{"id":49860,"date":"2021-03-18T17:56:24","date_gmt":"2021-03-18T16:56:24","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49860"},"modified":"2021-03-23T09:22:28","modified_gmt":"2021-03-23T08:22:28","slug":"people-with-learning-disabilities-are-equal-members-of-society-not-vulnerable-burdens","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/03\/18\/people-with-learning-disabilities-are-equal-members-of-society-not-vulnerable-burdens\/","title":{"rendered":"People with learning disabilities are equal members of society, not vulnerable burdens\u00a0\u00a0\u00a0\u00a0\u00a0"},"content":{"rendered":"<p class=\"standfirst\"><span style=\"font-weight: 400\">The battle to prioritise people with learning disabilities for the covid vaccine may be won, but did negative stereotypes and outdated prejudices have to be reinforced to get there?<\/span><\/p>\n<p><!--more--><span style=\"font-weight: 400\">Over the past year, people with learning disabilities have been told to \u201cshield\u201d and \u201cself-isolate,\u201d with little accessible communication about what these terms mean.<\/span> <span style=\"font-weight: 400\">Some have had <\/span><a href=\"https:\/\/www.learningdisabilitytoday.co.uk\/do-not-resuscitate-orders-and-learning-disability-where-are-we-now\"><span style=\"font-weight: 400\">inappropriate \u201cdo not attempt cardiopulmonary resuscitation\u201d notices<\/span><\/a><span style=\"font-weight: 400\"> placed on their hospital files, and many have died in much higher numbers than the general population, with researchers estimating that people with a learning disability <\/span><a href=\"https:\/\/www.gov.uk\/government\/news\/people-with-learning-disabilities-had-higher-death-rate-from-covid-19\"><span style=\"font-weight: 400\">have had a six times higher death rate from covid-19<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In recent weeks high profile figures have lent their voices to the call for this group to be vaccinated against covid-19 without delay. Celebrities like Jo Whiley and Ian Rankin, respectively the sister and father of a person with a severe learning disability and both Mencap ambassadors, made public pleas for this group to be prioritised for the vaccine. No sooner had Jo Whiley spoken up than the nation looked on as her sister Frances contracted <\/span><span style=\"font-weight: 400\">covid-19. The intense media attention that accompanied Frances\u2019s illness<\/span> <span style=\"font-weight: 400\">achieved more in a short time than families, scientists, and campaigners had over months of imploring the <\/span><span style=\"font-weight: 400\">government to pay more attention to the needs of people with learning disabilities.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Now that people with learning disabilities (or rather <\/span><a href=\"https:\/\/www.nhs.uk\/Scorecard\/Pages\/IndicatorFacts.aspx?MetricId=640&amp;orgType=null\"><span style=\"font-weight: 400\">the quarter to a third who are recognised as such<\/span><\/a><span style=\"font-weight: 400\"> on GP registers) are prioritised for vaccination, it is time to examine what has been gained and lost in the process.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We should make space to hear the experiences of <\/span><a href=\"https:\/\/www.nhs.uk\/conditions\/learning-disabilities\/#:~:text=Around%201.5%20million%20people%20in,have%20a%20severe%20learning%20disability.\"><span style=\"font-weight: 400\">the 1.5 million people<\/span><\/a><span style=\"font-weight: 400\"> in the UK who have a learning disability but whose voices have<\/span> <span style=\"font-weight: 400\">been, once again, almost entirely absent in the media reporting. For the four of us (CB, AB, HR, and PD) who speak as people with a learning disability, we see too little progress. Some people with learning disabilities<\/span> <span style=\"font-weight: 400\">are very vulnerable, but many of us can stand up for ourselves and<\/span> <span style=\"font-weight: 400\">we can<\/span> <span style=\"font-weight: 400\">contribute to our communities. Narratives that frame us as<\/span> <span style=\"font-weight: 400\">a burden on our families or the NHS negate our humanity and worth as<\/span> <span style=\"font-weight: 400\">loved and valued individuals.<\/span> <span style=\"font-weight: 400\">It\u2019s<\/span> <span style=\"font-weight: 400\">2021:<\/span> <span style=\"font-weight: 400\">there are 1.5 million of us in the UK and we have a right<\/span> <span style=\"font-weight: 400\">to be treated better and more fairly.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/chrishatton.blogspot.com\/\"><span style=\"font-weight: 400\">Researchers<\/span><\/a><span style=\"font-weight: 400\"> have long alerted the NHS and healthcare providers to the <\/span><span style=\"font-weight: 400\">crushing health inequalities<\/span><span style=\"font-weight: 400\"> experienced by people with learning disabilities<\/span><span style=\"font-weight: 400\">. Society as a whole and the government have largely turned a blind eye to the severe deprivation, social inequalities, and multiple barriers to accessing good quality health and social care which are the norm and not the exception for people with learning disabilities. Michael Marmot, a leading researcher on health inequalities, aptly noted in introducing <\/span><a href=\"http:\/\/www.instituteofhealthequity.org\/resources-reports\/a-fair-supportive-society-summary-report\"><span style=\"font-weight: 400\">an NHS commissioned report<\/span><\/a> <span style=\"font-weight: 400\">that this population group are more likely to experience <\/span><span style=\"font-weight: 400\">\u201c<\/span><span style=\"font-weight: 400\">some of the worst of what <\/span><span style=\"font-weight: 400\">society has to offer,\u201d including <\/span><span style=\"font-weight: 400\">low incomes, unemployment, poor housing, social isolation and loneliness, bullying, and abuse. The same report called for more efforts<\/span> <span style=\"font-weight: 400\">to integrate people with learning disabilities into society and to reduce stigma and discrimination as a way to <\/span><span style=\"font-weight: 400\">improve their lives and health outcomes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Unfortunately, during the pandemic, people with learning disabilities\u2019<\/span> <span style=\"font-weight: 400\">increased risks from <\/span><span style=\"font-weight: 400\">covid-19 haven\u2019t been understood and presented as an outcome that takes place against a background of severe health inequalities.<\/span> <span style=\"font-weight: 400\">Instead, it was recourse to the traditional story of people with learning disabilities as \u201cvulnerable\u201d and \u201chelpless\u201d that dominated in the media, and which finally moved the government to act. This story once again minimises the agency and capabilities of people with learning disabilities.<\/span> <span style=\"font-weight: 400\">It\u2019s yet another pillar propping up the<\/span> <span style=\"font-weight: 400\">negative stereotypes that<\/span> <span style=\"font-weight: 400\">people with learning disabilities and their allies have challenged for the past few decades.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Our <\/span><a href=\"https:\/\/www.storm-ucl.com\/team\"><span style=\"font-weight: 400\">STORM project<\/span><\/a><span style=\"font-weight: 400\"> places people with learning disabilities at the centre of efforts to challenge the prejudice and discrimination they still frequently face in their everyday lives. As we say to anyone with a learning disability in the introduction to our STORM manual, borrowing the words of the Nobel Prize winning writer Chimamanda Ngozie Adichie: \u201cYour solid unbending belief should be: I matter. I matter equally. Not \u2018if only.\u2019 Not \u2018as long as.\u2019 I matter equally. Full stop.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">If this statement sits uncomfortably with healthcare providers and policy makers when thinking of people with learning disabilities, we invite them to reflect on their perceptions. What is it that leads you to think of this population as lesser? Why do you address their parent or supporter when they present in your surgery or hospital? What might you and many of your patients gain if you stopped for a moment, slowed down a little, and used straightforward words to explain things? Doing so would not only pay people with learning disabilities the respect they deserve, but also benefit the other <\/span><a href=\"https:\/\/literacytrust.org.uk\/parents-and-families\/adult-literacy\/#:~:text=16.4%25%20of%20adults%20in%20England,having%20%27very%20poor%20literacy%20skills.\"><span style=\"font-weight: 400\">8.5 million people<\/span><\/a><span style=\"font-weight: 400\"> in the UK whose literacy skills are very poor.<\/span><\/p>\n<p><span style=\"font-weight: 400\">We await the rollout of the new <\/span><a href=\"https:\/\/www.hee.nhs.uk\/our-work\/learning-disability\"><span style=\"font-weight: 400\">Oliver McGowan mandatory training<\/span><\/a><span style=\"font-weight: 400\"> in learning disability and autism for all health and social care providers. In the meantime, <\/span><a href=\"https:\/\/www.gmc-uk.org\/ethical-guidance\/ethical-hub\/learning-disabilities\"><span style=\"font-weight: 400\">GMC guidance<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.bmj.com\/content\/355\/bmj.i5296\"><span style=\"font-weight: 400\">patients with learning disabilities<\/span><\/a><span style=\"font-weight: 400\"> offer some straightforward tips for healthcare providers on<\/span> <span style=\"font-weight: 400\">how to make reasonable adjustments and avoid<\/span> <span style=\"font-weight: 400\">discrimination.
