The recently published <\/span><a href=\"https:\/\/www.england.nhs.uk\/greenernhs\/wp-content\/uploads\/sites\/51\/2020\/10\/delivering-a-net-zero-national-health-service.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Delivering a &#8220;Net Zero&#8221; National Health Service<\/span><\/a><span style=\"font-weight: 400\"> document reports that the NHS is responsible for 4% of England\u2019s total carbon footprint. The <\/span><a href=\"https:\/\/www.longtermplan.nhs.uk\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">NHS Long Term Plan<\/span><\/a><span style=\"font-weight: 400\"> recognises <\/span><span style=\"font-weight: 400\">the health service\u2019s huge carbon footprint<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">about <\/span><span style=\"font-weight: 400\">60%<\/span><span style=\"font-weight: 400\"> of which is from the procurement of goods and services. Although there have been <\/span><a href=\"https:\/\/www.england.nhs.uk\/2016\/01\/climate-change\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">improvements<\/span><\/a><span style=\"font-weight: 400\">, there is a long way to go and time is running out.<\/span><\/p>\n<p><span style=\"font-weight: 400\">As covid-19 spread, so did panic. Guidelines were implemented quickly. Measures such as hand hygiene, face coverings, and social distancing were promoted to protect individuals and the public. To maintain public trust, every organisation had to be seen to be &#8220;doing something&#8221; that makes people &#8220;feel safer.&#8221; Was there enough evidence to back these policies?\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Many safety measures came with a price<\/span><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">disposable plastic. It seems to have come to the rescue. But while feeling safe is important, any measure must improve safety, not just create an illusion of it, particularly if that measure causes long-lasting <\/span><a href=\"https:\/\/www.nationalgeographic.com\/environment\/habitats\/plastic-pollution\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">harm<\/span><\/a><span style=\"font-weight: 400\"> to the environment.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Understandably, some targets set in the NHS Long Term Plan may be put on hold while the pandemic is dealt with. But the pandemic should not eclipse the climate emergency.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Personal protective equipment (PPE), has been a hot topic since the beginning of the covid-19 crisis, and policies regarding PPE have been updated multiple times. In the updated <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/910885\/COVID-19_Infection_prevention_and_control_guidance_FINAL_PDF_20082020.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Infection Prevention and Control guidance<\/span><\/a><span style=\"font-weight: 400\">,<\/span><span style=\"font-weight: 400\"> published by Public Health England, patients are categorised into low, medium, and high risk groups. The majority of attendees at outpatient services are &#8220;medium risk&#8221; where an asymptomatic individual with unknown covid-19 status is accessing care. The PPE advice includes single-use plastic aprons, single-use gloves, plus surgical face masks and visors\/goggles.<\/span><\/p>\n<p><span style=\"font-weight: 400\">A single clinic, with about 50 patients daily, disposes of approximately 500 plastic aprons and 1000 gloves every week. Multiplied by the number of outpatient clinics running in all trusts, the waste produced becomes immense.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Two questions need answers. Does wearing gloves and an apron when treating an asymptomatic patient offer any meaningful protection? And is there a more pragmatic alternative which reduces the amount of plastic waste without endangering healthcare workers?<\/span><\/p>\n<p><span style=\"font-weight: 400\">The <\/span><a href=\"http:\/\/www.nipcm.hps.scot.nhs.uk\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">National Infection Prevention and Control Manual (NIPCM)<\/span><\/a><span style=\"font-weight: 400\">, which provides the evidence published in the infection prevention and control guidance, refers to literature reviews on single-use <\/span><a href=\"https:\/\/hpspubsrepo.blob.core.windows.net\/hps-website\/nss\/2604\/documents\/1_sicp-lr-gowns-v2.0.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">aprons and gowns<\/span><\/a><span style=\"font-weight: 400\"> and single-use <\/span><a href=\"https:\/\/hpspubsrepo.blob.core.windows.net\/hps-website\/nss\/2607\/documents\/1_sicp-lr-gloves-v3.0.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">gloves<\/span><\/a><span style=\"font-weight: 400\">. But these documents provide no evidence for using aprons and gloves to prevent transmission of an airborne infection from an asymptomatic individual wearing a face covering.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">The NICE <\/span><a href=\"https:\/\/cks.nice.org.uk\/topics\/healthcare-associated-infections\/management\/management\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">guidance<\/span><\/a><span style=\"font-weight: 400\"> states \u201cwear (aprons) if there is a risk that clothing may be exposed to blood, body fluids, secretions or excretions\u201d. A WHO <\/span><a href=\"https:\/\/www.who.int\/news-room\/commentaries\/detail\/transmission-of-sars-cov-2-implications-for-infection-prevention-precautions\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">scientific brief<\/span><\/a><span style=\"font-weight: 400\"> (June 2020), states \u201cdespite consistent evidence as to SARS-CoV-2 contamination of surfaces and the survival of the virus on certain surfaces, there are no specific reports which have directly demonstrated fomite transmission. And although the WHO report emphasises the role of asymptomatic and presymptomatic transmission of covid-19, it states that according to a few studies, 0% to 2.2% of people with asymptomatic infection infected anyone else. <\/span><a href=\"https:\/\/www.who.int\/infection-prevention\/tools\/core-components\/evidence.pdf?ua=1\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Hand hygiene<\/span><\/a><span style=\"font-weight: 400\"> mitigates most of that risk.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">According to the Department of Health environment and sustainability <\/span><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/167976\/HTM_07-01_Final.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">manual<\/span><\/a><span style=\"font-weight: 400\">, if aprons and gloves are not mixed with other waste (often the case in clinical practice), they should be disposed of in orange waste bags to be incinerated or undergo &#8220;alternative treatment&#8221; (chemical or heat disinfection). Incinerators <\/span><a href=\"https:\/\/ukwin.org.uk\/files\/pdf\/UKWIN-2018-Incineration-Climate-Change-Report.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">release<\/span><\/a><span style=\"font-weight: 400\"> an average of 1 tonne of CO<\/span><span style=\"font-weight: 400\">2 <\/span><span style=\"font-weight: 400\">for every tonne of waste.<\/span><\/p>\n<p><span style=\"font-weight: 400\">As healthcare workers we should avoid measures aimed at mitigating the pandemic which are of no significant benefit and which increase the environmental damage caused by single-use plastics. The environmental damage caused by dealing with the pandemic has already been <\/span><a href=\"https:\/\/www.england.nhs.uk\/greenernhs\/wp-content\/uploads\/sites\/51\/2020\/10\/delivering-a-net-zero-national-health-service.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">recognised<\/span><\/a><span style=\"font-weight: 400\">. The NHS could continue to be at the forefront of efforts to contain covid-19 in a more environmentally friendly manner. <\/span><a href=\"https:\/\/www.england.nhs.uk\/greenernhs\/wp-content\/uploads\/sites\/51\/2020\/10\/delivering-a-net-zero-national-health-service.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Work<\/span><\/a><span style=\"font-weight: 400\"> has begun to reduce the environmental impact of NHS PPE, such as <\/span><a href=\"https:\/\/www.leedsccg.nhs.uk\/nhs-leeds-ccg-continuing-care-team\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">trialling reusable face masks<\/span><\/a><span style=\"font-weight: 400\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">By incorporating evidence-based medicine and common sense, we could reduce our plastic waste while continuing to work in a safe environment and help to make the NHS ambition of achieving net zero emissions by 2040 a reality.
