{"id":50253,"date":"2021-05-13T12:52:11","date_gmt":"2021-05-13T11:52:11","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=50253"},"modified":"2021-05-13T12:52:24","modified_gmt":"2021-05-13T11:52:24","slug":"placing-womens-voices-at-the-centre-of-their-care","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/05\/13\/placing-womens-voices-at-the-centre-of-their-care\/","title":{"rendered":"Placing women\u2019s voices at the centre of their care"},"content":{"rendered":"<p><span style=\"font-weight: 400\">It is very encouraging to see the UK government <\/span><a href=\"https:\/\/www.gov.uk\/government\/news\/government-launches-call-for-evidence-to-improve-health-and-wellbeing-of-women-in-england\"><span style=\"font-weight: 400\">announcing its intention to develop a Women&#8217;s Health Strategy<\/span><\/a><span style=\"font-weight: 400\">. Anyone who doubts the need for this just has to look back over the last year in the life of the NHS.<\/span><\/p>\n<p><span style=\"font-weight: 400\">2020 is thought of as the year of covid-19. But it was also a year that saw a succession of large scale avoidable harm scandals\u2014all affecting women.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In February 2020, <a href=\"https:\/\/www.gov.uk\/government\/publications\/paterson-inquiry-report\">the Paterson Inquiry<\/a> reported on the criminal activities of Ian Paterson, who performed unnecessary breast surgery on thousands of women. In the same month, Bill Kirkup, who led the investigation into deaths in maternity services at <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/408480\/47487_MBI_Accessible_v0.1.pdf\"><span style=\"font-weight: 400\">Morecambe Bay<\/span><\/a><span style=\"font-weight: 400\">, was appointed to carry out an <\/span><a href=\"https:\/\/www.nursingtimes.net\/news\/hospital\/kirkup-to-lead-independent-review-of-east-kent-maternity-services-14-02-2020\/\"><span style=\"font-weight: 400\">independent review<\/span><\/a><span style=\"font-weight: 400\"> into avoidable harm in East Kent Hospitals&#8217; maternity services.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In July 2020, the <\/span><a href=\"https:\/\/www.bmj.com\/content\/370\/bmj.m3099\"><span style=\"font-weight: 400\">Cumberlege review<\/span><\/a><span style=\"font-weight: 400\"> of medicines and medical devices described the &#8220;anguish, suffering and ruined lives&#8221; of thousands of women affected by treatments including pelvic mesh.\u00a0 And in December 2020, <a href=\"https:\/\/www.donnaockenden.com\/downloads\/news\/2020\/12\/ockenden-report.pdf\">Donna Ockenden&#8217;s <\/a><\/span><span style=\"font-weight: 400\">Emerging Findings<\/span><span style=\"font-weight: 400\"> report began to lift the lid on 1,862 reported cases of avoidable harm and death in maternity services at Shrewsbury and Telford.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">These harms were avoidable because the providers knew about them. Patients and bereaved relatives had been speaking up\u2014sometimes for years. But no one had wanted to hear them.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Men have been affected too\u2014for example as fathers and birth partners in the case of the maternity deaths. And in the aftermath of the harms inflicted by Paterson, and those described by Cumberlege, many husbands and partners have had to become carers.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The fact remains, though, that the brunt of the harm has been borne by women.\u00a0\u00a0<\/span><\/p>\n<p><b>Women&#8217;s voices at the centre<\/b><\/p>\n<p><span style=\"font-weight: 400\">A consultation is now open via a <\/span><a href=\"https:\/\/www.gov.uk\/government\/consultations\/womens-health-strategy-call-for-evidence\/womens-health-strategy-call-for-evidence\"><span style=\"font-weight: 400\">call for evidence<\/span><\/a><span style=\"font-weight: 400\"> which lists six core themes, the first of which is &#8220;Placing women\u2019s voices at the centre of their health and care.&#8221; This is undoubtedly the most important, given the disasters listed above. But how do we place women&#8217;s voices at the centre of their care?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Baroness Cumberlege described the women who contributed to her <\/span><a href=\"https:\/\/pexlib.net\/?223550\"><span style=\"font-weight: 400\">First Do No Harm<\/span><\/a><span style=\"font-weight: 400\"> report as &#8220;\u2026well informed, knowledgeable, and research based. They never failed to ensure we learnt from them and were up to date with emerging developments. They are outstanding communicators and expert in the subject matter.&#8221;<\/span><\/p>\n<p><span style=\"font-weight: 400\">Evidently, the problem is not with women not speaking up, but with the healthcare system not being willing or able to listen.<\/span><\/p>\n<p><b>Opening the ears of our healthcare system<\/b><\/p>\n<p><span style=\"font-weight: 400\">The NHS\u2014from the Long Term Plan downwards\u2014describes itself as &#8220;person-centred&#8221; and &#8220;evidence-based.&#8221;\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">A healthcare system that wants to be both person-centred and evidence-based has to have a really good evidence base for patient experience. Alongside that, it needs a professional learning infrastructure to help translate the evidence into practice.\u00a0 In these areas, the NHS is sadly lacking.<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><span style=\"font-weight: 400\">The NHS &#8220;National Core Content&#8221; contains the databases and journals deemed essential to underpin evidence-based practice, and made available to all staff.\u00a0 Patient experience evidence is noticeable by its absence.<\/span><\/span>&nbsp;<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><span style=\"font-weight: 400\">Patient experience staff (PALS teams, complaints managers etc) are almost unique within the NHS workforce in having no professional qualification, and no systematic approach to continuing professional development.<\/span><\/span>&nbsp;<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><span style=\"font-weight: 400\">While medical research is steered through formal research prioritisation processes, patient experience research is largely uncoordinated.\u00a0 No one has a strategic overview of the patient experience evidence base, so time and money are wasted on duplicate studies while big gaps in the evidence base are ignored.<\/span><\/span>&nbsp;<\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Clinicians have analytical and diagnostic tools to help ensure that practice is evidence-based.\u00a0 Analytical tools for patient experience work are, by contrast, few and far between.\u00a0\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">Baroness Cumberlege, in her First Do No Harm report, referred to the tendency of medical professions to dismiss patient feedback as &#8220;anecdotal evidence.&#8221; She said that patient experience &#8220;must no longer be considered anecdotal and weighted least in the hierarchy of evidence-based medicine.&#8221;\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We agree, and are working to address the fundamental system weaknesses outlined above. Action on these fronts will be of benefit to all patients. But since women, from recent evidence, bear the brunt of avoidable harm in medicine, it could benefit them most of all.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">So a women&#8217;s health strategy must address these issues. A strategy that aims to place women\u2019s voices at the centre of their health and care must not simply require women to carry on shouting into a void.
