{"id":49206,"date":"2020-12-11T12:07:25","date_gmt":"2020-12-11T11:07:25","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49206"},"modified":"2020-12-18T12:24:58","modified_gmt":"2020-12-18T11:24:58","slug":"covid-19-has-turned-the-spotlight-on-the-uneven-provision-of-oxygen-a-stark-health-inequity","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/12\/11\/covid-19-has-turned-the-spotlight-on-the-uneven-provision-of-oxygen-a-stark-health-inequity\/","title":{"rendered":"Covid-19 has turned the spotlight on the uneven provision of oxygen\u2014a stark health inequity"},"content":{"rendered":"<p class=\"standfirst\"><span style=\"font-weight: 400\">This pandemic is also an opportunity to close an oxygen divide that ranks among the greatest health injustices of our time, say Kevin Watkins and Adamu Isah<\/span><\/p>\n<p><!--more--><span style=\"font-weight: 400\">Last April Boris Johnson, the UK prime minister, emerged from his brush with covid-19 to deliver a tribute to his medical team and a celebration of the treatment that had kept him alive. \u201cI was going through litres and litres of oxygen,\u201d he said, adding: \u201cI\u2019m a very lucky man.\u201d <\/span><span style=\"font-weight: 400\">You can say that again.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Covid-19 has provided a brutal reminder of the importance of medical oxygen. In severe cases, the disease attacks the lungs in the form of pneumonia, causing blood oxygen to fall to levels that compromise critical organs\u2014a condition known as hypoxemia. <\/span><a href=\"https:\/\/www.who.int\/docs\/default-source\/coronaviruse\/clinical-management-of-novel-cov.pdf\"><span style=\"font-weight: 400\">Around one in five patients with covid-19 need oxygen<\/span><\/a><span style=\"font-weight: 400\">\u2014and lots of it.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Boris Johnson had the good fortune to be treated in a health system that provides oxygen therapy in abundance, free at the point of use. That is what universal health coverage looks like in practice.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Yet this stands in stark contrast to the crisis unfolding across much of the developing world. As the pandemic has spread, creaking medical oxygen systems have been overwhelmed. <\/span><a href=\"https:\/\/www.aljazeera.com\/news\/2020\/9\/16\/indian-hospitals-face-oxygen-shortage-as-covid-19-cases-surge\"><span style=\"font-weight: 400\">In India<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.thefinancialexpress.com.bd\/health\/health-management-disparity-2-oxygen-icu-shortage-in-districts-to-blame-1600314476\"><span style=\"font-weight: 400\">Bangladesh<\/span><\/a><span style=\"font-weight: 400\"> public health facilities have struggled to provide oxygen for patients with covid-19. In Peru and Ecuador surging prices for oxygen have created flourishing <\/span><a href=\"https:\/\/www.theguardian.com\/global-development\/2020\/may\/07\/peru-jungle-iquitos-coronavirus-covid-19\"><span style=\"font-weight: 400\">black markets<\/span><\/a><span style=\"font-weight: 400\"> in oxygen cylinders, with public health providers squeezed out of markets. Lives have been <\/span><a href=\"https:\/\/apnews.com\/article\/df97326ec00fb7cc4abf5b3821ace984\"><span style=\"font-weight: 400\">lost<\/span><\/a><span style=\"font-weight: 400\"> due to shortages.<\/span><\/p>\n<p><span style=\"font-weight: 400\">As in so many other areas, the uneven provision of oxygen has turned the spotlight on deep national and global inequities in health. Medical oxygen is universally available in rich countries. Yet in many developing countries, it is effectively a luxury item, which is available only to the wealthy through private hospitals.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">This oxygen divide has been neglected for far too long. That\u2019s partly because most of those on the wrong side of the divide are poor\u2014especially poor children. Pneumonia powerfully illustrates this. Covid-19, which poses the most immediate threat to older adults, has catapulted medical oxygen onto the global agenda. Meanwhile, the devastation caused by childhood pneumonia continues to escape the attention of policy makers.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Each year, around <\/span><a href=\"https:\/\/www.savethechildren.org.uk\/news\/media-centre\/press-releases\/pneumonia-4-million-children-need-oxygen\"><span style=\"font-weight: 400\">4.2 million children in low and middle income countries<\/span><\/a><span style=\"font-weight: 400\"> are admitted to health facilities with pneumonia-related hypoxemia. Yet across parts of Africa, where child pneumonia is endemic, <\/span><a href=\"https:\/\/blogs.rch.org.au\/grandrounds\/2020\/11\/04\/world-pneumonia-day-vaccines-oxygen-therapy-and-the-impact-of-covid-19\/\"><span style=\"font-weight: 400\">fewer than one in five receive the oxygen therapy they need<\/span><\/a><span style=\"font-weight: 400\">. For many children, that is a death sentence. Childhood pneumonia is now the <\/span><a href=\"https:\/\/data.unicef.org\/resources\/levels-and-trends-in-child-mortality\/\"><span style=\"font-weight: 400\">biggest killer of children<\/span><\/a><span style=\"font-weight: 400\">, claiming more than 800 000 lives each year.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Many of these lives could be saved.\u00a0 A combination of pulse oximeters (simple, low cost devices that measure blood oxygen levels), training nurses, and basic oxygen therapy can cut death rates among hypoxemic children hospitalised with pneumonia by half. Yet in <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1002\/ppul.24691\"><span style=\"font-weight: 400\">Nigeria<\/span><\/a><span style=\"font-weight: 400\">, which tops the world league table for childhood pneumonia deaths, effective oxygen treatment is seldom available in the rural clinics and hospitals that serve poor families. That helps to explain why death rates from pneumonia <\/span><a href=\"https:\/\/www.savethechildren.org.uk\/what-we-do\/policy-and-practice\/our-featured-reports\/fighting-for-breath\"><span style=\"font-weight: 400\">are three times higher<\/span><\/a><span style=\"font-weight: 400\"> for children in the poorest 20% of households than they are for children in the richest households.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Universal access to medical oxygen would do more than save lives from pneumonia. Oxygen is vital for the treatment of malaria, sepsis, and neonatal respiratory problems (all major causes of death), as well as in childbirth.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">So, why is medical oxygen routinely available in rich countries and routinely inaccessible to poor people in developing countries? Infrastructure is key. The oxygen provided to patients in hospitals in Europe and the US is typically manufactured in large scale industrial plants, delivered in low cost bulk liquid form, stored, converted into gas, and channelled to bedsides through internal pipes. Economies of scale secure large volumes at low prices.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The <\/span><a href=\"https:\/\/www.theguardian.com\/global-development\/2020\/aug\/10\/fighting-for-breath-how-the-medical-oxygen-industry-is-failing-african-hospitals\"><span style=\"font-weight: 400\">infrastructure in poor countries<\/span><\/a><span style=\"font-weight: 400\"> looks very different. Hospitals typically buy oxygen in low volume, compressed gas cylinders, which are then transported, often over vast distances. Small rural hospitals in Kenya or Nigeria <\/span><a href=\"https:\/\/www.thebureauinvestigates.com\/stories\/2020-08-09\/lack-of-oxygen-leaves-covid-19-patients-in-africa-gasping-for-air\"><span style=\"font-weight: 400\">can pay 5-10 times<\/span><\/a><span style=\"font-weight: 400\"> as much per litre of oxygen as the London hospital that treated Boris Johnson. When demand surges, prices rise and supplies get diverted from rural clinics dealing with childhood pneumonia to urban hospitals responding to covid-19.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Lacking the public finance to provide oxygen for free, health facilities invariably pass the cost of on to patients. Some of the world\u2019s poorest people are left paying among the highest prices. Treating a single case of childhood pneumonia with oxygen in Nigeria can cost $50-60\u2014and many people are just too poor to pay.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The oxygen crisis is a symptom of market failure, inequity, and a paralysing neglect of oxygen as an essential medicine. The director general of the World Health Organization (WHO), Tedros Adhanom Ghebreyesus, has rightly called for new business models to close the yawning gap between oxygen supply and demand.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Building these models is not rocket science. In Ethiopia, for example, the government has developed an ambitious roadmap for achieving universal oxygen access, bringing together investors, health authorities, and philanthropists. You can see the results in the <\/span><a href=\"https:\/\/newbusinessethiopia.com\/health\/ethiopias-amhara-region-hospitals-get-oxygen-centre\/\"><span style=\"font-weight: 400\">Amhara Regional Oxygen Centre<\/span><\/a><span style=\"font-weight: 400\">,<\/span><span style=\"font-weight: 400\"> an oxygen plant now supplying 40 hospitals. In Kenya, a social enterprise called <\/span><a href=\"https:\/\/hewatele.org\/index.php\/our-plants\"><span style=\"font-weight: 400\">Hewatele<\/span><\/a><span style=\"font-weight: 400\"> (Swahili for \u201cabundant air\u201d) has created three plants near clusters of hospitals, which have pooled demand and cut transport costs. Oxygen prices have fallen sharply.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">There are many examples of innovation with the potential to deliver oxygen to communities beyond the reach of hospitals. In Papua New Guinea an <\/span><a href=\"https:\/\/findanexpert.unimelb.edu.au\/scholarlywork\/1469176-solar-powered-oxygen--quality-improvement-and-child-pneumonia-deaths--a-large-scale-effectiveness-study.\"><span style=\"font-weight: 400\">oxygen improvement programme<\/span><\/a><span style=\"font-weight: 400\"> providing training, solar powered oxygen concentrators, and pulse oximeters to 38 facilities in remote areas saw child death rates fall by 40%.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">International cooperation should be supporting efforts like these that tackle the systemic oxygen crisis. Unfortunately, the response to covid-19 has been a case study in misplaced priorities. Aid donors have focused on providing ventilators<\/span> <span style=\"font-weight: 400\">and oxygen concentrators for covid-19 wards, neglecting investment in the large scale oxygen systems that would not only reduce deaths from covid-19 but save lives from many other causes, including pneumonia<\/span><span style=\"font-weight: 400\">.<\/span><span style=\"font-weight: 400\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The <\/span><a href=\"https:\/\/www.who.int\/initiatives\/act-accelerator\"><span style=\"font-weight: 400\">Access to Covid-19 Tools Accelerator,<\/span><\/a><span style=\"font-weight: 400\"> the multilateral response vehicle targeting $38bn in covid-19 support, has not mobilised any resources for investment in oxygen infrastructure. That\u2019s an outright dereliction of leadership. The <\/span><a href=\"https:\/\/www.theglobalfund.org\/en\/news\/2020-09-28-global-partnership-to-make-available-120-million-affordable-quality-covid-19-rapid-tests\/\"><span style=\"font-weight: 400\">Global Fund<\/span><\/a><span style=\"font-weight: 400\"> and the World Bank, who are responsible for oxygen in the Access to Covid-19 Tools Accelerator, should either be leading efforts to finance that infrastructure and operating with a sense of urgency on what is\u2014quite literally\u2014a life and death issue, or stepping aside.<\/span><\/p>\n<p><span style=\"font-weight: 400\">We can and must do better. Covid-19 is a global crisis that has ruthlessly exposed and exacerbated health inequalities across the world.
