{"id":50598,"date":"2021-07-06T14:49:38","date_gmt":"2021-07-06T13:49:38","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=50598"},"modified":"2021-07-13T07:43:02","modified_gmt":"2021-07-13T06:43:02","slug":"we-must-take-long-covid-into-account-when-easing-covid-restrictions","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/07\/06\/we-must-take-long-covid-into-account-when-easing-covid-restrictions\/","title":{"rendered":"We must take long covid into account when easing covid restrictions"},"content":{"rendered":"<p><em>As the UK government announce plans to roll back covid-19 restrictions in England, a group of patient advocates with long covid write an open letter to Sajid Javid, the secretary of state for health and social care<\/em>.<\/p>\n<p><span style=\"font-weight: 400\">Dear Mr Javid,<\/span><\/p>\n<p><span style=\"font-weight: 400\">In January of this year we wrote to Boris Johnson, the UK prime minister, <a href=\"https:\/\/blogs.bmj.com\/bmj\/2021\/01\/12\/the-risk-of-long-covid-must-be-a-primary-consideration-in-policy-decisions\/\">urging him to take long covid into account when making policy decisions surrounding the relaxation of covid-19 restrictions<\/a>. We predicted that once vulnerable groups and older people had been vaccinated, there would be pressure to &#8220;open up,&#8221; given that unvaccinated younger people would be less likely to overwhelm the NHS.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The country has now arrived at this juncture: the coronavirus is circulating freely among young adults and children; cases are rising exponentially, but government policy is focused only on hospitalisations and deaths. However, in January, it was not anticipated that we would be facing a more transmissible and possibly more virulent variant which renders vaccines <\/span><a href=\"https:\/\/www.israelnationalnews.com\/News\/News.aspx\/309259\"><span style=\"font-weight: 400\">less effective<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The Office for National Statistics<\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/bulletins\/prevalenceofongoingsymptomsfollowingcoronaviruscovid19infectionintheuk\/1april2021\"> <span style=\"font-weight: 400\">estimates<\/span><\/a><span style=\"font-weight: 400\"> that <\/span>13.7%<span style=\"font-weight: 400\"> of people who contract covid-19 continue to experience symptoms for at least 12 weeks. Their latest<\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/bulletins\/prevalenceofongoingsymptomsfollowingcoronaviruscovid19infectionintheuk\/1july2021\"> <span style=\"font-weight: 400\">report<\/span><\/a><span style=\"font-weight: 400\"> suggests that <\/span>962,000 people<span style=\"font-weight: 400\">, or 1.5% of the UK population, have symptoms persisting for four or more weeks and that as many as <\/span>376,000<span style=\"font-weight: 400\"> have been impacted for at least a year. Two thirds report that their daily activities are adversely affected. It is still not clear how many will go on to suffer long term chronic illness over years to come. Long covid has been described as our &#8220;<\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp2109285\"><span style=\"font-weight: 400\">next national health disaster.&#8221;<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400\">The need to avoid overwhelming the NHS has been given as the primary aim of the government\u2019s pandemic response.\u00a0 Unfortunately, continuing to allow SARS-CoV-2 to infect huge numbers of people\u201427,334 new cases reported on 5 July 2021, with <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/jul\/06\/covid-cases-rise-above-100000-a-day-sajid-javid-concedes-england\"><span style=\"font-weight: 400\">forecasts<\/span><\/a><span style=\"font-weight: 400\"> that daily rates could reach more than 100,000\u2014will have serious implications for the health service despite the lower levels of hospitalisation during acute infection. Thousands of predominantly young, active people are being condemned to prolonged ill-health and disability every day.\u00a0 As well as putting considerable pressure on the NHS, their reduced capacity to work will further contribute to the impact long covid is already having on society and the economy through reduced output and tax revenues, and increased sick pay and benefits claims.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Long covid has not only been ignored in policy making decisions, and barely mentioned in the roadmap out of lockdown\u2014it has been almost completely absent from the government\u2019s public messaging. The population are now being asked to take responsibility for their own safety and &#8220;<\/span><a href=\"https:\/\/www.theguardian.com\/society\/2021\/jul\/04\/wearing-covid-face-masks-in-england-will-soon-be-voluntary-jenrick-confirms\"><span style=\"font-weight: 400\">exercise good judgement<\/span><\/a>&#8220;<span style=\"font-weight: 400\">, but due to this communications failure many do not realise the extent of the risk they are taking when exposing themselves to this virus. We note with concern that your fellow cabinet member Rishi Sunak has<\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-politics-57596213\"> <span style=\"font-weight: 400\">announced<\/span><\/a><span style=\"font-weight: 400\"> his intention to stop wearing a mask as soon as possible, despite the strong<\/span><a href=\"https:\/\/www.cnbc.com\/2021\/06\/25\/delta-who-urges-fully-vaccinated-people-to-continue-to-wear-masks-as-variant-spreads.html\"> <span style=\"font-weight: 400\">recommendation<\/span><\/a><span style=\"font-weight: 400\"> from the World Health Organization (WHO) that even those double vaccinated should continue to wear masks to protect themselves and, importantly, others from the Delta variant.<\/span><\/p>\n<p><span style=\"font-weight: 400\">There may not be a perfect time to lift all restrictions, but the time is certainly not now. Transmission is increasing at a startling rate, and<\/span><a href=\"https:\/\/coronavirus.data.gov.uk\/\"> <span style=\"font-weight: 400\">36%<\/span><\/a><span style=\"font-weight: 400\"> of adults are not yet protected with two vaccine doses. <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/998282\/Weekly_COVID-19_and_Influenza_Surveillance_Graphs_w26.pdf\"><span style=\"font-weight: 400\">Infection levels<\/span><\/a><span style=\"font-weight: 400\"> in schools are now approaching those at the height of the second wave. We ask you to urgently reconsider the timing of the removal of all measures to mitigate the spread of covid-19. Any steps towards encouraging mass indoor events and scrapping the wearing of face coverings should be considered with utmost care, taking due heed of the data on infections.<\/span><\/p>\n<p><span style=\"font-weight: 400\">It was the<\/span><a href=\"https:\/\/www.facebook.com\/watch\/?v=739366256910744\"> <span style=\"font-weight: 400\">position<\/span><\/a><span style=\"font-weight: 400\"> of your predecessor that \u201c<\/span><span style=\"font-weight: 400\">the first responsibility of any government is to protect its citizens\u201d: you must agree that this is an essential part of your job.
