Hospital admissions are back down to levels last seen in September 2020, and over half of the adult population has received at least one dose of covid-19 vaccine. We\u2019ve experienced almost six months of national lockdown in the last year and are just emerging from what has been promised to be the last nationwide lockdown.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">But while we have all experienced this pandemic together, we have not all had the same experience. Deprived and minority ethnic communities have borne the brunt of the pandemic so far and there is now a very real danger that covid-19 will become entrenched as a disease of poverty.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Those living in more deprived communities have been more likely to contract covid-19, with weekly average case rates since September 50% higher than the least deprived communities (283\/week\/100,000 people compared to 184\/week\/100,000 people). Compared to the least deprived communities, people in the most deprived areas have had <\/span><a href=\"https:\/\/www.icnarc.org\/Our-Audit\/Audits\/Cmp\/Reports\"><span style=\"font-weight: 400\">more than double the number of ICU admissions<\/span><\/a><span style=\"font-weight: 400\"> and almost <\/span><a href=\"https:\/\/www.nature.com\/articles\/s41586-020-2521-4.pdf\"><span style=\"font-weight: 400\">double the risk of dying<\/span><\/a><span style=\"font-weight: 400\">, most likely because <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7221360\/\"><span style=\"font-weight: 400\">poverty often goes hand in hand with poorer health and c<\/span><\/a><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7221360\/\"><span style=\"font-weight: 400\">ovid-19 risk factors such as obesity, hypertension, and diabetes<\/span><\/a><span style=\"font-weight: 400\">. As the UK vaccination programme proceeds apace, we see differential uptake by deprivation there too<\/span><a href=\"http:\/\/opensafely.org\/research\/2021\/covid-vaccine-coverage\/\"><span style=\"font-weight: 400\">: Open Safely data to the end of March<\/span><\/a><span style=\"font-weight: 400\"> shows for instance that among shielding adults 16-69 years old, 79% had had one vaccine dose in most deprived communities compared to 92% in least deprived communities. The difference in vaccine uptake is <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/mar\/29\/study-low-covid-jab-take-up-black-people-england\"><span style=\"font-weight: 400\">most pronounced in minority ethnic communities<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">People in more deprived communities are more likely to catch covid-19 for a number of reasons. Firstly, they are much more likely to have to work outside the home and in roles where there is a lot of contact with others. For instance, health and care workers, security guards, public transport workers, cab drivers, retail assistants, and factory workers <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/causesofdeath\/bulletins\/coronaviruscovid19relateddeathsbyoccupationenglandandwales\/deathsregisteredbetween9marchand28december2020\"><span style=\"font-weight: 400\">are all associated with higher <\/span><\/a><span style=\"font-weight: 400\">exposure to catching covid-19.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Secondly, they are more likely to live in multi-occupancy housing: <\/span><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2021.03.03.21252856v1.full.pdf\"><span style=\"font-weight: 400\">those living in households with more than six<\/span><\/a><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2021.03.03.21252856v1.full.pdf\"><span style=\"font-weight: 400\">\u00a0people have twice the risk of infection<\/span><\/a><span style=\"font-weight: 400\"> compared to households with one or two people. Particularly relevant as we emerge from lockdown, those in deprived communities have <\/span><a href=\"https:\/\/t.co\/CsSrJhNwEA?amp=1\"><span style=\"font-weight: 400\">worse access to high quality green spaces<\/span><\/a><span style=\"font-weight: 400\"> which is likely to lead to more crowded meetings outdoors or more meetings in higher risk indoor spaces.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Thirdly, one key way to keep cases down in a community is through a robust test, trace, isolate, and support system as it keeps infectious people from mixing with susceptible people. However, such a system relies on people first getting a test and then being able and willing to isolate effectively at home for ten days. A <\/span><a href=\"https:\/\/www.bmj.com\/content\/372\/bmj.n608\"><span style=\"font-weight: 400\">recent BMJ paper<\/span><\/a><span style=\"font-weight: 400\"> reported that only about 20% of people with symptoms request a test and only about half of those with symptoms isolate. It identified that being financially unable to afford to isolate was a key factor in determining both testing and isolation behaviour. Dido Harding, head of test and trace, <\/span><a href=\"https:\/\/www.politicshome.com\/news\/article\/dido-harding-admits-20000-people-a-day-are-ignoring-calls-by-nhs-test-and-trace-to-selfisolate\"><span style=\"font-weight: 400\">acknowledged that lack of support for isolation was lowering the effectiveness of contact tracing<\/span><\/a><span style=\"font-weight: 400\">. Poor housing further exacerbates the problem as even those who do isolate risk infecting others in their household due to cramped accommodation. Cevik et al. <\/span><a href=\"https:\/\/www.bmj.com\/content\/372\/bmj.n224\"><span style=\"font-weight: 400\">argued persuasively<\/span><\/a><span style=\"font-weight: 400\"> that enhancing comprehensive support for isolation is crucial as we exit the most recent wave of infection.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The above three factors have two implications: firstly, that lockdowns are much less effective in deprived communities because <\/span><a href=\"https:\/\/twitter.com\/strelitz_jason\/status\/1369679289579409408?s=20\"><span style=\"font-weight: 400\">more people work outside the home<\/span><\/a><span style=\"font-weight: 400\">, <\/span><span style=\"font-weight: 400\">cannot afford to isolate, and so cannot lock down. Also poorer and more crowded housing means covid-19 continues to propagate more easily. This means that there is significantly more covid circulating in more deprived communities as we start coming out of lockdown\u2014almost <\/span><a href=\"https:\/\/spiral.imperial.ac.uk\/bitstream\/10044\/1\/87351\/2\/react1_r10_preprint.pdf\"><span style=\"font-weight: 400\">2.5 times as many cases per head in the last two weeks of March<\/span><\/a><span style=\"font-weight: 400\"> as in the least deprived areas. This makes outbreaks much more likely as people start to mix more\u2014particularly with worse access to safer outdoor spaces. And differential vaccine uptake also means that these communities are less protected once outbreaks occur. Even as the rest of the country enjoys a more normal summer, <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/975909\/S1182_SPI-M-O_Summary_of_modelling_of_easing_roadmap_step_2_restrictions.pdf\"><span style=\"font-weight: 400\">more deprived areas will likely be subject to waves<\/span><\/a><span style=\"font-weight: 400\"> of infections, the burden of ill health (and potentially death) that follows, and local lockdowns restricting local economies\u2014just where growth is needed is most.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Secondly, these differences will likely <\/span><a href=\"https:\/\/www.independent.co.uk\/news\/health\/covid-cases-north-south-lockdown-b1824083.html\"><span style=\"font-weight: 400\">recreate the north\/south divide<\/span><\/a><span style=\"font-weight: 400\"> in covid-19 cases and restrictions that we saw last summer and autumn in England as northern regions have substantially more deprived local authorities than the south. Only one out of the 25 local authorities with the highest case rates is in the South of England.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">This scenario is not inevitable. Improved support for self isolation, as <\/span><a href=\"https:\/\/twitter.com\/ReicherStephen\/status\/1379482678685282305?s=20\"><span style=\"font-weight: 400\">instituted now in Newham<\/span><\/a><span style=\"font-weight: 400\">, is fundamental: increased population testing will do nothing to prevent infections in people who test positive but cannot isolate. Secondly, work spaces and schools can be supported to improve ventilation to reduce transmission indoors\u2014again, a policy that would disproportionately help those who cannot work from home. Thirdly, gaps in vaccine uptake must be addressed by working with local communities to address vaccine concerns\u2014particularly in <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/health-56665517\"><span style=\"font-weight: 400\">the wake of new concerns about AstraZeneca<\/span><\/a><span style=\"font-weight: 400\">\/Oxford vaccine<\/span><span style=\"font-weight: 400\"> \u2013 and to make getting a jab as easy as possible. The government has touted a \u201clevelling up\u201d agenda for addressing regional inequality in England.
