{"id":49661,"date":"2021-02-24T16:28:34","date_gmt":"2021-02-24T15:28:34","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49661"},"modified":"2021-03-01T10:07:57","modified_gmt":"2021-03-01T09:07:57","slug":"in-the-covid-19-era-we-all-need-to-understand-public-health-especially-politicians","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/02\/24\/in-the-covid-19-era-we-all-need-to-understand-public-health-especially-politicians\/","title":{"rendered":"In the covid-19 era we all need to understand public health, especially politicians"},"content":{"rendered":"<p class=\"standfirst\">We are all public health workers now, but politicians need to step up, says <strong>Graham Mackenzie<\/strong><\/p>\n<p><!--more--><span style=\"font-weight: 400\">In the years before the covid-19 pandemic, I used to tell colleagues in other parts of the NHS, local authorities, and voluntary organisations that we are all \u201cpublic health workers.\u201d They didn\u2019t work in public health departments, but they still had an impact on population health and followed a number of public health principles, even if they might not recognise them as such.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The pandemic has demonstrated that across society we all need to understand public health. One simple model looks at <\/span><a href=\"https:\/\/scotpublichealth.com\/2017\/04\/17\/what-is-public-health-some-reflections-for-teaching\/\"><span style=\"font-weight: 400\">three overlapping domains<\/span><\/a>\u2014<span style=\"font-weight: 400\">health protection, health improvement, and quality improvement\u2014always considering their impact at a population level, including inequalities and vulnerable groups.\u00a0<\/span><\/p>\n<p>Health protection<span style=\"font-weight: 400\"> has become an increasingly familiar topic during the pandemic with lockdown, quarantine, vaccination, and other measures introduced to reduce the spread of infection both with individual cases and in outbreaks. Health protection includes communicable disease control and environmental health (e.g. air quality, food safety). The pandemic has highlighted the connections between these areas, for example, with large <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/05\/29\/learning-about-covid-19-outbreaks-from-non-viral-tweets\/\"><span style=\"font-weight: 400\">covid-19 outbreaks in meat processing plants<\/span><\/a><span style=\"font-weight: 400\"> spreading infection into rural communities. Some health protection measures such as quarantine have been used for centuries and much of the legislation we rely on to protect against communicable disease was first introduced in the 19<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> century with the <\/span><a href=\"https:\/\/www.parliament.uk\/about\/living-heritage\/transformingsociety\/towncountry\/towns\/tyne-and-wear-case-study\/about-the-group\/public-administration\/the-1848-public-health-act\/\"><span style=\"font-weight: 400\">1848 Public Health Act<\/span><\/a><span style=\"font-weight: 400\">, and has been <\/span><a href=\"https:\/\/www.gov.uk\/government\/organisations\/public-health-england\/about\/our-governance\"><span style=\"font-weight: 400\">updated since<\/span><\/a><span style=\"font-weight: 400\">. Further legislation (e.g. the <\/span><a href=\"https:\/\/www.parliament.uk\/globalassets\/documents\/post\/pn188.pdf\"><span style=\"font-weight: 400\">Clean Air Acts of 1956 and 1958<\/span><\/a><span style=\"font-weight: 400\">) was passed as new environmental threats were identified.\u00a0<\/span><\/p>\n<p>Health improvement<span style=\"font-weight: 400\"> encompasses the <\/span><a href=\"https:\/\/www.nhsggc.org.uk\/your-health\/public-health\/health-improvement\/\"><span style=\"font-weight: 400\">range of activities<\/span><\/a><span style=\"font-weight: 400\"> required to improve people&#8217;s health and life circumstances. While this is sometimes viewed narrowly as health education focusing on individual choices, this domain should also include the regulation, fiscal measures (price and tax), and legislation required to achieve health improvement at a population level. A good example of the combined individual and population level approach required for health improvement is provided by successes in <\/span><a href=\"https:\/\/tobaccocontrol.bmj.com\/content\/17\/4\/230\"><span style=\"font-weight: 400\">reducing smoking over the past 50 years<\/span><\/a><span style=\"font-weight: 400\">. Michael Marmot neatly summarises the perils of focusing on individual change without addressing population level factors as \u201c<\/span><a href=\"http:\/\/www.instituteofhealthequity.org\/\"><span style=\"font-weight: 400\">lifestyle drift.<\/span><\/a><span style=\"font-weight: 400\">\u201d Just as an individual cannot readily improve the air quality of the community where they live (through no fault of their own), it is very difficult for somebody in precarious employment or damp housing to change their circumstances without considerable support.\u00a0<\/span><\/p>\n<p>Quality improvement<span style=\"font-weight: 400\"> is sometimes called \u201chealth service quality improvement.\u201d However, as public health needs to consider the social determinants of health\u2014e.g. housing, education, employment, income, environment, and the economy\u2014I usually drop the \u201chealth service\u201d tag. <\/span><a href=\"http:\/\/www.1000livesplus.wales.nhs.uk\/sitesplus\/documents\/1011\/Quality%20Improvement%20Guide%20-%203rd%20edition%20%28IQT%29%20WEB.pdf\"><span style=\"font-weight: 400\">Quality improvement<\/span><\/a><span style=\"font-weight: 400\"> can be summarised in six words\u2014\u201cthink big, start small, test fast.\u201d We start with an ambitious population level aim that we are struggling to meet (e.g. \u201c<\/span><a href=\"https:\/\/www.newstatesman.com\/science-tech\/coronavirus\/2021\/01\/devi-sridhar-uk-needs-zero-covid-strategy-prevent-endless-lockdowns\"><span style=\"font-weight: 400\">zero covid<\/span><\/a><span style=\"font-weight: 400\">\u201d); identify areas where we can start work immediately (e.g. vaccine delivery in a local nursing home); and start testing ideas for improvement, measuring as we go (e.g. recording uptake, time taken to vaccinate, wasted doses of vaccine, illness and morale among staff and residents). To reach \u201czero covid\u201d we would need to work across a range of areas\u2014e.g. in achieving physical distancing in workplaces and public places, contact tracing, widespread uptake of vaccination. Approaches that work at a small scale are tested more widely, adapting over time, until they can be used with confidence at a population level. This approach could be applied, for example, to England\u2019s <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/jan\/11\/test-and-trace-needs-radical-overhaul-to-prevent-further-covid-surges-in-england-experts\"><span style=\"font-weight: 400\">test and trace<\/span><\/a><span style=\"font-weight: 400\"> system. In contrast to the government&#8217;s series of U turns during recent months, quality improvement work requires being <\/span><a href=\"http:\/\/www.ihi.org\/resources\/Pages\/IHIWhitePapers\/HighImpactLeadership.aspx\"><span style=\"font-weight: 400\">connected, curious, and committed<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There have been some grounds for optimism during the pandemic that illustrate public health in action. Marcus Rashford\u2019s <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/explainers-53053337\"><span style=\"font-weight: 400\">school meal campaigns<\/span><\/a><span style=\"font-weight: 400\"> have demonstrated that the UK government\u2019s policies on child poverty can be changed after years of trying. The measures taken to support homeless people during the <\/span><a href=\"https:\/\/www.gov.uk\/government\/news\/3-2-million-emergency-support-for-rough-sleepers-during-coronavirus-outbreak\"><span style=\"font-weight: 400\">early stages of the first UK lockdown<\/span><\/a><span style=\"font-weight: 400\"> were invaluable while they lasted. The move by <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/technology-55175222\"><span style=\"font-weight: 400\">social media platforms to remove vaccine misinformation<\/span><\/a><span style=\"font-weight: 400\"> is also an important but long awaited first step. However, such measures are not an adequate replacement for effective and coordinated government action, and have often been too little, too late.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We are all public health workers now, from the way we organise our work, leisure, and day to day tasks such as shopping. Politicians, however, need to take up their public health roles in full. Just as <\/span><a href=\"https:\/\/wakelet.com\/wake\/9f8d4b22-4226-4c4a-8fc7-a71928297cfe\"><span style=\"font-weight: 400\">politicians in the early 20<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> century<\/span><\/a><span style=\"font-weight: 400\"> understood the importance of housing, nutrition, and workplace on health\u2014and legislated effectively to improve people\u2019s life circumstances\u2014our governments must take steps to protect people in overcrowded accommodation (home and work), insecure employment (including during periods of enforced self-isolation to minimise risk of infection), and living in poverty. Our recovery from the pandemic relies on effective public health action, and that means <\/span><a href=\"https:\/\/www.bmj.com\/content\/369\/bmj.m1557\"><span style=\"font-weight: 400\">whole-system planning<\/span><\/a><span style=\"font-weight: 400\"> and legislation, adapting to new circumstances as they arise, and adjusting approaches to meet the needs of different communities.\u00a0<\/span><\/p>\n<p><em><span style=\"font-weight: 400\"><strong>Graham Mackenzie<\/strong> has been a doctor for 25 years, and is currently retraining as a GP.
