{"id":49644,"date":"2021-02-19T19:03:40","date_gmt":"2021-02-19T18:03:40","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49644"},"modified":"2021-02-20T16:41:39","modified_gmt":"2021-02-20T15:41:39","slug":"adopting-an-anti-racist-medical-curriculum","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/02\/19\/adopting-an-anti-racist-medical-curriculum\/","title":{"rendered":"Adopting an anti-racist medical curriculum"},"content":{"rendered":"<p><span style=\"font-weight: 400\">There are currently calls for changes to the way migration, colonialism, and Britain\u2019s role in the slave trade are taught in the school curriculum and included in teacher training. We call for a similar reassessment of medical training.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There is now overwhelming and undeniable evidence that racism is a determinant of ill health. Health inequalities that disproportionately affect Black, Asian and other ethnic minorities include increased maternal mortality, mental health treatment, diabetes treatment and poorer breast cancer outcomes, to name just a few. [1]\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Genetic determinism, socioeconomics and co-morbidities have regularly been given as the explanations for these disparities. There has been only infrequent attention paid to the way that <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/10540593\/\"><span style=\"font-weight: 400\">racial discrimination causes physiological stress<\/span><\/a><span style=\"font-weight: 400\">, affects access to healthcare and impacts on treatment. Conscious and unconscious racial biases in clinician-patient interactions damage patient management and outcomes. [2]<\/span><\/p>\n<p><span style=\"font-weight: 400\">For NHS staff, racial discrimination accounts for lack of career progression, an ethnicity-linked pay gap, and stress and psychological burdens that affect their performance. [3] These differences begin with medical training, where bias permeates the education of doctors and nurses at every step\u2014in <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31425184\/\"><span style=\"font-weight: 400\">admissions<\/span><\/a><span style=\"font-weight: 400\">, clinical signs and <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/2313403\/\"><span style=\"font-weight: 400\">case presentations<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/21385802\/\"><span style=\"font-weight: 400\">undergraduate examination performance and postgraduate training<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Teaching on racism is largely absent from nursing and medical curricula\u2014health disparities are documented but not contested, and multi-culturalism and diversity training are confused with anti-racist pedagogy.<\/span> <span style=\"font-weight: 400\">Truly anti-racist teaching confronts prejudice through the discussion of racism, stereotyping and discrimination in society. It teaches the economic, structural and historical roots of inequality. [4]<\/span><\/p>\n<p><span style=\"font-weight: 400\">The Association of American Medical Colleges (AAMC) has produced an <\/span><a href=\"https:\/\/www.mededportal.org\/anti-racism\"><span style=\"font-weight: 400\">Anti-racism in Medicine Collection<\/span><\/a><span style=\"font-weight: 400\"> to provide educators with practice-based, peer-reviewed resources to teach anti-racist knowledge and clinical skills.<\/span><span style=\"font-weight: 400\"> The US-based <\/span><span style=\"font-weight: 400\">Association for Prevention Teaching and Research (APTR)<\/span> <span style=\"font-weight: 400\">has designed an <\/span><a href=\"https:\/\/www.aptrweb.org\/page\/curricula\"><span style=\"font-weight: 400\">anti-racism toolkit<\/span><\/a><span style=\"font-weight: 400\"> to assist health professions faculty address and reduce systemic racism through their teaching.<\/span><span style=\"font-weight: 400\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Recently, many UK higher education institutions, including medical schools, have released statements claiming commitment to tackling structural racism. Student activism has prompted one medical school to outline measures in which they will \u2018<\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/health-53465113\"><span style=\"font-weight: 400\">de-colonise the curriculum\u2019<\/span><\/a><span style=\"font-weight: 400\"> and\u00a0 the UK Medical Schools Council, in collaboration with the Dental Schools Council, has announced <\/span><a href=\"https:\/\/www.medschools.ac.uk\/news\/an-update-on-medical-schools-work-to-tackle-racism-and-inequality\"><span style=\"font-weight: 400\">workstreams to improve outcomes for BAME students, including tackling the award gap and supporting medical schools to create an inclusive environment<\/span><\/a><span style=\"font-weight: 400\">. Adopting an explicitly anti-racist curriculum which includes training for educators that echoes that of the AAMC and APTR would mean these statements and desires would be backed up by concrete, measurable actions.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Anti-racist education addresses <\/span><span style=\"font-weight: 400\">three interconnected components\u2014making systemic oppression visible, recognising personal complicity in oppression through unearned privilege and developing strategies to transform structural inequalities. [6] <\/span><span style=\"font-weight: 400\">We suggest using this framework to create an anti-racist medical curriculum that includes the following:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">teaching the structural causes of racism and inequity, including recognising and addressing privilege, conscious and unconscious bias, and the concept of <\/span><a href=\"https:\/\/www.angelasaini.co.uk\/superior\"><span style=\"font-weight: 400\">race as a social construct without genetic basis<\/span><\/a><span style=\"font-weight: 400\">\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">including eugenics and wrongs against ethnic minorities and other marginalised people in science and medicine in the history of medicine [7]<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">diversifying clinical teaching to include <\/span><a href=\"https:\/\/www.blackandbrownskin.co.uk\/mindthegap\"><span style=\"font-weight: 400\">clinical signs on darker skin tones.<\/span><\/a><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">making \u201cunderstanding and challenging the health effects of structural racism\u201d an <\/span><a href=\"https:\/\/www.nejm.org\/doi\/full\/10.1056\/NEJMp2021072\"><span style=\"font-weight: 400\">essential professional medical competency<\/span><\/a><span style=\"font-weight: 400\">, recognised within the domains of <\/span><span style=\"font-weight: 400\">\u00a0<\/span><a href=\"https:\/\/www.gmc-uk.org\/ethical-guidance\/ethical-guidance-for-doctors\/good-medical-practice\"><span style=\"font-weight: 400\">good medical practice<\/span><\/a><span style=\"font-weight: 400\">. <\/span><span style=\"font-weight: 400\">\u00a0<\/span><span style=\"font-weight: 400\">\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">understanding that representation matters and investing in the development and progression of BAME staff as teaching faculty and as mentors [8]<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">Dismantling structural racism<\/span><span style=\"font-weight: 400\"> means that &#8220;healthcare professionals must recognise, name, understand and talk about racism competently.&#8221;<\/span><span style=\"font-weight: 400\">\u00a0[9] <\/span><span style=\"font-weight: 400\">Real change will only be achieved if the medical curriculum is reformed to equip trainees with the tools to tackle historic inequalities. Now is the time to rethink what we do, what we have learned, and what we will teach future generations.
