{"id":49506,"date":"2021-02-01T18:25:02","date_gmt":"2021-02-01T17:25:02","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49506"},"modified":"2021-02-08T18:21:46","modified_gmt":"2021-02-08T17:21:46","slug":"we-are-setting-ourselves-on-fire-to-keep-everyone-else-warm-what-does-the-recovery-look-like-for-nhs-staff","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2021\/02\/01\/we-are-setting-ourselves-on-fire-to-keep-everyone-else-warm-what-does-the-recovery-look-like-for-nhs-staff\/","title":{"rendered":"\u201cWe are setting ourselves on fire to keep everyone else warm\u201d\u2014what does the recovery look like for NHS staff?"},"content":{"rendered":"<p class=\"standfirst\">Recovery from the pandemic will be as challenging as the pandemic itself, argue Christina Pagel and Edward Palmer<\/p>\n<p><!--more--><span style=\"font-weight: 400\">The <\/span><a href=\"https:\/\/coronavirus.data.gov.uk\/details\/healthcare\"><span style=\"font-weight: 400\">number of people in UK hospitals with c<\/span><\/a><a href=\"https:\/\/coronavirus.data.gov.uk\/details\/healthcare\"><span style=\"font-weight: 400\">ovid-19<\/span><\/a><span style=\"font-weight: 400\"> is currently just under 37,000 having reached twice the April peak in mid-January. There are 50% more intensive care beds in use than during the first peak. <\/span><a href=\"https:\/\/www.ft.com\/content\/b466d0d3-45f4-47e4-b681-fead24ae1f08\"><span style=\"font-weight: 400\">More<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.facebook.com\/watch\/?v=195352459008422\"><span style=\"font-weight: 400\">more<\/span><\/a><span style=\"font-weight: 400\"> stories are appearing in the media about the incredible strain that staff are under. Those stories will likely disappear once occupancy has fallen significantly, but the medium and long term impact on staff will not. The NHS needs to plan now for how to support the wellbeing of its staff over the coming months and years. A \u201cback to normal\u201d approach risks the mental health of staff, staff retention, and the ability to deliver NHS services over the coming years.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Even before covid-19, NHS staff were at higher risk of mental health problems, self-harm, and death by suicide than the general population. For instance, female doctors have a risk <\/span><a href=\"https:\/\/t.co\/aXRRJVCJOC?amp=1\"><span style=\"font-weight: 400\">up to four times higher<\/span><\/a><span style=\"font-weight: 400\"> and nurses <\/span><a href=\"https:\/\/t.co\/FUTkKdNUEj?amp=1\"><span style=\"font-weight: 400\">up to five times<\/span><\/a><span style=\"font-weight: 400\"> higher. In the <\/span><a href=\"https:\/\/www.nhsstaffsurveyresults.com\/wp-content\/uploads\/2020\/01\/P3255_ST19_National-briefing_FINAL_V2.pdf\"><span style=\"font-weight: 400\">2019 NHS Staff Survey<\/span><\/a><span style=\"font-weight: 400\">, over 40% of staff reported feeling unwell as a result of work-related stress over the previous year. Then the pandemic hit. The first wave put the NHS under immense strain and when the British Medical Association did <\/span><a href=\"https:\/\/t.co\/5NQtp1RIp2?amp=1\"><span style=\"font-weight: 400\">a survey of i<\/span><\/a><a href=\"https:\/\/t.co\/5NQtp1RIp2?amp=1\"><span style=\"font-weight: 400\">ntensive c<\/span><\/a><a href=\"https:\/\/t.co\/5NQtp1RIp2?amp=1\"><span style=\"font-weight: 400\">are u<\/span><\/a><a href=\"https:\/\/t.co\/5NQtp1RIp2?amp=1\"><span style=\"font-weight: 400\">nit (ICU) staff last July<\/span><\/a><span style=\"font-weight: 400\">, they found that 45% showed signs of mental ill-health, and 40% signs of post traumatic stress disorder (PTSD), more than <\/span><a href=\"https:\/\/europepmc.org\/article\/MED\/30295216\"><span style=\"font-weight: 400\">double that reported by recent combat veterans<\/span><\/a><span style=\"font-weight: 400\">. A high proportion (1 in 5) of ICU nurses reported thoughts of self-harm or suicide.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Then, after a summer of working flat-out to tackle the backlog of care from the first covid wave, the second wave started in September. In the first wave hospitals in England spent 40 days with an occupancy of more than 10,000 covid-19 patients. The second wave has already seen well over 80 days with over 10,000 patients in hospitals (since 2<\/span><span style=\"font-weight: 400\">nd<\/span><span style=\"font-weight: 400\"> November 2020). Even with the most optimistic estimates of decline, as experienced during the first lockdown, it will take at least another 40 days to get down to 10,000 covid-19 patients again. And longer still to get back down to the few thousand we had at the beginning of September. If restrictions are eased too quickly as the most vulnerable groups are vaccinated, <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/uk-55291520\"><span style=\"font-weight: 400\">a third wave of admissions among younger patients is possible<\/span><\/a><span style=\"font-weight: 400\"> in the Spring. What is certain is that by the time this is over, <\/span><a href=\"https:\/\/www.independent.co.uk\/voices\/nhs-nurses-medical-staff-covid-mental-health-b1788864.html\"><span style=\"font-weight: 400\">staff will have been working beyond their limits for many months<\/span><\/a><span style=\"font-weight: 400\">. As one doctor memorably put it <\/span><a href=\"https:\/\/twitter.com\/HPIAndyCowper\/status\/1352909058961977344?s=20\"><span style=\"font-weight: 400\">\u201c[we] are setting ourselves on fire to keep everyone else warm.\u201d<\/span><\/a><\/p>\n<p><span style=\"font-weight: 400\">Much of the impact will be the mental trauma experienced by healthcare workers caring for patients in both far greater number and severity than normal. Many patients will have died, often away from their loved ones. Many healthcare staff will have been patients themselves, or have seen their colleagues become patients. The vast majority will know of someone who was critically ill or died. Thousands will have had covid-19 themselves, and <\/span><a href=\"https:\/\/www.bbc.co.uk\/news\/health-55635451\"><span style=\"font-weight: 400\">5-<\/span><\/a><a href=\"https:\/\/www.bbc.co.uk\/news\/health-55635451\"><span style=\"font-weight: 400\">10% will be left struggling with \u201clong covid<\/span><\/a><a href=\"https:\/\/www.bbc.co.uk\/news\/health-55635451\"><span style=\"font-weight: 400\">\u201d<\/span><\/a><span style=\"font-weight: 400\"> afterwards.<\/span><\/p>\n<p><span style=\"font-weight: 400\">These issues will be amplified for the thousands of healthcare workers deployed away from their specialty area and away from their teammates. This brings the added stress of feeling de-skilled and out of one\u2019s depth. The feeling of working far out of your comfort zone can be a dragging stress\u2014particularly if continued for weeks on end. This burden will be particularly acute for those who are not used to seeing death in their usual role. Added to this is the knowledge that many of your \u201cusual\u201d patients are not getting the benefit of your expertise for weeks and months. Will delays to their treatment have repercussions for their outcomes? How will you see them all?<\/span><\/p>\n<p><span style=\"font-weight: 400\">Then there is the additional burden of losing months of training for junior staff, who will also often be the ones who have been providing the patient facing care. Exams have been postponed, and placements\u2014where they were meant to learn the intricacies of their craft\u2014spent deployed on covid wards. Many will put enormous pressure on themselves to try to catch up as soon as possible, leading to further strain.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Staff are physically and mentally exhausted. As frontline staff manage the current peak they will be in coping mode. It is only when the immediate pressure abates that staff will have time to process what has happened and will need to access support. But by summer, the pressure will be on the NHS, not just to deliver \u201cnormal\u201d levels of care to all who need it, but to offer extra clinics, extra surgeries, extra everything to start tackling the backlog of cases <\/span><a href=\"https:\/\/bjssjournals.onlinelibrary.wiley.com\/doi\/10.1002\/bjs.11817\"><span style=\"font-weight: 400\">that will take years to work through<\/span><\/a><span style=\"font-weight: 400\">. <\/span><a href=\"https:\/\/www.nursingtimes.net\/news\/workforce\/level-of-staff-burnout-real-concern-for-nhs-trust-leaders-06-10-2020\/\"><span style=\"font-weight: 400\">Trying to do this without letting traumatised staff recover or putting in place active measures to support their wellbeing would be an enormous mistake<\/span><\/a><span style=\"font-weight: 400\">. <\/span><span style=\"font-weight: 400\">It is imperative that frontline staff have access to evidence-based psychological therapies in the two\u00a0 \u00a0 \u00a0 year recovery period as the pandemic subsides to prevent poor mental health outcomes.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The NHS has already increased help for staff throughout the pandemic with access to wellbeing support through counselling, often online or via an app, <\/span><a href=\"https:\/\/www.theguardian.com\/world\/2021\/jan\/23\/meet-the-people-treating-nhs-workers-for-trauma\"><span style=\"font-weight: 400\">trauma counsellors<\/span><\/a><span style=\"font-weight: 400\">, or <\/span><a href=\"https:\/\/www.cntw.nhs.uk\/news\/10-things-weve-learned-about-looking-after-your-mental-health-during-lockdown\/\"><span style=\"font-weight: 400\">advice on wellbeing<\/span><\/a><span style=\"font-weight: 400\">. However, access can vary greatly across different NHS Trusts. All of this must continue and be expanded as pressure eases. Leadership at all levels must encourage staff to care for themselves and each other, openly discussing impacts on them to reduce stigma and encourage conversation and mutual support. Careful plans must be made to allow all staff to access evidence-based psychological support as part of their work hours. They must be enabled and encouraged to take the annual leave they have sacrificed, in long enough blocks to allow for recuperation. This might mean less activity in the short term, but it will mean more activity in the longer term as staff recover and recharge.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Perhaps most importantly, gaps in NHS staffing, particularly for <\/span><a href=\"https:\/\/t.co\/3Lf8GSa7p6?amp=1\"><span style=\"font-weight: 400\">nurses<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.rcplondon.ac.uk\/news\/more-1-4-doctors-have-sought-mental-health-support-during-pandemic\"><span style=\"font-weight: 400\">doctors<\/span><\/a><span style=\"font-weight: 400\">, must be urgently addressed through maximising staff retention, with better rates of pay, encouraging immigration and, for the longer term, greatly increased training places with proper funding, particularly for nursing qualifications.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Recovery from the pandemic will be as challenging as the pandemic itself. Maintaining the health and wellbeing of our staff\u2014 and resuming the delivery of high quality care for all\u2014will require courageous and novel thinking. Staff wellbeing and patient care are intrinsically interdependent.
