Yet, as the <\/span><a href=\"https:\/\/edition.cnn.com\/2021\/08\/03\/africa\/delta-variant-covid-africa-intl\/index.html\"><span style=\"font-weight: 400\">Delta variant<\/span><\/a><span style=\"font-weight: 400\"> of SARS-COV-2 spreads across the world, Africa, with a covid-19 vaccination rate of <\/span><a href=\"https:\/\/ourworldindata.org\/covid-vaccinations\"><span style=\"font-weight: 400\">less than 2%<\/span><\/a><span style=\"font-weight: 400\">,<\/span><span style=\"font-weight: 400\"> faces a high risk of surges in infection and deaths. Various initiatives such as <\/span><a href=\"https:\/\/www.africanews.com\/2021\/03\/04\/covax-features-heavily-in-africa-s-covid-19-rollout-this-week\/\"><span style=\"font-weight: 400\">COVAX<\/span><\/a><span style=\"font-weight: 400\"> and the <\/span><a href=\"https:\/\/africacdc.org\/news-item\/amsp-opens-covid-19-vaccines-pre-orders-for-55-african-union-member-states\/\"><span style=\"font-weight: 400\">African Vaccine Acquisition Task Team<\/span><\/a><span style=\"font-weight: 400\"> (AVATT) negotiate with pharmaceutical companies and high-income countries (HICs) to secure covid-19 vaccines for Africa, but the world can\u2019t avail itself of enough doses to rapidly respond to the demand. This presents a threat to Africa\u2019s development as the reorientation of resources to fight the pandemic is having a <\/span><a href=\"https:\/\/www.oecd.org\/coronavirus\/policy-responses\/the-territorial-impact-of-covid-19-managing-the-crisis-and-recovery-across-levels-of-government-a2c6abaf\/\"><span style=\"font-weight: 400\">negative impact<\/span><\/a><span style=\"font-weight: 400\"> on economies, as well as the provision of education and health services. This is why many African countries are fighting to start manufacturing the much needed covid-19 vaccines in order to avoid being the last to be served as always.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The response of many African countries early in the pandemic was <\/span><a href=\"https:\/\/www.ft.com\/content\/124dd4f4-8a0b-11ea-9dcb-fe6871f4145a\"><span style=\"font-weight: 400\">hailed<\/span><\/a><span style=\"font-weight: 400\"> for its focus on strict implementation of prevention measures backed by science under<\/span><a href=\"https:\/\/africacdc.org\/download\/report-of-an-emergency-meeting-of-africa-ministers-of-health-on-the-covid-19-outbreak\/\"><span style=\"font-weight: 400\"> the leadership of Africa Centres for Disease C<\/span><\/a><span style=\"font-weight: 400\">ontrol and Prevention<\/span><span style=\"font-weight: 400\">. This contributed to a <\/span><a href=\"https:\/\/ourworldindata.org\/covid-cases\"><span style=\"font-weight: 400\">low<\/span><\/a><span style=\"font-weight: 400\"> rate of covid-19 infections and mortality. However, the implementation of public health measures can only hold off a major catastrophe and ensure recovery for so long. <\/span><a href=\"https:\/\/www.who.int\/emergencies\/diseases\/novel-coronavirus-2019\/covid-19-vaccines\"><span style=\"font-weight: 400\">Vaccines are critical<\/span><\/a><span style=\"font-weight: 400\">,<\/span><span style=\"font-weight: 400\"> especially with the emergence of more infectious and deadlier <\/span><a href=\"https:\/\/www.nytimes.com\/2021\/07\/30\/health\/covid-cdc-delta-masks.html\"><span style=\"font-weight: 400\">variants<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Public health measures have largely relied on the ability of governments to implement them, with external support for testing and personal protective equipment (PPE) as Africa <\/span><a href=\"https:\/\/www.un.org\/technologybank\/content\/local_production_could_solve_shortages_pandemic_fighting_equipment\"><span style=\"font-weight: 400\">did not produce enough of these tools<\/span><\/a><span style=\"font-weight: 400\">. However, the continent has to look outwards to secure covid-19 vaccines. The continent currently accounts for <\/span><a href=\"https:\/\/www.mckinsey.com\/industries\/pharmaceuticals-and-medical-products\/our-insights\/africa-needs-vaccines-what-would-it-take-to-make-them-here\"><span style=\"font-weight: 400\">25% of the global demand<\/span><\/a><span style=\"font-weight: 400\"> for all types of vaccines, but only produces <\/span><a href=\"https:\/\/www.nature.com\/articles\/d41586-021-01048-1\"><span style=\"font-weight: 400\">1% of the vaccines <\/span><\/a><span style=\"font-weight: 400\">consumed<\/span><span style=\"font-weight: 400\">. This makes Africa\u2019s health security heavily reliant on the availability of surplus vaccines beyond the needs of HICs, and the willingness of pharmaceutical companies and their governments to extend an arm and supply Africa with these vaccines.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The story of how Africa got here is not news to many. Centuries of slavery, followed by colonialism, have enriched Europe while depriving Africa of its wealth and the capacity to build the necessary health and industrial systems to innovate medical interventions. Colonial powers created <\/span><a href=\"https:\/\/papers.ssrn.com\/sol3\/papers.cfm?abstract_id=3292189\"><span style=\"font-weight: 400\">extractive, resource-based economies<\/span><\/a><span style=\"font-weight: 400\"> rather than building health, education, and other systems needed for Africa\u2019s sustainable development.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the 1960s, <\/span><a href=\"https:\/\/journals.openedition.org\/poldev\/78\"><span style=\"font-weight: 400\">newly independent<\/span><\/a><span style=\"font-weight: 400\"> African countries that were economically deprived and lacked qualified human resources, looked to the colonizers of yesterday for support. Under the fa\u00e7ade of helping Africa, the new partnerships and multilateral organizations left little room for system building. Instead, Europe further entrenched colonial-time power dynamics and continued to divert African wealth from the continent, resulting in shockingly very little reduction in poverty, despite the abundance of natural resources on the continent. A few decades later, <\/span><a href=\"https:\/\/publichealthreviews.biomedcentral.com\/articles\/10.1186\/s40985-017-0059-2\"><span style=\"font-weight: 400\">structural adjustment programmes<\/span><\/a><span style=\"font-weight: 400\"> (SAPs) from the World Bank and the International Monetary Fund (IMF) required disinvestment in public programmes focused on building systems for human development, favoring tax-based private investments. History proved them wrong, as countries with the <\/span><a href=\"https:\/\/www.usnews.com\/news\/best-countries\/slideshows\/countries-with-the-most-well-developed-public-health-care-system\"><span style=\"font-weight: 400\">best public health systems<\/span><\/a><span style=\"font-weight: 400\"> are among those with the <\/span><a href=\"https:\/\/worldpopulationreview.com\/country-rankings\/best-healthcare-in-the-world\"><span style=\"font-weight: 400\">best population health outcomes<\/span><\/a><span style=\"font-weight: 400\">.<\/span><span style=\"font-weight: 400\"> Note that better health outcomes also contribute to higher levels of <\/span><a href=\"https:\/\/www.hsph.harvard.edu\/news\/magazine\/public-health-economy-election\/\"><span style=\"font-weight: 400\">economic growth<\/span><\/a><span style=\"font-weight: 400\">. Despite this proof, HICs and these international organizations expressed no regrets. No one was held accountable for HICs\u2019 misconduct and no reparations were provided to compensate for their wrong advice. On the contrary, HICs still essentially blame African countries for the current situation.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Colonialism and its legacy continue to handicap Africa, leaving it to provide rudimentary health services needed to keep populations alive instead of developing new interventions. <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5345397\/#:~:text=Migration%20of%20health%20workers%20'Brain,conditions%20in%20different%20places%20worldwide.\"><span style=\"font-weight: 400\">Brain drain<\/span><\/a><span style=\"font-weight: 400\">, which<\/span><span style=\"font-weight: 400\"> is estimated to result in a loss of <\/span><a href=\"https:\/\/mo.ibrahim.foundation\/news\/2018\/brain-drain-bane-africas-potential#:~:text=Annually%2C%20it%20is%20estimated%20that,in%20the%20health%20sector%20alone.&amp;text=It%20is%20estimated%20that%20Africa%20has%20lost%20%244.6%20billion%20in,these%20four%2Dtop%20destination%20countries\"><span style=\"font-weight: 400\">$2billion annually<\/span><\/a><span style=\"font-weight: 400\">, contributes substantially to the continent\u2019s poverty. Furthermore, despite the common narrative of large aid flows into Africa, the extraction of resources from Africa continues, with an estimated net value of <\/span><a href=\"https:\/\/www.theguardian.com\/global-development\/2017\/may\/24\/world-is-plundering-africa-wealth-billions-of-dollars-a-year\"><span style=\"font-weight: 400\">$41.3billion<\/span><\/a><span style=\"font-weight: 400\"> flowing out of Africa to rich countries in 2017. This further leads to <\/span><a href=\"https:\/\/theconversation.com\/african-countries-must-muscle-up-their-support-and-fill-massive-randd-gap-161024\"><span style=\"font-weight: 400\">inadequate<\/span><\/a><span style=\"font-weight: 400\"> investment in research and the <\/span><a href=\"https:\/\/www.avmi-africa.org\/manufacturers\/#1490181278853-496b3000-e3fc\"><span style=\"font-weight: 400\">limited number of vaccine manufacturers<\/span><\/a><span style=\"font-weight: 400\"> that currently prevent Africa from meeting the demand for vaccines on the continent. Today the continent has built a strong <\/span><a href=\"https:\/\/www.devex.com\/news\/au-launches-partnership-for-african-vaccine-manufacturing-99654\"><span style=\"font-weight: 400\">momentum for vaccine manufacturing<\/span><\/a><span style=\"font-weight: 400\"> in the context of covid-19, but for it to succeed, each step of the process requires leaders to fight the colonial legacies that undermine the real narrative and undercut Africa\u2019s progress.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The world needs to understand that if Africa reaches out to HICs for the resources needed to bolster vaccine manufacturing, it is because the disastrous impacts of centuries of enslavement, colonialism, and its continued legacy force Africa to look outwards. Supporting this request is not a question of compassion or beneficence, but rather one of overdue reparations to payback for centuries of injustice that diverted wealth from Africa. By doing so, Africa will be more autonomous in its efforts to protect the health of its population, while pharmaceutical firms in HICs will continue to make high profits through licensing.<\/span><\/p>\n<p><em><strong>Agnes Binagwaho<\/strong>,\u00a0vice chancellor of the University of Global Health Equity in Rwanda.
