{"id":49188,"date":"2020-12-09T10:39:39","date_gmt":"2020-12-09T09:39:39","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49188"},"modified":"2020-12-09T10:40:03","modified_gmt":"2020-12-09T09:40:03","slug":"raps-battle-with-mental-health-how-hip-hops-progressive-narratives-are-helping-to-tackle-stigma","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/12\/09\/raps-battle-with-mental-health-how-hip-hops-progressive-narratives-are-helping-to-tackle-stigma\/","title":{"rendered":"How hip-hop\u2019s progressive narratives are helping to tackle mental health stigma"},"content":{"rendered":"<p><span style=\"font-weight: 400\">Hip-hop is one of the world\u2019s most popular music genres, with <\/span><span style=\"font-weight: 400\">its global prominence transcending <\/span><a href=\"https:\/\/www.billboard.com\/articles\/news\/international\/8533280\/rap-goes-global-streaming-hip-hop-artists-language\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">language and geography<\/span><\/a><span style=\"font-weight: 400\">.<sup>1<\/sup> More than ever before, hip-hop artists are publicly acknowledging their mental health struggles, promoting anti-stigma campaigns around mental health, and<\/span><span style=\"font-weight: 400\"> normalising seeking treatment for mental health issues<\/span><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In our opinion,<\/span><span style=\"font-weight: 400\"> hip-hop can be a vehicle for tackling stigma around mental health and addressing <\/span><span style=\"font-weight: 400\">cultural imbalances<\/span><span style=\"font-weight: 400\">. <\/span><span style=\"font-weight: 400\">Hip-hop connects with groups that healthcare has historically struggled to reach, particularly men within the Black community<\/span><span style=\"font-weight: 400\">. Underrepresented communities are\u00a0at higher risk of developing mental health problems, and they are more likely to experience worse mental health outcomes. This is, in part, due to socioeconomic disparities. They are also less likely to use mental health services. Stigma around mental health issues is common in underserved communities, and it is a substantial <\/span><span style=\"font-weight: 400\">barrier to<\/span><span style=\"font-weight: 400\"> accessing health services. <\/span><span style=\"font-weight: 400\">Discrimination, bias, and a lack of cultural competence from healthcare professionals can also lead to unmet needs, late presentation of symptoms, and poorer quality of care.<sup>2-5<\/sup><\/span><\/p>\n<p><span style=\"font-weight: 400\">We highlight <\/span><span style=\"font-weight: 400\">various <\/span><span style=\"font-weight: 400\">progressive hip-hop songs and artists, which have progressed the narrative of normalising conversations about mental health.<\/span><span style=\"font-weight: 400\"> The purpose of sharing this information is to <\/span><span style=\"font-weight: 400\">develop innovative ways of engaging in culturally sensitive contexts to normalise seeking treatment for mental health issues<\/span><span style=\"font-weight: 400\">. We also hope this information helps to foster new ways for health professionals to open up conversations about mental health with their patients, given that some patients may find it difficult to talk about this in clinical settings. T<\/span><span style=\"font-weight: 400\">his may be particularly relevant when service users come from some marginalised communities\u2014for example, ethnic minorities or younger people who are interested in hip-hop but may not necessarily trust health services.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the 1970s, pioneering rappers like <\/span><a href=\"https:\/\/youtu.be\/PobrSpMwKk4\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Grandmaster Flash and the Furious Five<\/span><\/a><span style=\"font-weight: 400\"> documented the <\/span><a href=\"https:\/\/www.bbc.co.uk\/programmes\/b04nvtl4\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">harsh living conditions and social inequalities<\/span><\/a><span style=\"font-weight: 400\"> that affected their mental states.<sup>6<\/sup><\/span>\u00a0<span style=\"font-weight: 400\">I<\/span><span style=\"font-weight: 400\">n the 1980s and 1990s, mental health issues started to emerge more explicitly in lyrics, but were camouflaged in masculinity. Mainstream \u201c<\/span><span style=\"font-weight: 400\">gangsta rap\u201d <\/span><span style=\"font-weight: 400\">portrayed <\/span><span style=\"font-weight: 400\">machismo and discouraged weakness. However, some hints of <\/span><span style=\"font-weight: 400\">vulnerability<\/span><span style=\"font-weight: 400\"> were exemplified by <\/span><span style=\"font-weight: 400\">songs like \u201c<\/span><a href=\"https:\/\/youtu.be\/KutXyPEEbQs\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Mind Playing Tricks On Me<\/span><\/a><span style=\"font-weight: 400\">\u201d<\/span> <span style=\"font-weight: 400\">by the Ghetto Boys, which depicts <\/span><a href=\"https:\/\/youtu.be\/HqLfzIUam10\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">hallucinations<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/www.npr.org\/2019\/05\/29\/726615663\/geto-boys-mind-playing-tricks-on-me-anxiety-american-anthem\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">delusions<\/span><\/a><span style=\"font-weight: 400\"> along with Tupac Shakur\u2019s song \u201c<\/span><a href=\"https:\/\/youtu.be\/PIvjfEy4Dk4\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">So Many Tears<\/span><\/a><span style=\"font-weight: 400\">&#8221; which illustrates signs of <\/span><a href=\"https:\/\/youtu.be\/VO6kcIqyuCI\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">trauma and depression<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The relation between mental health and masculinity is complex, as societies sometimes promote narratives such as \u201cstrong men don\u2019t cry\u201d or \u201chis emotions got the better of him.\u201d These negative views, which portray men who express their emotions as \u201cweak,\u201d add to the stigma around mental health issues. Men under the age of 50 are at an increased risk of taking their own lives and are less likely to seek help when experiencing a mental health crisis, which suggests that this stigma needs to be overcome.<sup>7-8<\/sup> A jarring inner conflict between anger and sadness is portrayed in Lil Wayne\u2019s song \u201c<\/span><a href=\"https:\/\/youtu.be\/ErCAOMi5EGM\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Drop The World<\/span><\/a><span style=\"font-weight: 400\">,\u201d which dramatically shifts in language use and tone. Lil Wayne once talked about accidently shooting himself in the chest when he was young, but he later revealed in his <\/span><a href=\"https:\/\/genius.com\/Solange-mad-lyrics\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">lyrics<\/span><\/a><span style=\"font-weight: 400\"> and during interviews that it was an attempt to end his life: <\/span><span style=\"font-weight: 400\">\u201c<\/span><a href=\"https:\/\/youtu.be\/Bv9KSa-vkAY\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">When I attempted suicide, I didn\u2019t die, I remember how mad I was on that day . . . you gotta let it go before it get up in the way<\/span><\/a><span style=\"font-weight: 400\">\u201d<\/span><span style=\"font-weight: 400\">.<\/span><span style=\"font-weight: 400\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the 2000s and particularly the 2010s, rappers increasingly showed a more sensitive side of hip-hop, acknowledging struggles with depression and anxiety. Artists such as Meek Mill have also discussed mental health issues around coping with trauma in urban environments, <\/span><a href=\"https:\/\/toofab.com\/2019\/03\/01\/meek-mill-likens-ptsd-from-military-combat-to-living-in-philadelphia\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">comparing this to experiences of soldiers with post traumatic stress disorder from war zones<\/span><\/a><span style=\"font-weight: 400\">, which has also been referenced in the scientific literature.<sup>9<\/sup> Mental health risk factors, such as childhood sexual abuse,<sup>10<\/sup> have been articulated in lyrics, for example, by <\/span><span style=\"font-weight: 400\">Angel <\/span><span style=\"font-weight: 400\">Haze. She remixed <\/span><span style=\"font-weight: 400\">Eminem\u2019s song \u201c<\/span><a href=\"https:\/\/www.hiphoppsych.co.uk\/pdf\/hhp_tupac_eminem_freestyle_mixtape_essay.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Cleaning Out My Closet<\/span><\/a><span style=\"font-weight: 400\">\u201d and spoke <\/span><span style=\"font-weight: 400\">very candidly about her own abuse as a child, saying that this <\/span><a href=\"https:\/\/youtu.be\/olBOFLqEREI\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">particular song<\/span><\/a><span style=\"font-weight: 400\"> was \u201c<\/span><a href=\"https:\/\/www.spin.com\/2012\/10\/angel-haze-sexual-assault-eminem-cleaning-out-my-closet\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">probably the realest song I ever recorded<\/span><\/a><span style=\"font-weight: 400\">&#8220;.<\/span><\/p>\n<p><span style=\"font-weight: 400\">P Diddy helped to normalise the <\/span><a href=\"https:\/\/hiphopwired.com\/805232\/diddy-confesses-to-crying-for-more-than-three-hours-straight-says-it-felt-great\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">acceptability of men publicly crying<\/span><\/a><span style=\"font-weight: 400\"> and grieving\u00a0over\u00a0the loss of a partner when his social media post <\/span><span style=\"font-weight: 400\">received an outpouring of supportive comments, such as: <\/span><span style=\"font-weight: 400\">\u201c<\/span><a href=\"https:\/\/www.instagram.com\/p\/BqVMxmFHzYR\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Real men cry too&#8211;it\u2019s healing, and your healing heals the rest of the world<\/span><\/a><span style=\"font-weight: 400\">\u201d. In Rico Nasty\u2019s last album, Anger Management, she channels expressions of rage and anger into a form of empowerment, saying that her \u201c<\/span><a href=\"https:\/\/pitchfork.com\/thepitch\/rico-nasty-and-the-importance-of-black-womens-anger-in-rap\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">expression of anger is a form of rejuvenation<\/span><\/a><span style=\"font-weight: 400\">\u201d. These examples show that mainstream hip-hop artists are making topics related to mental health and vulnerability more central to their lyrics.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In recent years, mental health <\/span><span style=\"font-weight: 400\">anti-stigma campaigns have progressed even further in hip-hop culture, with rappers actively encouraging <\/span><span style=\"font-weight: 400\">people to seek out resources for mental health issues<\/span><span style=\"font-weight: 400\"> and to be open to <\/span><span style=\"font-weight: 400\">the idea of seeking treatment. For example, the rapper Logic partnered with <\/span><span style=\"font-weight: 400\">the National Suicide Prevention Lifeline [in the US], releasing a song entitled \u201c<\/span><a href=\"https:\/\/youtu.be\/Kb24RrHIbFk\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">1-800-273-8255<\/span><\/a><span style=\"font-weight: 400\">\u201d about a suicidal hotline caller getting support. After he performed this song at the <\/span><a href=\"https:\/\/youtu.be\/_Ju6Q8Azcmg\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Grammy Awards<\/span><\/a><span style=\"font-weight: 400\">, <\/span><span style=\"font-weight: 400\">where people who had attempted suicide appeared alongside him on stage<\/span><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/edition.cnn.com\/2018\/02\/03\/us\/logic-grammys-performance-suicide-prevention-calls-trnd\/index.html\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">calls to the helpline tripled<\/span><\/a><span style=\"font-weight: 400\">.<\/span><span style=\"font-weight: 400\">\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Hip-hop culture is shifting away from denying professional help or managing one\u2019s feelings in isolation (\u201c<\/span><a href=\"https:\/\/pitchfork.com\/thepitch\/therapy-is-gangsta-hip-hops-views-on-mental-health-are-evolving\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">I\u2019m usually to myself . . . . I\u2019m my own psychiatrist<\/span><\/a><span style=\"font-weight: 400\">\u201d <\/span><span style=\"font-weight: 400\">said Tech N9ne) towards views such as \u201c<\/span><a href=\"https:\/\/www.vibe.com\/2016\/07\/darryl-mcdaniels-mental-health-memoir\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">When I went to therapy I realized . . . therapy isn\u2019t soft . . . <\/span><span style=\"font-weight: 400\">Therapy is gangsta<\/span><span style=\"font-weight: 400\">. It actually empowered me<\/span><\/a><span style=\"font-weight: 400\">,\u201d<\/span><span style=\"font-weight: 400\"> said <\/span><span style=\"font-weight: 400\">Darryl McDaniels <\/span><span style=\"font-weight: 400\">from <\/span><span style=\"font-weight: 400\">Run-DMC. Another advocate of therapy is rapper <\/span><a href=\"https:\/\/www.facebook.com\/kidcudi\/posts\/its-been-difficult-for-me-to-find-the-words-to-what-im-about-to-share-with-you-b\/10154706102758586\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Kid Cudi<\/span><\/a><span style=\"font-weight: 400\">, who posted on social media about seeking rehabilitation treatment and later stated: \u201c<\/span><a href=\"https:\/\/pitchfork.com\/thepitch\/therapy-is-gangsta-hip-hops-views-on-mental-health-are-evolving\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">A year ago I wouldn\u2019t even go to a therapist or psychiatrist. But I gave it a shot . . . It\u2019s good for me to talk to someone who helps me see things<\/span><\/a><span style=\"font-weight: 400\">\u201d. Kudi\u2019s announcement on social media about seeking treatment received positive comments from his fans on social media, such as: \u201c<\/span><a href=\"https:\/\/www.facebook.com\/kidcudi\/posts\/its-been-difficult-for-me-to-find-the-words-to-what-im-about-to-share-with-you-b\/10154706102758586\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">You helped me a lot too, and I hope you get the help you gave me and a lot of other lost kids<\/span><\/a><span style=\"font-weight: 400\">\u201d. <\/span><a href=\"https:\/\/www.refinery29.com\/en-gb\/2018\/01\/189515\/jay-z-interview-mental-health-masculinity-therapy\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Jay Z<\/span><\/a><span style=\"font-weight: 400\">\u2019s<\/span> <span style=\"font-weight: 400\">album \u201c<\/span><a href=\"https:\/\/en.wikipedia.org\/wiki\/4:44\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">4:44<\/span><\/a><span style=\"font-weight: 400\">\u201d documents <\/span><span style=\"font-weight: 400\">his own experiences in therapy, <\/span><span style=\"font-weight: 400\">pointing out the benefits of \u201c<\/span><a href=\"https:\/\/www.nbcnews.com\/pop-culture\/music\/hip-hop-works-break-down-mental-health-stigma-black-men-n819461\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">having someone respond to those concessions and challenging your view of yourself to help you reassess<\/span><\/a><span style=\"font-weight: 400\">\u201d.<\/span><\/p>\n<p><span style=\"font-weight: 400\">There is still more that hip-hop can do to tackle stigma, and there are some remaining negative aspects too. There have been insensitive statements from some artists and managers within the hip-hop community\u2014for example, around <\/span><a href=\"https:\/\/pagesix.com\/2018\/12\/17\/chance-the-rappers-manager-slammed-for-criticizing-mental-health-medication\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">stigmatising patients using medication<\/span><\/a><span style=\"font-weight: 400\"> as part of treatment for mental health problems and insensitive remarks about people who have <\/span><a href=\"https:\/\/www.sohh.com\/royce-da-59-says-troy-ave-went-far-joey-bada-beef-owe-capital-steezs-family-apology-video\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">died by suicide<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Self medication is a serious risk in hip-hop culture,<sup>6<\/sup> with some rappers <\/span><span style=\"font-weight: 400\">contributing to the normalisation of poor self care by ignoring destructive symptoms.<\/span><span style=\"font-weight: 400\"> For example, references to self medication and suicidal ideation are arguably being normalised in some lyrics within the hip-hop subgenre known as \u201cEmo-rap\u201d (also known as\u00a0 \u201cSoundCloud rap\u201d or \u201csad rap\u201d). Artists within this genre have openly articulated their mental health struggles and attempts to cope, but this also raises concerns that it could <\/span><a href=\"https:\/\/www.theguardian.com\/society\/2019\/mar\/17\/rap-music-mental-health-issues\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">increase the risk of a contagion effect<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><a href=\"https:\/\/www.hiphoppsych.co.uk\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Hip Hop Psych<\/span><\/a><span style=\"font-weight: 400\">, a social venture that we co-founded, aims to bridge the gap between the medical community and hip-hop culture by working directly with health professionals and the public to raise awareness about mental health in accessible and evidence based ways.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We explore the mental health symptoms and emotional states of characters described in hip hop songs, and we interpret the lyrics and language in order to raise awareness about various mental disorders (including depression, psychosis, anxiety, borderline personality disorder, etc). We also translate medical terms and information (such as the \u201cbiopsychosocial model\u201d) into much more accessible and contextualised language (eg,<sup>11-12<\/sup>) so the general public can understand them. In addition to generating culturally sensitive psychoeducational resources, we perform \u201canti-stigma\u201d events in various settings (prisons, nightclubs, African and Caribbean societies, etc),<sup>13<\/sup><\/span><span style=\"font-weight: 400\">\u00a0<\/span><span style=\"font-weight: 400\">and we use a similar approach to educate health professionals and academics to help increase their awareness of how some people describe their personal experiences with mental health.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">We <\/span><span style=\"font-weight: 400\">\u00a0need to create <\/span><span style=\"font-weight: 400\">innovative ways of engaging with people from <\/span><span style=\"font-weight: 400\">ethnic minority communities who face culturally sensitive barriers to accessing mental health services.<sup>14<\/sup><\/span><span style=\"font-weight: 400\">\u00a0For those who embrace hip-hop music and culture, we believe this medium could facilitate engagement, help <\/span><span style=\"font-weight: 400\">normalise mental health, and promote seeking access to treatment.<\/span> <span style=\"font-weight: 400\">In addition, we hope that the awareness gained from engaging with resources related to mental health and hip-hop can provide new ways for opening up conversations between health professionals and patients (such as asking if the patient likes music, and whether any particular genre, artist, and\/or song resonates with how they are feeling at the moment). Health professionals do not need to have any knowledge of hip-hop. Culturally sensitive and context relevant approaches might open up more patient centred ways of building trust and deepening discussions about mental health, which can otherwise feel very stigmatising. Likewise, understanding how mental health is portrayed in hip-hop can offer a different perspective for healthcare professionals, helping them to build empathy with someone whose experiences may be completely different to their own. This knowledge might also help health professionals to be more aware of any potential trends that link the contagion effects of suicide, self harm, and self medication from hip-hop icons to some of their patients, and to be prepared for this.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Hip-hop is an art form filled with passionate expression, and its lyrics resonate with people all over the world. Art and culture are powerful mediums for self expression and for translating experiences. It can also help shape perceptions, challenge opinions, bridge communication, and help to understand other people&#8217;s points of views.\u00a0<\/span><span style=\"font-weight: 400\">Since the genre\u2019s conception almost five decades ago, hip-hop\u2019s progressive narratives have increasingly addressed stigma around mental health problems. <\/span><span style=\"font-weight: 400\">Hip-hop artists are speaking candidly through their art form, and they may be helping people around the world to acknowledge their own inner <\/span><span style=\"font-weight: 400\">struggles. <\/span><\/p>\n<p><b><i>Akeem Sule<\/i><\/b><i><span style=\"font-weight: 400\"> is a co-founder of Hip Hop Psych.
