{"id":50286,"date":"2021-05-18T10:19:19","date_gmt":"2021-05-18T09:19:19","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=50286"},"modified":"2021-06-01T16:47:23","modified_gmt":"2021-06-01T15:47:23","slug":"martin-mckee-what-went-wrong-in-the-uks-covid-19-response","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/05\/18\/martin-mckee-what-went-wrong-in-the-uks-covid-19-response\/","title":{"rendered":"Martin McKee: What went wrong in the UK\u2019s covid-19 response?"},"content":{"rendered":"<p><em>One day we may know, but Boris Johnson is in no hurry, writes <strong>Martin McKee<\/strong><\/em><\/p>\n<p><span style=\"font-weight: 400\">In May 2020, Sarah Wollaston, Mike Gill, and I <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m2052\"><span style=\"font-weight: 400\">called for a rapid inquiry<\/span><\/a><span style=\"font-weight: 400\"> into the British government\u2019s response to the covid-19 pandemic. This would have been an opportunity to learn lessons and not to repeat them. At that time we knew that the United Kingdom\u2019s response to the news of an emerging infection had been slow, leading to <\/span><a href=\"https:\/\/www.theguardian.com\/uk-news\/2021\/mar\/18\/delaying-englands-winter-lockdown-caused-up-to-27000-extra-covid-deaths\"><span style=\"font-weight: 400\">tens of thousands<\/span><\/a><span style=\"font-weight: 400\"> of avoidable deaths. We were anxious that this should not be repeated. Our call was ignored and the events of debris in March 2020 were repeated in December 2020, as the prime minister, anxious to be seen to \u201csave Christmas,\u201d dithered as a new variant, B.1.1.7, spread rapidly. More recently, he delayed imposing a ban on travel from India until 23<\/span><span style=\"font-weight: 400\">\u00a0April 2021. The reasons <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/56801288\"><span style=\"font-weight: 400\">remain obscure<\/span><\/a><span style=\"font-weight: 400\">. Bangladesh and Pakistan had been placed on the red list on 9 April 2021 despite having much lower incidence rates. Television screens in the United Kingdom were full of horrendous pictures from India, with people dying in the streets. Of course, the prime minister insisted that this had nothing to do with his desire to sign a trade agreement with India and we must take his word for it. The consequences of enabling entry of the B.1.617.2 variant, now rising rapidly in several parts of the United Kingdom, are still unclear, but it does seem like the same mistake is being made yet again.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The prime minister has, at last, agreed that there <\/span><a href=\"https:\/\/www.politico.eu\/article\/boris-johnson-launches-public-inquiry-into-pandemic-handling\/\"><span style=\"font-weight: 400\">will be an inquiry<\/span><\/a><span style=\"font-weight: 400\">. It will begin in spring 2022. The reason he gave for the delay is that <\/span><span style=\"font-weight: 400\">\u201cWe must not inadvertently divert or distract the people on whom we depend in the heat of our struggle against this disease.\u201d<\/span><span style=\"font-weight: 400\"> We are to believe that the government\u2019s abolition of Public Health England and the creation of successor bodies, which will require many of those essential public health staff to reapply for their jobs, will somehow not be a distraction. Similarly, the government\u2019s planned reorganisation of the NHS will be simple and straightforward. The prime minister has given few details except that it would be \u201cindependent,\u201d something that he has not elaborated on.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Public inquiries are designed to <\/span><a href=\"https:\/\/www.instituteforgovernment.org.uk\/publications\/how-public-inquiries-can-lead-change\"><span style=\"font-weight: 400\">answer three questions<\/span><\/a><span style=\"font-weight: 400\">. First, what happened? Second, why did it happen and who is to blame? Third, what can be done to prevent it happening again? The prime minister has implied that the inquiry will be a statutory one, held under the Inquiries Act 2005 as he has said that it will have <\/span><span style=\"font-weight: 400\">\u201cthe ability to compel the production of all relevant materials and take oral evidence in public, under oath.\u201d This means that it is likely to take quite a long time. Once the inquiry chair and panel have been appointed they will spend some time agreeing terms of reference. There will be many legal complexities to resolve. The Grenfell Tower inquiry <\/span><a href=\"https:\/\/www.theguardian.com\/uk-news\/2020\/feb\/23\/grenfell-public-inquiry-delayed-again-over-evidence-concerns\"><span style=\"font-weight: 400\">was delayed<\/span><\/a><span style=\"font-weight: 400\"> as the Attorney General considered the balance between witnesses speaking freely and the potential for self-incrimination. The sheer volume of material that will have to be examined will create further delays. The Iraq War inquiry, perhaps the most comparable, stretched to <\/span><a href=\"https:\/\/webarchive.nationalarchives.gov.uk\/20171123122743\/http:\/www.iraqinquiry.org.uk\/the-report\/\"><span style=\"font-weight: 400\">12 volumes<\/span><\/a><span style=\"font-weight: 400\"> containing 2.6 million words. That inquiry took seven years to report, during which one of the panel died. Indeed, it is not uncommon for public inquiries to lose key members. The Independent Inquiry into Child Sexual Abuse, which <\/span><a href=\"https:\/\/www.iicsa.org.uk\/about-us\/timeline\"><span style=\"font-weight: 400\">commenced in 2015<\/span><\/a><span style=\"font-weight: 400\">, has gone through four chairs and is still in progress, although has issued several thematic reports. Often, these delays mean that any report will appear long after those involved have left office. In this case it would perhaps be optimistic to expect a report before 2028. It is to be hoped that there is no further pandemic in the meantime.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Although the newly announced inquiry is unlikely to provide anything much of value for some years, it is important that it takes place to uphold the principle of accountability. However, there are some important considerations that must be addressed. First, what will its scope be? It is already apparent that there were failures at many levels, set out in detail in a number of accounts, with the book <\/span><a href=\"https:\/\/harpercollins.co.uk\/products\/failures-of-state-the-inside-story-of-britains-battle-with-coronavirus-jonathan-calvertgeorge-arbuthnott?variant=32622317338702\"><span style=\"font-weight: 400\">Failures of State<\/span><\/a><span style=\"font-weight: 400\"> a particularly good example. We know that the prime minister was not fully engaged at the beginning of the pandemic, with his attention focused on other political and personal matters. But how well was the machinery of government functioning? The chaotic events around the Brexit negotiations had revealed a <\/span><a href=\"https:\/\/academic.oup.com\/jpubhealth\/article-abstract\/42\/4\/778\/5700760?redirectedFrom=fulltext\"><span style=\"font-weight: 400\">deeply dysfunctional state<\/span><\/a><span style=\"font-weight: 400\">, with other accounts, such as the <\/span><a href=\"https:\/\/harpercollins.co.uk\/products\/in-the-thick-of-it-the-private-diaries-of-a-minister-alan-duncan?variant=39254286663758\"><span style=\"font-weight: 400\">diaries of Alan Duncan<\/span><\/a><span style=\"font-weight: 400\">, Minister of State for Europe and the Americas until 2019, described a ruling party characterised by internal conflict. So will it cover decision making structures and processes in central government?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Unlike inquiries in some other areas, such as the Iraq War, where defence is a matter reserved to Westminster, the devolved administrations played a major role in the response, often out of step with England. Presumably their actions will also be scrutinised so will they have a say in the appointment of the chair and panel?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Another area that was obviously problematic was procurement, especially of testing and tracing capacity and personal protective equipment. This is an area where there could be criminal prosecutions, given growing evidence of, to say the least, questionable use of public funds. How will the legal issues that arise be dealt with?<\/span><\/p>\n<p><span style=\"font-weight: 400\">One reason why the United Kingdom suffered so greatly seems to be the weaknesses of social safety nets, with many people leading precarious lives, unwilling to risk losing income if they came forward to be tested and had to isolate, or living in crowded housing that created the conditions for spread. This is in part the consequence of austerity measures adopted since 2010. Will these fall within the scope of the inquiry?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Fortunately, those who want to fix some of the things that went wrong, as opposed to attributing responsibility to individuals and institutional structures, do not have to wait. Already quite a few of the problems have been identified in evidence given to parliamentary committees, such as that on <\/span><a href=\"https:\/\/committees.parliament.uk\/work\/81\/management-of-the-coronavirus-outbreak\/publications\/\"><span style=\"font-weight: 400\">Management of the Coronavirus Outbreak<\/span><\/a><span style=\"font-weight: 400\">, being undertaken by the Health and Social Care Committee. Other valuable insights come from the report and hearings of the <\/span><a href=\"https:\/\/appgcoronavirus.marchforchange.uk\/\"><span style=\"font-weight: 400\">All Party Parliamentary Group on Coronavirus<\/span><\/a><span style=\"font-weight: 400\">. There is also the <\/span><a href=\"https:\/\/www.peoplescovidinquiry.com\/\"><span style=\"font-weight: 400\">Peoples\u2019 COVID inquiry<\/span><\/a><span style=\"font-weight: 400\">, organised by Keep our NHS Public. At an international level we have the report of the <\/span><a href=\"https:\/\/theindependentpanel.org\/\"><span style=\"font-weight: 400\">Independent Panel on Pandemic Preparedness and Response<\/span><\/a><span style=\"font-weight: 400\">. However, the Westminster government has shown no sign that it will pay attention to any of them.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Unsurprisingly, the prime minister\u2019s seeming lack of curiosity in what went wrong or of any sense of urgency has been heavily criticised by, among others, the <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/may\/13\/hell-on-earth-bereaved-families-on-the-battle-for-a-covid-inquiry\"><span style=\"font-weight: 400\">Covid-19 Bereaved Families for Justice group<\/span><\/a><span style=\"font-weight: 400\">, who had threatened the government with judicial review if it failed to hold an inquiry, and the APPG, whose members have <\/span><a href=\"https:\/\/www.theguardian.com\/uk-news\/2021\/may\/16\/call-for-covid-inquiry-panel-to-be-fully-independent\"><span style=\"font-weight: 400\">called for the inquiry members<\/span><\/a><span style=\"font-weight: 400\"> to be selected by a cross-party committee with involvement of the devolved administrations.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Although we are just over a year into the current Westminster government, it is clear that the prime minister is focusing on the next election. He is <\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2021\/mar\/05\/the-guardian-view-on-ending-fixed-terms-johnson-grabs-the-crown\"><span style=\"font-weight: 400\">repealing the Fixed Term Parliaments Act<\/span><\/a><span style=\"font-weight: 400\"> to allow him to call an election at a time that is most advantageous to him. He is also introducing a requirement for voters to present photo ID, based on arguments that the former leader of the Scottish Conservative Party has described as <\/span><a href=\"https:\/\/www.thetimes.co.uk\/article\/former-scottish-conservatives-leader-ruth-davidson-lashes-out-at-voter-id-plans-mzbwlndsv\"><span style=\"font-weight: 400\">\u201cbollocks\u201d<\/span><\/a>,<span style=\"font-weight: 400\"> but widely seen as copying US policies on voter suppression. There is no way that his inquiry will report before the next election.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">This is a prime minister who seems very uncomfortable with the concept of accountability. He is notorious for <\/span><a href=\"https:\/\/www.independent.co.uk\/news\/uk\/politics\/boris-johnson-pmqs-speaker-lindsay-hoyle-b1761660.html\"><span style=\"font-weight: 400\">not answering parliamentary questions<\/span><\/a><span style=\"font-weight: 400\"> or, if he does, to mislead and fail to <\/span><a href=\"https:\/\/www.politicshome.com\/news\/article\/boris-johnson-labour-nhs-pay-incorrect\"><span style=\"font-weight: 400\">correct the record<\/span><\/a><span style=\"font-weight: 400\">. There are continuing unanswered questions about his adherence to <\/span><a href=\"https:\/\/www.itv.com\/news\/2021-05-14\/johnsons-mustique-holiday-was-worth-double-the-amount-declared\"><span style=\"font-weight: 400\">parliamentary standards<\/span><\/a><span style=\"font-weight: 400\">. We can debate how important these issue are and the prime minister has always argued that they are not what the public is interested in. But his government\u2019s ability to suppress the pandemic, especially in the face of a new and rapidly increasing variant, is of interest.
