{"id":49698,"date":"2021-02-25T19:53:50","date_gmt":"2021-02-25T18:53:50","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49698"},"modified":"2021-03-03T15:12:59","modified_gmt":"2021-03-03T14:12:59","slug":"is-englands-exit-plan-from-lockdown-a-road-to-nowhere","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/02\/25\/is-englands-exit-plan-from-lockdown-a-road-to-nowhere\/","title":{"rendered":"Is England\u2019s exit plan from lockdown a road to nowhere?"},"content":{"rendered":"<p class=\"standfirst\">The more we accept our coexistence with covid, the more it becomes an inevitability, writes John Middleton<\/p>\n<p><!--more--><span style=\"font-weight: 400\">There has been much talk about Boris Johnson\u2019s<\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2021\/feb\/22\/the-guardian-view-on-boris-johnsons-covid-plan-a-risky-bet-on-vaccines?CMP=Share_iOSApp_Other\"><span style=\"font-weight: 400\"> negotiated plan for taking England out of lockdown<\/span><\/a><span style=\"font-weight: 400\">, and it seems the scientists have won over <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/feb\/13\/tory-mps-tell-johnson-to-commit-to-lifting-covid-restrictions-by-end-of-april\"><span style=\"font-weight: 400\">the political pressures to relax restrictions<\/span><\/a><span style=\"font-weight: 400\"> quickly<\/span><span style=\"font-weight: 400\">. We know that <\/span><a href=\"https:\/\/ourworldindata.org\/grapher\/q2-gdp-growth-vs-confirmed-deaths-due-to-covid-19-per-million-people?tab=chart&amp;stackMode=absolute&amp;country=&amp;region=World\"><span style=\"font-weight: 400\">national economies have done better<\/span><\/a><span style=\"font-weight: 400\"> where the virus has been suppressed to very low levels so the aims of public health and economics are the same in this case. The prime minister is to be encouraged in his efforts not to make the mistakes of the past, but the virus is not negotiating. England\u2019s roadmap still contains major weaknesses, which threaten to see us <\/span><a href=\"https:\/\/inews.co.uk\/opinion\/lockdown-roadmap-covid-restrictions-plan-public-compliance-882366\"><span style=\"font-weight: 400\">bounce back into future lockdowns<\/span><\/a><span style=\"font-weight: 400\"> because there is not the political will to stamp out the virus.\u00a0<\/span><\/p>\n<p><b>The road is running on weak foundations<\/b><\/p>\n<p><span style=\"font-weight: 400\">Community infection rates are still nearly four times the level they were when lockdown was reduced last May. The <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/datasets\/coronaviruscovid19infectionsurveydata\"><span style=\"font-weight: 400\">ONS community infection survey<\/span><\/a><span style=\"font-weight: 400\"> estimates that there were 657 600 infections in the two weeks to 12 February, compared with 184 400 in the two middle weeks of May 2020, and 25 900 at our lowest levels at the end of June 2020. Modelling assumptions show just how precarious covid-19\u2019s level of transmission is\u2014coming down but able <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/feb\/22\/relaxing-covid-rules-too-early-likely-to-bring-huge-death-toll-scientists-say\"><span style=\"font-weight: 400\">to shoot back up with any weakening of current resolve<\/span><\/a><span style=\"font-weight: 400\">. The <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/963565\/S1130_SPI-M-O_Summary_of_further_modelling_of_easing_restrictions.pdf\"><span style=\"font-weight: 400\">SAGE\/SCI-MO modelling scenarios<\/span><\/a><span style=\"font-weight: 400\"> released on Monday show how even under the most optimistic conditions, we can still expect thousands more people dying from covid-19. The government cites improvements in test and trace, but it is still inadequate. <\/span><a href=\"https:\/\/www.bmj.com\/content\/372\/bmj.n224\"><span style=\"font-weight: 400\">Support for self-isolation<\/span><\/a><span style=\"font-weight: 400\"> is also still not sufficient to enable people to stay off work and is bureaucratically operated. Reliance on lateral flow testing risks false negatives and clusters of new cases.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">With community infection rates so stubbornly high it is entirely fair to ask, why relax now? What is imperative enough not to continue lockdown, through to the Easter holiday and get the community levels down as low as possible? There have been extraordinary commitments made across the breadth of society and to throw those away before the virus is returned even to the low levels of the last summer would be to <\/span><a href=\"https:\/\/www.theguardian.com\/commentisfree\/2021\/feb\/21\/uk-lockdown-health-livelihoods-2021-sage-committee\"><span style=\"font-weight: 400\">waste our communal sacrifice<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><b>The road is uneven<\/b><\/p>\n<p><span style=\"font-weight: 400\">England&#8217;s data on cases, deaths, and hospitalisations assemble together a national position that is made up of varying pictures around the country and in different communities. Areas where vaccination rates are lower, added to areas where people belong to higher risk ethnic minority groups and work in higher risk environments, will undermine progress in reducing the virus. These pockets provide foci for new outbreaks, potentially in younger ages, leaving us vulnerable to new variants.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.medicalnewstoday.com\/articles\/covid-19-vaccine-live-updates\"><span style=\"font-weight: 400\">Three promising papers<\/span><\/a><span style=\"font-weight: 400\"> were published earlier this week showing that vaccines do what they should in real world conditions. But we still do not know how long immunity will last in vaccinated individuals. It\u2019s not a case of either vaccination or containment strategies to control covid; we need both to give us our best chance of success.<\/span><\/p>\n<p><b>The pandemic is not a straight road\u00a0\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">Doing a deal with the virus is not a linear process\u2014if we do this, the virus will do that. People continue to be surprised at the ever changing pattern of the virus\u2019s behaviour, yet new variants will continue to appear; some will be more infectious (like the <\/span><a href=\"https:\/\/www.forbes.com\/sites\/carlieporterfield\/2021\/01\/28\/more-contagious-uk-coronavirus-strain-has-been-detected-in-over-half-of-us-states\/\"><span style=\"font-weight: 400\">Kent<\/span><\/a><span style=\"font-weight: 400\"> variant<\/span><span style=\"font-weight: 400\">) and some may be more deadly (as early reports show the <\/span><a href=\"https:\/\/www.sciencemag.org\/news\/2021\/02\/coronavirus-strain-first-identified-california-may-be-more-infectious-and-cause-more\"><span style=\"font-weight: 400\">California<\/span><\/a><span style=\"font-weight: 400\"> strain may be<\/span><span style=\"font-weight: 400\">).<\/span><\/p>\n<p><b>Driving on the right\u2014the schools big bang\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">While the roadmap may be cautious, its first action clearly isn\u2019t. Sending all children back to schools in the so called \u201cbig bang\u201d is a major risk. In Johnson\u2019s roadmap, we are all eventually going to drive on the right hand side; the schools are going to do it first, and if that succeeds we will all do it.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The government has set out a compelling case for children to be in school; we can all agree how important it is for their learning and education, and even more so on the grounds of its impact on children\u2019s <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/lanpsy\/article\/PIIS2215-0366(20)30570-8\/fulltext\"><span style=\"font-weight: 400\">socialisation and mental health<\/span><\/a><span style=\"font-weight: 400\">. Five year olds have lost a fifth of their lives to covid-19. It has long been accepted that children generally are not severely affected by the infection and more evidence is appearing that suggests children are <\/span><a href=\"https:\/\/adc.bmj.com\/content\/106\/2\/111\"><span style=\"font-weight: 400\">not major sources of spread<\/span><\/a><span style=\"font-weight: 400\">. However, schools are major foci for gatherings of adults in any community; their opening involves teachers, parents, services, and all will be major users of public transport. <\/span><a href=\"https:\/\/www.thelancet.com\/journals\/laninf\/article\/PIIS1473-3099(20)30882-3\/fulltext\"><span style=\"font-weight: 400\">High circulating levels of virus in the community can interact<\/span><\/a><span style=\"font-weight: 400\"> with the spread of virus in schools. <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-scotland-55918154\"><span style=\"font-weight: 400\">Scotland<\/span><\/a><span style=\"font-weight: 400\"> has opted for a phased return of children to school. While Israel, further ahead than the UK in vaccinating its population, began by only <\/span><a href=\"https:\/\/www.haaretz.com\/israel-news\/israel-reopens-with-more-covid-rules-relaxed-here-s-everything-you-need-to-know-1.9553867\"><span style=\"font-weight: 400\">reopening schools for preschool children to year 4.<\/span><\/a><span style=\"font-weight: 400\"> The government\u2019s justification for the big bang seems weak and high risk.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">And are we assured that everything has been done that could be done to create safe environments? Not really. It appears that schools will be going back with very few additional protections. Masks are required in classrooms for secondary school pupils but not for others. The vaccination of teachers is not planned, even though teachers are now the <\/span><a href=\"https:\/\/www.ons.gov.uk\/peoplepopulationandcommunity\/healthandsocialcare\/conditionsanddiseases\/articles\/coronaviruscovid19infectionsinthecommunityinengland\/characteristicsofpeopletestingpositiveforcovid19inengland22february2021\"><span style=\"font-weight: 400\">fourth highest occupational group<\/span><\/a><span style=\"font-weight: 400\"> likely to<\/span><span style=\"font-weight: 400\"> test positive for covid-19. Lateral flow <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-england-56170736\"><span style=\"font-weight: 400\">testing in secondary schools<\/span><\/a><span style=\"font-weight: 400\"> for staff and pupils presents a &#8220;logistical nightmare,&#8221; and <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2021\/01\/12\/covid-19-government-must-urgently-rethink-lateral-flow-test-roll-out\/\"><span style=\"font-weight: 400\">the tests cannot be relied on<\/span><\/a><span style=\"font-weight: 400\">. False negatives mean fresh outbreaks could kick off. During lockdown, there appears to have been very little extra provision for <\/span><a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.1080\/02786826.2021.1877257\"><span style=\"font-weight: 400\">improving ventilation<\/span><\/a><span style=\"font-weight: 400\"> in classrooms<\/span><span style=\"font-weight: 400\">, despite the roadmap acknowledging the role of aerosol transmission in spreading the virus. The government has given no consideration to a phased return with alternating year groups, rotas, or shortened daytime attendance.\u00a0<\/span><\/p>\n<p><b>Road blocks\u2014messaging\u00a0\u00a0<\/b><\/p>\n<p><span style=\"font-weight: 400\">The prime minister seems to be seeking to avoid the mistakes of the past, yet despite his talk of going slowly and leaving gaps between the new rules to measure their effect, this was at odds with other parts of his rhetoric. What, for example, will some people have made of talk of the plan being<\/span><a href=\"https:\/\/www.reuters.com\/article\/us-health-coronavirus-britain\/cautious-but-irreversible-johnson-sets-out-englands-lockdown-exit-plan-idUKKBN2AL0NT?edition-redirect=uk\"><span style=\"font-weight: 400\"> \u201ccautious, but also irreversible\u201d? <\/span><\/a><span style=\"font-weight: 400\">\u00a0It\u2019s perhaps no wonder that many people will be working to the dates set out in the plan, rather than the cautions of \u201cno sooner than.\u201d The \u201c<\/span><a href=\"https:\/\/www.independent.co.uk\/voices\/lockdown-roadmap-boris-johnson-covid-deaths-b1805845.html\"><span style=\"font-weight: 400\">one way road to freedom<\/span><\/a><span style=\"font-weight: 400\">\u201d is likely to join \u201cHappy Monday,\u201d \u201c<\/span><a href=\"https:\/\/www.theneweuropean.co.uk\/brexit-news\/boris-johnson-politico-claim-on-ventilators-74216\"><span style=\"font-weight: 400\">Operation Last Gasp<\/span><\/a><span style=\"font-weight: 400\">\u201d and \u201c<\/span><a href=\"https:\/\/news.sky.com\/video\/coronavirus-prime-minister-says-well-be-doing-whack-a-mole-on-local-flare-ups-11986745\"><span style=\"font-weight: 400\">Whack a mole<\/span><\/a><span style=\"font-weight: 400\">,\u201d and \u201c<\/span><a href=\"https:\/\/www.independent.co.uk\/news\/uk\/politics\/boris-johnson-christmas-covid-restrictions-b1775062.html\"><span style=\"font-weight: 400\">Have yourself a merry little Christmas and I do mean little.\u201d<\/span><\/a><span style=\"font-weight: 400\"> We can all recognise the desire to do something, but the government needs to emphasise that it is still more in hope, than in knowledge and expectation. Maybe it is the best we can expect given the political pressures within government.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The more we accept our coexistence with covid, the more it becomes an inevitability and a self-fulfilling prophecy.
