{"id":50336,"date":"2021-05-28T18:51:56","date_gmt":"2021-05-28T17:51:56","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=50336"},"modified":"2021-06-04T07:52:37","modified_gmt":"2021-06-04T06:52:37","slug":"leave-no-one-behind-prioritising-inclusion-health-groups-for-covid-19-vaccination","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/05\/28\/leave-no-one-behind-prioritising-inclusion-health-groups-for-covid-19-vaccination\/","title":{"rendered":"Leave no one behind: prioritising inclusion health groups for covid-19 vaccination"},"content":{"rendered":"<p><span style=\"font-weight: 400\">The covid-19 pandemic has exacerbated long standing health inequalities experienced by socially excluded groups and without an inclusive approach to covid-19 vaccination these inequalities could be widened further by the vaccine rollout.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The Doctors of the World UK <\/span><a href=\"http:\/\/www.doctorsoftheworld.org.uk\/wp-content\/uploads\/2020\/05\/covid19-full-rna-report.pdf\"><span style=\"font-weight: 400\">Rapid Needs Assessment of Excluded People in England During the 2020 covid-19 Pandemic<\/span><\/a><span style=\"font-weight: 400\"> revealed that inclusion health populations, including people experiencing homelessness, migrants in vulnerable circumstances, Gypsy, Roma, and Traveller communities, and sex-workers are:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">at increased risk of being exposed to covid-19 due to crowded and shared living conditions, reduced access to hygiene and sanitation facilities, or high-risk working conditions;<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">more likely to have poorly managed chronic health problems that increase their risk of morbidity or mortality from covid-19;<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">less likely to access timely healthcare if they become unwell with covid-19;<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">less able to self-isolate and therefore at increased risk of experiencing outbreaks, a particular concern in view of increased transmissibility of the new covid-19 variant.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">In acknowledgement of this, on 24 February 2021, an NHS England (NHSE) <\/span><a href=\"https:\/\/www.england.nhs.uk\/coronavirus\/publication\/supporting-ccgs-to-address-vaccine-inequalities\/\"><span style=\"font-weight: 400\">communication<\/span><\/a><span style=\"font-weight: 400\"> stated that inclusion health populations \u201cshould be considered a priority for vaccination.\u201d Similarly, in a <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/letter-from-the-health-and-social-care-secretary-on-covid-19-vaccination-phase-1-advice\/letter-from-the-jcvi-to-the-health-and-social-care-secretary-on-further-considerations-on-phase-1-advice-1-march-2021\"><span style=\"font-weight: 400\">letter to the Secretary of State for Health<\/span><\/a><span style=\"font-weight: 400\">, dated 1 March 2021, the JCVI advised local teams \u201cconsider a universal offer to people experiencing homelessness and rough sleeping, alongside delivery of the programme to priority group 6, where appropriate.\u201d The NHS mobilisation guidance published 12 March 2021 to support this recommendation<\/span><span style=\"font-weight: 400\"> also explicitly mentions asylum seekers, refugee, and migrant populations who may need additional routes to the vaccine. <\/span><span style=\"font-weight: 400\">This approach recognises the need to take into account the complex inter-play of health and social risk factors for serious illness from covid-19 when prioritising patients for the vaccine.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Despite their increased risk of morbidity and mortality from covid-19, inclusion health populations are more likely to face barriers accessing the vaccine. In England, Scotland, and Wales, everyone is entitled to register with a GP and receive the covid-19 vaccine free of charge irrespective of immigration status. To facilitate this NHSE has launched the <\/span><a href=\"https:\/\/www.doctorsoftheworld.org.uk\/gp-access-cards\/\"><span style=\"font-weight: 400\">GP Access campaign <\/span><\/a><span style=\"font-weight: 400\">and updated its <\/span><a href=\"https:\/\/www.england.nhs.uk\/coronavirus\/wp-content\/uploads\/sites\/52\/2020\/03\/C1046-access-to-general-practice-comms-toolkit-feb-2021.pdf\"><span style=\"font-weight: 400\">toolkit<\/span><\/a><span style=\"font-weight: 400\"> to increase GP registration among people who frequently face barriers to registration, including people with no fixed abode and migrant populations. GP registration would ensure individuals have an NHS number and are invited for a vaccination in line with the<\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/955548\/Greenbook_chapter_14a_v6.pdf\"> <span style=\"font-weight: 400\">JCVI recommendations<\/span><\/a><span style=\"font-weight: 400\">. However, vaccine access issues go beyond GP registration as in order to access a covid-19 vaccination using the current mainstream delivery model an individual must also:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">be identified on the GP system as eligible to receive the vaccination \u2013 which relies on up-to-date medical records, documented clinical diagnoses and recorded risk factors required for Population Risk Assessment;\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">have a consistent address to receive the vaccine invitation;\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">be able to read and understand the vaccine invitation and have the language, literacy and digital skills to engage with the booking system;\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">be able to access the vaccination centre;\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">feel confident that accessing the vaccine is free and independent of immigration control activities;\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">believe they would benefit from having the vaccination.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">For the above reasons, tailored and <\/span><a href=\"https:\/\/www.doctorsoftheworld.org.uk\/wp-content\/uploads\/2021\/03\/DOTW-UK-Policy-Briefing_Outreach-and-bespoke-vaccination-delivery-services-for-socially-excluded-groups_15.03.2021.pdf?utm_medium=email&amp;utm_source=engagingnetworks&amp;utm_campaign=utm_email&amp;utm_content=Covid+resources+mailing+160321\"><span style=\"font-weight: 400\">outreach vaccination delivery services<\/span><\/a><span style=\"font-weight: 400\"> should run in parallel to an inclusive mainstream delivery approach.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Approval of the AstraZeneca vaccine has made an outreach delivery model possible and\u00a0<\/span><span style=\"font-weight: 400\">mobile devices with access to the vaccine recording system Pinnacle enable live documentation of vaccination. Government guidance is clear that <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/covid-19-vaccination-uptake-plan\/uk-covid-19-vaccine-uptake-plan\"><span style=\"font-weight: 400\">vaccination can be offered without an NHS number<\/span><\/a><span style=\"font-weight: 400\"> thus allowing outreach vaccination of people not registered with a GP. Good practice would include vaccination with simultaneous support to register with a GP for long term healthcare benefit and to facilitate delivery of the second vaccine dose. Several areas in the UK including <\/span><a href=\"https:\/\/www.bigissue.com\/latest\/homeless-couple-given-covid-19-vaccine-in-oldham-world-first\/\"><span style=\"font-weight: 400\">Oldham<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.nhsconfed.org\/blog\/2021\/02\/building-a-fairer-society\"><span style=\"font-weight: 400\">Shrewsbury<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/secure.manchester.gov.uk\/news\/article\/8688\/homeless_help_%E2%80%93_scheme_providing_covid-19_vaccinations_hailed_a_success\"><span style=\"font-weight: 400\">Manchester<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.redbridge.gov.uk\/news\/january-2021\/lfd-testing-and-covid-vaccination-for-rough-sleepers\/\"><span style=\"font-weight: 400\">Redbridge<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.bigissue.com\/latest\/proud-of-this-city-500-homeless-people-vaccinated-in-liverpool\/\"><span style=\"font-weight: 400\">Liverpool<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.qni.org.uk\/resources\/vaccination-for-people-experiencing-homelessness-in-brighton\/\"><span style=\"font-weight: 400\">Brighton<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/www.standard.co.uk\/news\/health\/homeless-vulnerable-covid-vaccine-watford-b920809.html\"><span style=\"font-weight: 400\">Watford<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/bevanhealthcare.org\/covid19-vaccination\/\"><span style=\"font-weight: 400\">Leeds and Bradford<\/span><\/a><span style=\"font-weight: 400\"> are already successfully vaccinating inclusion health populations.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The JCVI recently updated its <\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/use-of-the-astrazeneca-covid-19-vaccine-jcvi-statement-7-may-2021\/use-of-the-astrazeneca-covid-19-azd1222-vaccine-updated-jcvi-statement-7-may-2021\"><span style=\"font-weight: 400\">guidance<\/span><\/a><span style=\"font-weight: 400\"> to advise that unvaccinated adults aged under 40 years who are not in a clinical priority group at higher risk of severe covid-19 disease, should be preferentially offered an alternative to the AstraZeneca covid-19 vaccine. Every effort should be made to support adults encountered on outreach who are aged under 40 years to access an alternative vaccine. However, the JCVI advises that where this is not possible and the risk of acquiring or experiencing adverse health outcomes from covid-19 outweigh the rare risk of blood clots from the AstraZeneca vaccine then the AstraZeneca vaccine should be offered.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Improving vaccine accessibility is crucial to increasing vaccine uptake, but it is also necessary to inspire confidence in the covid-19 vaccine and in the system and personnel delivering it. Accurate information that accounts for language and literacy needs delivered via trusted channels is essential for developing confidence in vaccine safety and effectiveness. In response to this need Doctors of The World have <\/span><a href=\"https:\/\/www.doctorsoftheworld.org.uk\/coronavirus-vaccine-information\/\"><span style=\"font-weight: 400\">translated information on the covid-19 vaccine<\/span><\/a><span style=\"font-weight: 400\">. Areas that have vaccinated inclusion health populations emphasise the value of partnering with housing providers; voluntary, community and social enterprise organisations; community leaders; and representatives and peer advocates from inclusion health groups. These partners can assist with identifying potential clinic sites, inform culturally appropriate services, and encourage engagement by acting as bridges of trust between vaccine providers and patients.<\/span><\/p>\n<p><span style=\"font-weight: 400\">While the swift roll out of mass vaccination in the UK is something to be celebrated it is imperative that <\/span><span style=\"font-weight: 400\">partners responsible for covid-19 vaccine roll-out do not neglect their duty to ensure vaccine access is equitable and that no one is left behind.
