{"id":49734,"date":"2021-03-08T17:46:04","date_gmt":"2021-03-08T16:46:04","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49734"},"modified":"2021-05-28T19:22:28","modified_gmt":"2021-05-28T18:22:28","slug":"the-hesitancy-paradox-of-data-not-dates-and-the-governments-roadmap-to-unlocking","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/03\/08\/the-hesitancy-paradox-of-data-not-dates-and-the-governments-roadmap-to-unlocking\/","title":{"rendered":"The hesitancy paradox of &#8220;data, not dates&#8221; and the government\u2019s roadmap to unlocking"},"content":{"rendered":"<p class=\"standfirst\">It&#8217;s a mistake for the government to be so opaque about the data they&#8217;ll use to guide our exit from lockdown, writes Andy Cowper<\/p>\n<p><!--more--><span style=\"font-weight: 400\">As schools return in England, the government\u2019s <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/www.gov.uk\/government\/publications\/covid-19-response-spring-2021\/covid-19-response-spring-2021\">roadmap<\/a> to lifting covid-19 restrictions comes into sharper focus. There is, of course, a risk that the return of in-person education for all in schools (rather than just for vulnerable children or the children of key workers, as previously) will drive an increase in the transmission of covid-19.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The principle expounded by prime minister Boris Johnson at the roadmap&#8217;s launch was that the lifting of lockdown restrictions would be driven by \u201cdata, not dates.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400\">Inevitably, the four stages listed in this document (of which stage one is actually two separate stages: the 8 March &#8220;schools reopening&#8221; and the 29 March &#8220;rule of six\/two families outdoors\/outdoor sport&#8221;) were accompanied by indicative dates, albeit with an emphasis of \u201cno earlier than\u201d and a five week gap between each to assess the impacts of the last stage\u2019s easing of restrictions. This five week gap provides the government with four weeks\u2019 data to allow assessment of the last changes&#8217; impact; followed by one week\u2019s notice for businesses and communities to prepare.<\/span><\/p>\n<p><span style=\"font-weight: 400\">To guide its assessment of progress, the government set itself four rules:<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The vaccine deployment programme continues successfully.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Evidence shows vaccines are sufficiently effective in reducing hospitalisations and deaths in those vaccinated.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Infection rates do not risk a surge in hospitalisations, which would put unsustainable pressure on the NHS.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Our assessment of the risks is not fundamentally changed by new &#8220;variants of concern.&#8221;<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400\">That all seems quite sensible. What is more opaque, however, is the actual data and thresholds that will be used to guide their decision making. It is not obviously clear why this should not be clarified.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Given the levels of anxiety or paranoia among some parts of the community about covid-19 being a hoax (which, of course, it isn&#8217;t), and that <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2020.12.27.20248899v1\">research shows that<\/a> \u201c<\/span><span style=\"font-weight: 400\">gender, education, and ethnicity were independently associated with vaccine hesitancy,\u201d this feels like a mistake. <\/span><span style=\"font-weight: 400\">Research has also shown that <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/www.bmj.com\/content\/372\/bmj.n460\">vaccine take-up is lower<\/a> among ethnic minority NHS staff.<\/span><\/p>\n<p><b>The hesitancy paradox<\/b><\/p>\n<p><span style=\"font-weight: 400\">More than a mistake, the failure to release this information could actually become a paradox. To date, take-up of the vaccine has been high: <a href=\"https:\/\/www.england.nhs.uk\/statistics\/statistical-work-areas\/covid-19-vaccinations\/\">the figures show <\/a><\/span><span style=\"font-weight: 400\">well over 90% in the 65+ age groups targeted so far. This is unsurprising, as the most vulnerable groups have been first to be offered the vaccine: those at highest risk of serious illness can most see its importance to them. Analyst Duncan Robertson has offered <a href=\"https:\/\/twitter.com\/Dr_D_Robertson\/status\/1367504221424214019\">pertinent analysis<\/a> of the take-up data<\/span><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Yet given that vaccine hesitancy lies more in the populations yet to be vaccinated, it would obviously mitigate against the government\u2019s first self-test if the lack of maximum transparency about the data and criteria to be used were to entrench these worries\u2014however scientifically illiterate they may be.<\/span><\/p>\n<p><span style=\"font-weight: 400\">We also need to bear in mind the legacy of public concern about the government\u2019s handling of the pandemic so far. <\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/www.kcl.ac.uk\/news\/majority-of-britons-distrust-government-covid-response-for-first-time\">Research in December 2020<\/a> found that the majority of UK respondents distrusted the government\u2019s handling of the pandemic: this distrust had doubled since the start of the pandemic. This study also found that <\/span><span style=\"font-weight: 400\">nearly two thirds (64%) of respondents thought that the UK government had failed to prepare properly for a second wave of SARS-CoV-2 infections.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The vaccination programme and its procurement have been rare bright spots in the government&#8217;s pandemic response. For many, the refusal to take SAGE<\/span><span style=\"font-weight: 400\">\u2019s <a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/925854\/S0769_Summary_of_effectiveness_and_harms_of_NPIs.pdf\">September 2020 advice<\/a> for a circuit breaker lockdown, and the subsequent rising tide of infections (clashing with the government\u2019s <a href=\"https:\/\/inews.co.uk\/news\/politics\/boris-johnson-plans-save-christmas-covid-restrictions-england-lockdown-762969\">&#8220;save Christmas&#8221;
