Like you, and in agreement with the <\/span><a href=\"https:\/\/www.unicef.org\/ukraine\/en\/press-releases\/all-schools-europe-central-asia-should-remain-open-and-made-safer-covid-19-say-who\"><span style=\"font-weight: 400\">World Health Organization<\/span><\/a><span style=\"font-weight: 400\"> (WHO), we recognise the importance of schools staying open over the autumn and in the longer term. However, as the WHO also notes, schools must be made safe by adopting measures to minimise transmission of the SARS-CoV-2 virus. We are therefore writing to express our concerns about the lack of mitigations for children and educational staff, and the subsequent risk to children from covid-19 as schools reopen in England this September. We offer nine evidence based recommendations to reduce new infections in children.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Children have suffered significant harms from covid-19. In just the past two months there have been <\/span><a href=\"https:\/\/t.co\/JUxFY63THw?amp=1\"><span style=\"font-weight: 400\">over 2300 hospitalisations<\/span><\/a><span style=\"font-weight: 400\"> of under 18s in England. There are an estimated 34 000 children living with long covid in the UK already, with 22 000 reporting significant impacts on their day to day activities, 7000 of whom have had symptoms for more than a year (<\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/bulletins\/prevalenceofongoingsymptomsfollowingcoronaviruscovid19infectionintheuk\/5august2021\"><span style=\"font-weight: 400\">Office for National Statistics<\/span><\/a><span style=\"font-weight: 400\">). Up to <\/span><a href=\"https:\/\/www.researchsquare.com\/article\/rs-798316\/v1\"><span style=\"font-weight: 400\">one in seven<\/span><\/a><span style=\"font-weight: 400\"> of those infected are expected to have persisting symptoms at <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/bulletins\/prevalenceofongoingsymptomsfollowingcoronaviruscovid19infectionintheuk\/1april2021\"><span style=\"font-weight: 400\">12-15 weeks<\/span><\/a><span style=\"font-weight: 400\">. Long covid can be associated with multisystem disease in some children, including persistent cognitive symptoms. SARS-CoV-2 is a neurotrophic and pro-inflammatory virus with neuroinvasive potential that can infect the brain, with <\/span><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2021.06.11.21258690v3\"><span style=\"font-weight: 400\">structural brain changes<\/span><\/a><span style=\"font-weight: 400\"> having been observed in adults, including those with mild infection. Allowing mass infection of children is therefore reckless.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The evidence from <\/span><a href=\"https:\/\/www.thetimes.co.uk\/article\/scots-pupils-face-more-disruption-as-covid-persists-q6cx9x07g\"><span style=\"font-weight: 400\">Scotland<\/span><\/a><span style=\"font-weight: 400\"> and the <\/span><a href=\"https:\/\/edition.cnn.com\/2021\/08\/25\/health\/us-coronavirus-wednesday\/index.html\"><span style=\"font-weight: 400\">USA<\/span><\/a><span style=\"font-weight: 400\">, where schools reopened a few weeks ago, suggests that the lack of adequate mitigations will likely lead to infections spreading among children and significant absences due to student and staff illness, further disrupting learning.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Children are now returning to school against a background of community infection levels <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/aug\/28\/covid-cases-in-england-are-26-times-higher-than-a-year-ago\"><span style=\"font-weight: 400\">26 times higher<\/span><\/a><span style=\"font-weight: 400\"> than at the same time last year, and with the much more transmissible Delta variant accounting for almost all infections. Currently, one in 50 primary school children and one in 40 secondary school children have prevalent infection (<\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/bulletins\/coronaviruscovid19infectionsurveypilot\/27august2021\"><span style=\"font-weight: 400\">ONS<\/span><\/a><span style=\"font-weight: 400\">) and we are experiencing 40-50 hospitalisations in under 18s every day. The Scientific Advisory Group for Emergencies (SAGE)\u00a0 has warned that schools returning will likely lead to significant<\/span><a href=\"https:\/\/www.sec-ed.co.uk\/news\/back-to-school-trepidation-ahead-of-highly-likely-exponential-increases-in-covid-infections-sage-jcvi-vaccination\"><span style=\"font-weight: 400\"> increases<\/span><\/a><span style=\"font-weight: 400\"> in cases in school age groups. This will coincide with increased pressure on the NHS over winter due to other respiratory viruses, and potentially alongside waning SARS-CoV-2 immunity among the most vulnerable. This puts everyone at risk, \u200b\u200band exacerbates the additional burden on people from disadvantaged areas, as well as those predisposed to more severe disease from Black and Asian communities.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Both the <\/span><a href=\"https:\/\/www.ecdc.europa.eu\/sites\/default\/files\/documents\/COVID-19-in-children-and-the-role-of-school-settings-in-transmission-second-update.pdf\"><span style=\"font-weight: 400\">European Centre for Disease Prevention and Control<\/span><\/a><span style=\"font-weight: 400\"> (ECDC) and the <\/span><a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/community\/schools-childcare\/k-12-guidance.html\"><span style=\"font-weight: 400\">US Centers for Disease Control and Prevention<\/span><\/a><span style=\"font-weight: 400\">\u00a0(CDC) have recommended vaccination of teenagers, together with a combination of measures such as masking, small bubbles, testing, and improved ventilation in schools this autumn to keep children safer and in education. England has not followed this advice.<\/span> <span style=\"font-weight: 400\">There has been no plan for robust mitigation measures in schools to reduce the risk to children from infection and the consequences thereof, including long covid, hospitalisations, staff safety, and educational disruption. In England there is no requirement for masks, bubbles, physical distancing, or contact tracing within schools. <\/span><\/p>\n<p><span style=\"font-weight: 400\">Despite MHRA approval, the UK Joint Committee on Vaccination and Immunisation (JCVI) has <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/covid-19-vaccination-of-children-and-young-people-aged-12-to-17-years-jcvi-statement\/jvci-statement-on-covid-19-vaccination-of-children-and-young-people-aged-12-to-17-years-15-july-2021\"><span style=\"font-weight: 400\">not yet recommended<\/span><\/a><span style=\"font-weight: 400\"> vaccination of all 12-15 year olds, and their recommendation to vaccinate 16-17 year olds has come <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/jcvi-statement-august-2021-covid-19-vaccination-of-children-and-young-people-aged-12-to-17-years\/jcvi-statement-on-covid-19-vaccination-of-children-and-young-people-aged-12-to-17-years-4-august-2021\"><span style=\"font-weight: 400\">too late<\/span><\/a><span style=\"font-weight: 400\"> to complete vaccination of adolescents before they return to schools, despite <\/span><a href=\"https:\/\/osf.io\/grzma\/\"><span style=\"font-weight: 400\">clear evidence<\/span><\/a><span style=\"font-weight: 400\"> that <\/span><a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.121.056135\"><span style=\"font-weight: 400\">benefits far outweigh<\/span><\/a><span style=\"font-weight: 400\"> risk at current levels of infection. Meanwhile, <\/span><a href=\"https:\/\/covid.cdc.gov\/covid-data-tracker\/#vaccination-demographic\"><span style=\"font-weight: 400\">over 12 million<\/span><\/a><span style=\"font-weight: 400\"> under 18s have been vaccinated <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/aug\/04\/which-countries-are-vaccinating-minors-against-covid\"><span style=\"font-weight: 400\">across the world<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">England\u2019s policies mean that we will soon have a large susceptible population with a high prevalence of infection mixing in crowded environments with hardly any mitigations.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The government has a duty to protect children, our wider communities, and the NHS and healthcare workers from the impact of a fourth wave following schools re-opening.
