{"id":49695,"date":"2021-02-25T18:30:24","date_gmt":"2021-02-25T17:30:24","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49695"},"modified":"2021-02-26T12:43:54","modified_gmt":"2021-02-26T11:43:54","slug":"karl-friston-and-anthony-costello-a-measured-approach-to-zero-covid","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/02\/25\/karl-friston-and-anthony-costello-a-measured-approach-to-zero-covid\/","title":{"rendered":"Karl Friston and Anthony Costello: A measured approach to zero covid"},"content":{"rendered":"<p class=\"standfirst\">It\u2019s possible for a context-sensitive containment strategy to not only work in principle, but in practice, say <strong>Karl Friston and Anthony Costello<\/strong><\/p>\n<p><!--more--><span style=\"font-weight: 400\">As the <a href=\"https:\/\/www.gov.uk\/government\/publications\/covid-19-response-spring-2021\">government reveals its roadmap<\/a> for unlocking<\/span><span style=\"font-weight: 400\"> England&#8217;s nationwide lockdown, a meme has emerged in response: &#8220;<a href=\"https:\/\/www.theguardian.com\/politics\/2021\/feb\/19\/data-not-dates-expert-views-on-how-to-lift-englands-covid-lockdown\">data not dates<\/a>.&#8221;<\/span><span style=\"font-weight: 400\"> The roadmap entails a measured and cautious approach to the phased lifting of restrictions\u2014that is underwritten by quantitative measures of vaccination, variants, and viral spread. This approach would be fully endorsed by epidemiologists and <a href=\"https:\/\/spectrum.ieee.org\/biomedical\/diagnostics\/how-control-theory-can-help-control-covid19\">control theorists<\/a><\/span><span style=\"font-weight: 400\"><a href=\"https:\/\/spectrum.ieee.org\/biomedical\/diagnostics\/how-control-theory-can-help-control-covid19\"> alike<\/a>\u2014namely, make a move, see what happens, and respond in a way that controls viral spread. Perhaps a useful metaphor here is a thermostat: when things get too hot, switch the heating off\u2014and switch it on again when the temperature falls. This is known as <a href=\"https:\/\/en.wikipedia.org\/wiki\/Bang%E2%80%93bang_control\">bang-bang control<\/a> in engineering<\/span><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The measured response announced by the government\u2014with four phases, interspersed by several weeks\u2014has the latitude to implement this approach: reopen schools, see what happens, legalise outdoor socialising, see what happens, and so on. So, what are the measures in question?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Several measures are implicit in the four tests that have been adopted: namely, vaccination rollout and efficacy, supplemented with measures of viral spread and evolution. Ultimately, these tests rest upon the prevalence of infection, which speaks to the quantitative criteria used to licence escalation\u2014or de-escalation\u2014of unlocking. This has led some to posit thresholds that would trigger, defer, or revert mitigating restrictions. For example, <a href=\"https:\/\/www.independentsage.org\/a-sustainable-suppression-strategy-for-keeping-society-open\/\">Independent SAGE has suggested<\/a> an incidence threshold of 100 cases per 100 000 population for the reopening of schools. At the other end of the scale, a threshold of 10 cases per 100 000 population has been proposed as a criterion for relaxing all exceptional measures (excepting social distancing, mask wearing, etc) in so-called &#8220;green zones<\/span><span style=\"font-weight: 400\">.&#8221; One might ask, where do these thresholds come from?<\/span><\/p>\n<p><span style=\"font-weight: 400\">In part, they reflect <a href=\"https:\/\/www.ecdc.europa.eu\/en\/covid-19\/situation-updates\/weekly-maps-coordinated-restriction-free-movement\">received wisdom<\/a>,<\/span><span style=\"font-weight: 400\"> and in part, they&#8217;re inherited from straightforward epidemiology. If the aim is to preclude exponential growth, then the threshold is simple to specify: when R is one, the incidence of new cases is exactly balanced by the rate at which people recover or die; namely, the prevalence divided by the mean period of infectiousness<\/span><span style=\"font-weight: 400\">.* With <a href=\"https:\/\/bmjopen.bmj.com\/content\/10\/8\/e039856\">a period of infectiousness of about five days<\/a><\/span><span style=\"font-weight: 400\">, the critical incidence is 1\/5 of the prevalence. For example, a prevalence of 1% gives a daily incidence of 0.2%\u2014or 200 cases per 100 000 population per day. This could furnish a threshold for opening versus closing schools. However, the implicit bang-bang control will, by construction, not reduce prevalence because prevalence will be maintained at the controlled setpoint of 1%.<\/span><span style=\"font-weight: 400\"> To implement a suppression strategy\u2014that minimises incidence and prevalence\u2014one can halve the threshold, giving 100 cases per 100 000 population per day.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There are two important aspects of this approach to controlled suppression: first, the incidence threshold\u2014that licences progressive unlocking\u2014depends upon prevalence. Under the above analysis, it is 1\/10 of the prevalence. With successful implementation, the prevalence and accompanying threshold will fall, ensuring a graceful transition from an epidemic to an endemic phase. Technically, the endpoint is a minimum-prevalence endemic equilibrium. This could be read as the aspiration of zero covid, where the zero pertains to growth (that may or may not entail zero prevalence). The second key issue is that to implement bang-bang control, one needs real time measures of incidence. Retrospective estimates of R\u2014or long term outcomes like hospital admissions\u2014are not apt because they reflect states of affairs several weeks previously. So, where might these measures come from?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Clearly, they should not be based upon the number of confirmed cases (a.k.a. notification rate), but on estimates of the true incidence and prevalence. This follows because confirmed cases underestimate incidence when prevalence is high\u2014because most cases are missed\u2014and overestimate incidence when prevalence is low\u2014because <a href=\"https:\/\/www.thelancet.com\/journals\/lanres\/article\/PIIS2213-2600(20)30453-7\/fulltext\">tests have a finite specificity<\/a><\/span><span style=\"font-weight: 400\">.** Happily, <a href=\"https:\/\/www.fil.ion.ucl.ac.uk\/spm\/covid-19\/dashboard\/local\/\">real time estimates are available<\/a>, down to the area of (lower tier) local authorities<\/span><span style=\"font-weight: 400\">. These estimates are, effectively, the best guess of incidence and prevalence that explains all the data at hand.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Estimates of this sort anticipate a containment strategy that is context sensitive, which can use targeted responses<\/span><span style=\"font-weight: 400\"> and lifting of restrictions (for example, surge testing, school closures, enhanced support for social isolation, etc.), depending upon the local level of prevalence. This brings us to the final issue: national versus local? A control theoretic approach would suggest that the same principles apply at all scales. This might lead to the paradoxical situation where life could return to normal in many &#8220;green zone&#8221; local authorities, while the UK remained under quarantine because &#8220;red zones&#8221; remain somewhere. In fact, this situation is not paradoxical but entirely sensible\u2014and is currently enjoyed by countries like Australia and previously by South Korea, China, and <a href=\"https:\/\/qap.ecdc.europa.eu\/public\/extensions\/COVID-19\/COVID-19.html#global-overview-tab\">Germany<\/a><\/span><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Various projections have been made about the number of deaths we can expect to see from further covid-19 surges unless a cautious approach to unlocking is followed. These projections depend upon the timeframe in question; the risk of new variants increasing transmissibility or escaping the vaccine; and upon the success of community based services to find, test, trace, and isolate. If we were able to leverage the <a href=\"https:\/\/covid.i-sense.org.uk\/\">untapped efficacy<\/a> of supported isolation<\/span><span style=\"font-weight: 400\">, the &#8220;cautious&#8221; approach could be the &#8220;<a href=\"https:\/\/www.medrxiv.org\/content\/10.1101\/2021.01.03.20248972v1\">quickest<\/a>&#8221; to having covid under control.<\/span><\/p>\n<p><span style=\"font-weight: 400\">It&#8217;s possible for a context-sensitive containment strategy\u2014based upon real time, prevalence dependent incidence thresholds\u2014to not only work in principle, but in practice. We can see this by the example of countries that have successfully suppressed viral spread.<\/span><\/p>\n<p><i><span style=\"font-weight: 400\"><strong>Karl J Friston<\/strong><\/span><\/i><em><span style=\"font-weight: 400\">, <\/span><\/em><i><span style=\"font-weight: 400\">s<\/span><\/i><i><span style=\"font-weight: 400\">cientific director, Wellcome Centre for Human Neuroimaging.
