{"id":49872,"date":"2021-03-19T18:39:49","date_gmt":"2021-03-19T17:39:49","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49872"},"modified":"2021-03-20T09:04:54","modified_gmt":"2021-03-20T08:04:54","slug":"psychological-ppe-do-we-need-to-start-fit-testing-earlier-in-medical-training","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/03\/19\/psychological-ppe-do-we-need-to-start-fit-testing-earlier-in-medical-training\/","title":{"rendered":"Psychological PPE: do we need to start fit testing earlier in medical training?"},"content":{"rendered":"<p><span style=\"font-weight: 400\">As we exit another covid-19 peak and start to ease out of a third UK lockdown, concerns are being raised about <\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2020\/oct\/16\/psychological-ppe-britain-health-professionals-covid-medical-mental-face-masks\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">the resilience of the medical workforce<\/span><\/a><span style=\"font-weight: 400\"> and ability of individuals to cope with the anticipated practical and psychological strain of the months ahead.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Thinking back to the hurried preparations during the first peak in March, valid concerns were raised about the ability of the NHS to cope with the expected influx of patients and the public stayed at home to &#8220;<\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/coronavirus-covid-19-information-leaflet\/coronavirus-stay-at-home-protect-the-nhs-save-lives-web-version\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">protect the NHS<\/span><\/a><span style=\"font-weight: 400\">.<\/span><span style=\"font-weight: 400\">&#8221; As doctors faced stress and moral injury, intensified by the pandemic, some sort of psychological reaction was inevitable, but concern about how doctors as individuals would cope psychologically was not widely discussed.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the public eye, healthcare professionals, and doctors and nurses in particular, were seen as &#8220;heroes&#8221; and rewarded with weekly rounds of applause<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">little public consideration was given to the question of whether the \u201chero\u201d<\/span><a href=\"https:\/\/www.theguardian.com\/society\/2020\/may\/22\/clapping-distracts-from-the-real-issues\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\"> label sat comfortably with them<\/span><\/a><span style=\"font-weight: 400\">. It might be difficult for a doctor acting in a &#8220;hero&#8221; role to admit both to themselves and others that they feel stressed or unwell. This may lead to a delay in them seeking appropriate support or help.<\/span><\/p>\n<p><span style=\"font-weight: 400\">While the government was scrabbling to secure sufficient personal protective equipment (PPE) for the workforce, Nightingale Hospitals were ahead of the curve in equipping all their staff with a proactive response to emotional wellbeing<\/span><span style=\"font-weight: 400\">\u2014&#8221;<\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/08\/14\/psychological-ppe-the-space-between-signposting-and-action\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">psychological PPE.&#8221;<\/span><\/a><span style=\"font-weight: 400\"> But medical training has given the current workforce little preparation about how to don &#8220;psychological PPE.&#8221;<\/span><\/p>\n<p><span style=\"font-weight: 400\">As psychiatry trainees we are privileged to attend reflective groups, but even as self-selected trainees with an interest in psychology, reflecting on our own emotions and psychological responses at work can feel alien and difficult, particularly in the early stages of training. Trainees in other programmes have no such dedicated time and space for reflection, and doctors ordinarily only access wellbeing initiatives or psychological support if they are having difficulties in their training and\/or work.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The experience of working as a doctor, even outside a pandemic, provides encounters with death, trauma and distress as daily occurrences. Of course doctors support each other with informal conversations, cups of tea and typical NHS <\/span><a href=\"https:\/\/pmj.bmj.com\/content\/92\/1093\/691\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">gallows humour<\/span><\/a><span style=\"font-weight: 400\">. The experience of having feelings validated and normalised can provide emotional relief. But some doctors\u2019 social circumstances mean they will not be able to access these informal medical support networks, and others will find they have <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1510\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">become more isolated<\/span><\/a><span style=\"font-weight: 400\"> since the pandemic.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Even before the current situation, evidence has shown that doctors who receive appropriate emotional support are able to provide better care for their patients. Surgeons with <\/span><a href=\"https:\/\/protect-eu.mimecast.com\/s\/J4v_CMZ4lh2kMMMSwya9l?domain=pubmed.ncbi.nlm.nih.gov\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">worse self-reported mental health<\/span><\/a><span style=\"font-weight: 400\"> reported more major medical errors than their colleagues and burnout has been strongly associated with self-reported <\/span><a href=\"https:\/\/protect-eu.mimecast.com\/s\/XVAMCNO4mHZPDDDU4PKUc?domain=pubmed.ncbi.nlm.nih.gov\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">suboptimal patient care<\/span><\/a><span style=\"font-weight: 400\"> practices <\/span><a href=\"https:\/\/protect-eu.mimecast.com\/s\/QV08COg4nINZzzzIkD9w8?domain=pubmed.ncbi.nlm.nih.gov\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">across specialties<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/protect-eu.mimecast.com\/s\/WLX2CPj4os0N888ujbSg-?domain=pubmed.ncbi.nlm.nih.gov\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">grades<\/span><\/a><span style=\"font-weight: 400\">. Emotional regulation can be taught and results in a <\/span><a href=\"https:\/\/protect-eu.mimecast.com\/s\/jFgOCQk4puXlMMMHrkdLW?domain=pubmed.ncbi.nlm.nih.gov\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">reduction in burnout<\/span><\/a><span style=\"font-weight: 400\">, which is crucial because doctors\u2019 occupational wellbeing is significantly associated with patients\u2019 <\/span><a href=\"https:\/\/protect-eu.mimecast.com\/s\/1aOpCRl9qIG0jjjsofbFp?domain=pubmed.ncbi.nlm.nih.gov\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">adherence to treatment<\/span><\/a><span style=\"font-weight: 400\"> and even with lower rates of <\/span><a href=\"https:\/\/protect-eu.mimecast.com\/s\/aH2lCVv6xc2kNNNSWUE1d?domain=som.org.uk\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400\">hospital-borne infections<\/span><\/a><span style=\"font-weight: 400\">. We must provide doctors with strategies to prevent burnout and allow them to provide the best patient care.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In a terrible way, the pandemic may finally be giving mental health &#8220;parity of esteem&#8221; with physical health, something which has been sought for so long. The conversation has changed<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">politicians are attempting to weigh up the risk of covid-19 transmission against the risks of further lockdown, a large part of which is the anticipated mental health burden. Perhaps psychological elements of life as a doctor will also get increased recognition in medical training and doctors will be taught to identify and reflect on their emotional responses. Space and infrastructure may also be introduced so doctors are able to have important conversations about wellbeing as a matter of routine.
