But past evidence suggests this may be unwise, unnecessary, and potentially damaging, says Nigel Edwards<\/p>\n<p><!--more--><\/p>\n<p><span style=\"font-weight: 400\">One of the few positives of the health secretary Andrew Lansley\u2019s NHS reforms was his determination to remove the Secretary of State for Health and Social Care from the day-to-day running of the NHS. But the 2021-22 Health and Care Bill has created some anxiety about a possible <\/span><a href=\"https:\/\/www.thetimes.co.uk\/article\/matt-hancocks-nhs-power-grab-9zqrmt5gl\"><span style=\"font-weight: 400\">power grab<\/span><\/a><span style=\"font-weight: 400\"> by ministers and Number 10 to take more direct control over the running of the health service. Shortly before his departure, then Secretary of State for Health and Social Care, Matt Hancock said that the legislation would give him the power to <\/span><a href=\"https:\/\/www.hsj.co.uk\/policy-and-regulation\/government-to-take-joint-role-in-appointing-nhs-chairs-and-ceos\/7029728.article\"><span style=\"font-weight: 400\">sign off on senior appointments<\/span><\/a><span style=\"font-weight: 400\"> to local NHS boards. At the same time, in other parts of the public sector, we have seen interest in appointing <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-politics-57280438\"><span style=\"font-weight: 400\">politically connected individuals<\/span><\/a><span style=\"font-weight: 400\"> to board positions at institutions like the BBC and Ofcom.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Now Hancock\u2019s successor, Sajid Javid, begins his time at the Department of Health with a bill which gives new powers for politicians to control the detail of how the health service runs. An insider recently told me how unsurprising it should be that the government should seek to exert such influence. \u201cWhy shouldn\u2019t we seek to appoint Conservatives to positions,\u201d they argued, not least when \u201cthe organisation might go AWOL.\u201d In this case, they were not referring to the NHS, even if off-the-record briefings have suggested <\/span><a href=\"https:\/\/www.theguardian.com\/society\/2021\/apr\/29\/simon-stevens-to-step-down-as-nhs-england-boss-at-end-of-july\"><span style=\"font-weight: 400\">ministerial frustrations<\/span><\/a><span style=\"font-weight: 400\"> that Simon Stevens, outgoing CEO of NHS England, was not always compliant with government demands. That Stevens has appeared to be significantly more politically skilled than some senior politicians might also have proved irksome.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">It is worth noting that Labour had a similar view on favourable appointments in the 2000s, so this approach is nothing new. But against this, Javid\u2019s advisors should present him with the risks of such an approach, and explain why it may not deliver what politicians expect.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The most obvious point is that large, complex organisations require the best people. Whichever party might be in power, the size of their membership means that the talent pool for recruitment would be highly constrained. Appointing those from a wider group known to be sympathetic might get around this problem somewhat, but fails to avoid others that are more fundamental.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There is a strong argument for more diversity in the leadership of NHS organisations. This is associated with <\/span><a href=\"https:\/\/www.mckinsey.com\/featured-insights\/diversity-and-inclusion\/diversity-wins-how-inclusion-matters\"><span style=\"font-weight: 400\">improved organisational performance<\/span><\/a><span style=\"font-weight: 400\"> and innovation. NHS England have made the <\/span><a href=\"https:\/\/www.england.nhs.uk\/publication\/nhs-workforce-race-equality-a-case-for-diverse-board\/\"><span style=\"font-weight: 400\">case strongly<\/span><\/a><span style=\"font-weight: 400\">, including the important point that it is not reasonable for the boards of NHS organisations to look very different from the population they serve or the staff that work in them.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Mixing political and managerial control can create confusion about where and how decisions are taken. It creates a risk that different messages are transmitted down different routes and reduces transparency. It also cuts across some important principles of good governance. Those responsible for organisations have a key duty of stewardship of the organisation\u2014safeguarding its long-term future.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In countries where key health officials are politically appointed, I have observed a lack of willingness among hospital directors to make difficult decisions, and an incentive to postpone anything contentious. As in the UK, hospitals often serve many constituencies, and there is the risk of important decisions being distorted by short-term political considerations. NHS folklore already has it that there is only a two-year window to make significant changes to local services before the proposal might become entangled in politics.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The leaders of organisations also have an obligation to advocate for their organisations and generally take their stewardship role seriously. This can lead to a reverse problem from the point of view of the politicians, who can find that the people they appointed to roles because they were thought to be sympathetic to government policy have quickly gone native, and then use their political skills to oppose it.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The case of the famous chair of Mersey Regional Health Authority in the 1980s and 1990s shows the risks of the <\/span><a href=\"https:\/\/www.liverpool.ac.uk\/media\/livacuk\/iphs\/2publichealthandpolicy\/Mersey,RHA,WS,Transcript,Final,Version,September,2020.pdf\"><span style=\"font-weight: 400\">feudal fiefdoms<\/span><\/a><span style=\"font-weight: 400\"> that can result from appointing individuals with a strong personal political power base. For many years ministers and the Department of Health found that Mersey RHA interpreted policy in its own way and could successfully resist attempts to control it.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Where appointees do follow the party line, they risk losing credibility with staff and stakeholders by failing to advocate effectively for the organisation. It is unwise to create a culture of compliance in which there is no challenge, and this has been at the heart of many of the <\/span><a href=\"https:\/\/quarterly.blog.gov.uk\/2014\/07\/15\/the-blunders-of-our-governments-review-by-sir-david-normington-gcb\/\"><span style=\"font-weight: 400\">biggest errors<\/span><\/a><span style=\"font-weight: 400\"> made by governments over the years.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Introducing local politicians into elements of NHS governance is often helpful in ensuring that plans are locally sensitive and that links are made between different public sector plans. But that is very different from politicising key appointments within the system.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Political oversight of publicly funded institutions is legitimate and unavoidable.
