sensitive information. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5894650\/\"><span style=\"font-weight: 400\">Lack of culturally appropriate information<\/span><\/a><span style=\"font-weight: 400\"> can result in fear of Western medications, leading patients to seek advice on traditional medicines.<\/span><\/p>\n<p><i><span style=\"font-weight: 400\">Recommendations from previous research\u00a0\u00a0\u00a0<\/span><\/i><\/p>\n<p><span style=\"font-weight: 400\">Before<\/span><span style=\"font-weight: 400\"> the pandemic, research focusing on South Asian communities emphasised the need for health information to be made available mainly via video, rather than leaflets, to overcome potential problems with literacy. Culturally sensitive educational videos for South Asian communities have been produced on <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5894650\/\"><span style=\"font-weight: 400\">biologic medications in rheumatoid arthritis<\/span><\/a><span style=\"font-weight: 400\">,<\/span> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5330360\/\"><span style=\"font-weight: 400\">coronary artery disease<\/span><\/a><span style=\"font-weight: 400\"> (aimed at Bangladeshi women), and <\/span><a href=\"https:\/\/doi.org\/10.1016\/j.pec.2015.04.018\"><span style=\"font-weight: 400\">insulin treatment for South Asian patients<\/span><\/a><span style=\"font-weight: 400\"> with type 2 diabetes. When evaluated, these resources showed statistically significant improvement in knowledge, changes in patients\u2019 attitudes and understanding, and an improvement in consultations between patients and health professionals.<\/span><\/p>\n<p><i><span style=\"font-weight: 400\">Developing health information for South Asian groups\u00a0<\/span><\/i><\/p>\n<p><span style=\"font-weight: 400\">The <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/spi-b-consensus-on-bame-communication-22-july-2020\"><span style=\"font-weight: 400\">July 2020 Scientific Advisory Group for Emergencies report<\/span><\/a><span style=\"font-weight: 400\"> on public health messaging for ethnic minority communities recommended qualitative engagement work with minority groups to understand how government messages regarding covid-19 were received and what their impacts were. Learning from, and building on, the <\/span><a href=\"https:\/\/bjgplife.com\/2021\/03\/31\/will-reached-communities-become-hard-to-reach-again-after-the-covid-19-vaccine-rollout\/\"><span style=\"font-weight: 400\">substantial financial and community initiatives<\/span><\/a><span style=\"font-weight: 400\"> taken\/started during the pandemic is vital to improving health inequalities. We conducted five online community engagement workshops with British South Asian communities to learn about views and experiences of covid-19 health messaging, and lessons for developing health information resources for South Asian groups after covid. Fifty patient and public contributors took part in the workshops held in February-March 2021. Key learning from these discussions is set out below [<\/span><a href=\"https:\/\/secureservercdn.net\/160.153.138.177\/7xh.81e.myftpupload.com\/wp-content\/uploads\/2021\/06\/Coproduction-and-health-information-for-British-South-Asians.pdf?time=1623844540\"><span style=\"font-weight: 400\">see full report<\/span><\/a><span style=\"font-weight: 400\">].\u00a0\u00a0<\/span><\/p>\n<ol>\n<li><span style=\"font-weight: 400\"> Health information in a range of formats (written, audio, video) can be helpful in urgent situations such as the covid-19 pandemic. For long term conditions, participants prefer multilingual educational videos\u00a0 to written information because videos can be understood by community members regardless of their education and literacy levels.\u00a0\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Educational videos aimed at South Asian communities need to be co-designed and co-produced with patients, health professionals, and researchers to ensure relevance and acceptability.<\/span><\/li>\n<li><span style=\"font-weight: 400\"> People value having evidence based information from healthcare professionals, examples of patients\u2019 experiences, information about the mental health impacts of a long term condition, and sources of support. Evidence based medical information communicated by healthcare professionals needs to be simple, clear, and consistent. Examples of patients\u2019 experiences need to include a wide range of perspectives reflecting the diversity within and across communities (such as age, education, religion, migration history).\u00a0\u00a0\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Evaluating co-produced educational resources with patients, healthcare professionals, and other stakeholders can help ensure that the content included is accurate, sensitive, appropriate, and unlikely to cause fear or anxiety.\u00a0\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400\"> Educational resources for South Asian communities need to be disseminated through as wide a range of credible sources as is practical. Dissemination through healthcare settings includes showing educational videos in clinic waiting areas, referring patients to resources aimed at South Asian communities, and having posters advertising these resources in GP surgeries and community pharmacies.\u00a0\u00a0\u00a0<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400\">Dedicated funding is needed to co-produce high quality resources based on rigorous research and conducted in a range of languages. Working with local communities and partners can help ensure that resources reach those that need them most.
