These charities have a deep understanding of what is going on in the lives of the people, families, and communities that they support due to\u00a0 their close relationships with these communities, from answering helplines, and from going about their work. These charities\u00a0 know that people are confused, anxious, and looking for more information\u00a0or in need of\u00a0more support than they are getting right now. National Voices summarised these\u00a0 concerns in a <\/span><a href=\"https:\/\/www.nationalvoices.org.uk\/publications\/our-publications\/communication-key\"><span style=\"font-weight: 400\">public statement last week<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Things are very, very hard right now for the many thousands of people who live with ill health or disability. We <\/span><a href=\"https:\/\/www.hsj.co.uk\/quality-and-performance\/the-pandemic-has-broken-the-promise-of-universal-healthcare\/7029293.article\"><span style=\"font-weight: 400\">have gone as far as saying that the\u00a0NHS cannot be described as still providing universal healthcare to everyone who needs<\/span><\/a><span style=\"font-weight: 400\"> it\u00a0during this phase of the pandemic. Instead, it is morphing into an \u201curgent and emergency\u201d care service only.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Focusing on acute episodes only is the worst possible way of achieving good outcomes and experiences for people with long-term conditions. And tightening the access criteria to health or social care\u2014where you\u00a0can\u2019t\u00a0get help unless your asthma is\u00a0totally out of control, until your cancer is diagnosed in A&amp;E, or your depression makes you acutely suicidal\u2014is the worst possible way to organise care for millions of people.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Proactive care\u00a0and support\u00a0that enables people to live well, to recover, or to manage their symptoms, is how we add years to life and life to years.\u00a0It is also how we at least partially address the inequality and unfairness that\u00a0blights our health outcomes.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The National Voices coalition is worried that when the \u201cfog of war\u201d of the pandemic finally lifts, it will be challenging to rebuild services that respond flexibly to people\u2019s needs, or that helps them cope with whatever limitations ill health or disability impose on their lives. We are concerned that health and care will have run \u201curgent and emergency\u201d only services for most parts of the year. There is a reluctance\u00a0among\u00a0NHS\u00a0leaders\u00a0and politicians\u00a0to admit that this approach comes at a high price for people living with ill health. Corresponding\u00a0talk of \u201cspare capacity,\u201d looks like we can get away with just focusing on the most urgent, on those who will die if we don\u2019t look after them right now.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">However the need for care to resume in much more holistic ways will have grown exponentially. There will be:\u00a0<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Built up need from people who couldn\u2019t be seen during the\u00a0pandemic\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Need from people who didn\u2019t even ask for care, or a diagnosis, or a\u00a0referral\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Need from people whose mental or physical health has deteriorated because of the circumstances they found themselves in (we have found\u00a0<\/span><a href=\"https:\/\/www.nationalvoices.org.uk\/sites\/default\/files\/public\/news\/download\/what_we_know_now_1.pdf\"><span style=\"font-weight: 400\">strong evidence<\/span><\/a><span style=\"font-weight: 400\">\u00a0of this during the first wave)\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">New needs from people who suffered from\u00a0covid-19, and\u00a0have not fully\u00a0recovered.\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">If past experience is anything to go by, the health and care system will respond to the risk of being overwhelmed by further rationing with the tightening of access criteria: you are not anorexic enough, you can still just about walk despite your pain, you are only self-harming and not actively trying to end your life.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In order to\u00a0prevent this from happening, we all need to work together to build and present a consensus\u00a0on <\/span><a href=\"https:\/\/www.nationalvoices.org.uk\/sites\/default\/files\/public\/publications\/what_we_need_now_-_full_report.pdf\"><span style=\"font-weight: 400\">what is needed\u00a0now<\/span><\/a><span style=\"font-weight: 400\">. This needs to start with a truthful assessment of where we are at present. No selective reporting of waiting times statistics, no distracting drive to reach some national targets on tests or vaccines that\u00a0don\u2019t\u00a0focus on where the actual (and unequal) need is.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We, as organisations that are embedded in communities of people with\u00a0substantial needs, can help bring to life what it looks like when those needs\u00a0aren\u2019t\u00a0met: What is it like to be over 85 and the carer for your spouse when home visits stop? What is it like to look after\u00a0someone dying at home when community nursing is stripped right back? What is like to live with severe mental ill health and without\u00a0face-to-face\u00a0contact with professionals?<\/span><\/p>\n<p><span style=\"font-weight: 400\">We will\u00a0speak truth to power about what is\u00a0actually going\u00a0on for people.\u00a0Others need to bring their considerable data analytics abilities to the challenge of quantifying those needs and to pricing what resources we need to begin to put things right for people: clearly more staff, so we never have to have one nurse look after five critically ill patients again, but also investment in\u00a0better healthcare buildings,\u00a0in more proactive and joined-up primary and community care and much more.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">And all of us need to work together to argue for\u00a0and design\u00a0new models of care: closer to home, less focused on\u00a0body parts and more on people, able to listen and to respond\u00a0to disadvantage\u00a0and the unequal burdens of ill health.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Access to timely and efficient care will remain restricted for quite some time. We need to invest in the <\/span><a href=\"https:\/\/www.nationalvoices.org.uk\/sites\/default\/files\/public\/publications\/patient._noun._adjective._october_2020.pdf\"><span style=\"font-weight: 400\">things that\u00a0help people cope\u00a0while they wait\u00a0<\/span><\/a><span style=\"font-weight: 400\">and\u00a0that make for a good life: connection, good information and advice, emotional and practical support,\u00a0and purposeful and reciprocal relationships with the people who make our communities.
