{"id":49127,"date":"2020-11-27T19:11:00","date_gmt":"2020-11-27T18:11:00","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=49127"},"modified":"2020-12-02T23:08:52","modified_gmt":"2020-12-02T22:08:52","slug":"miles-sibley-we-need-to-change-the-hierarchy-of-evidence-based-medicine","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/11\/27\/miles-sibley-we-need-to-change-the-hierarchy-of-evidence-based-medicine\/","title":{"rendered":"Miles Sibley: We need to change the hierarchy of evidence-based medicine"},"content":{"rendered":"<p><span style=\"font-weight: 400\">In July 2020, the Independent Medicines and Medical Devices Safety Review published a report entitled \u201cFirst Do No Harm\u201d<\/span><span style=\"font-weight: 400\">.<sup>1 <\/sup><\/span><span style=\"font-weight: 400\">Also known as the Cumberlege Review, the report set out extensive evidence of avoidable harm in healthcare. It described \u201cheart wrenching stories of acute suffering, families fractured, children harmed\u201d. The suffering had occurred over years, even decades.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The harm was avoidable because patients and families had repeatedly spoken out about the ill effects of the treatments they had received. But no one had wanted to hear them.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In a letter to the Secretary of State of Health and Social Care, Baroness Cumberlege (chair of the review) said, \u201cThe healthcare system\u2026 does not adequately recognise that patients are its raison d\u2019etre. It has failed to listen to their concerns\u2026 and has too often moved glacially\u201d<\/span><span style=\"font-weight: 400\">.<sup>2<\/sup><\/span><\/p>\n<p><span style=\"font-weight: 400\">The words \u201chealthcare system\u201d are important. The NHS is full of dedicated staff who, at a one-to-one level with patients, offer deeply personal and compassionate care. But too often the system as a whole seems institutionally deaf to the patient voice.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Taking Cumberlege as its starting point, The Patient Library\u2019s latest report, <\/span><a href=\"https:\/\/www.patientlibrary.net\/InadmissibleEvidenceFINAL.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Inadmissible Evidence<\/span><\/a><span style=\"font-weight: 400\"><sup>3<\/sup><\/span><span style=\"font-weight: 400\"> reveals a double standard in evidence-based medicine. Medical evidence is respected, and built into professional training and practice. Evidence based on patients\u2019 experience, described as \u201canecdotal\u201d or \u201csoft\u201d evidence, is all too often ignored.<\/span><\/p>\n<p><span style=\"font-weight: 400\">It was ignored at the <\/span><a href=\"http:\/\/pexlib.net\/?195069\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Mid Staffordshire NHS Foundation Trust<\/span><\/a>,<span style=\"font-weight: 400\">\u00a0where, in the words of Sir Robert Francis, the Board \u201cdid not listen sufficiently to its patients&#8221;<\/span><span style=\"font-weight: 400\">.<sup>4<\/sup> It was ignored at <\/span><a href=\"http:\/\/pexlib.net\/?10203\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Morecambe Bay<\/span><\/a><span style=\"font-weight: 400\">,<sup>5<\/sup> where patients\u2019 concerns about harms in maternity services simply disappeared within \u201ca pattern of failure to recognise\u2026the problem, with, in some cases, denial that any problem existed\u201d. The same kind of pattern occurred at <\/span><a href=\"http:\/\/pexlib.net\/?80610\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Southern Health<\/span><\/a><span style=\"font-weight: 400\">,<sup>6<\/sup> and at <\/span><a href=\"http:\/\/pexlib.net\/?177680\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Gosport<\/span><\/a><span style=\"font-weight: 400\">,<sup>7<\/sup> and in the Northern Ireland\u00a0<\/span><a href=\"http:\/\/pexlib.net\/?171239\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">hyponatraemia scandal<\/span><\/a><span style=\"font-weight: 400\"><sup>8<\/sup><\/span><span style=\"font-weight: 400\"> and at <\/span><a href=\"http:\/\/pexlib.net\/?194265\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Cwm Taf<\/span><\/a><span style=\"font-weight: 400\">.<sup>9<\/sup>\u00a0 It seems likely that ongoing investigations at Shrewsbury &amp; Telford<sup>10<\/sup><\/span><span style=\"font-weight: 400\"> and East Kent<sup>11<\/sup><\/span><span style=\"font-weight: 400\"> will reach the same basic conclusion: that when things go badly wrong patient experience evidence is deemed inadmissible.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Over and over again, our evidence-based healthcare system has not wanted to accept patient feedback as evidence. And so, over and over again, patients have been harmed\u2014unnecessarily, and in large numbers.<\/span><\/p>\n<p><span style=\"font-weight: 400\">It doesn\u2019t have to be this way. We\u2014the <a href=\"http:\/\/www.patientlibrary.net\" target=\"_blank\" rel=\"noopener noreferrer\">Patient Experience Library\u2014<\/a>are on a mission to put patient experience work on the same evidence-based footing as clinical work.<\/span><\/p>\n<p><span style=\"font-weight: 400\">We started by <\/span><span style=\"font-weight: 400\">setting up the library<\/span><span style=\"font-weight: 400\"> as the national evidence base for patient experience and involvement. We have gone on to develop <\/span><a href=\"https:\/\/www.patientlibrary.net\/cgi-bin\/library.cgi?page=Surveys;\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">analytical tools<\/span><\/a><span style=\"font-weight: 400\"> to help NHS staff find and make sense of patient feedback. We routinely <\/span><a href=\"https:\/\/www.patientlibrary.net\/cgi-bin\/library.cgi?page=Services_Publications\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">disseminate research findings<\/span><\/a><span style=\"font-weight: 400\"> in formats that can be easily assimilated by busy professionals.<\/span><\/p>\n<p><span style=\"font-weight: 400\">But we want to go further. We want to see:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Parity of access<\/span><span style=\"font-weight: 400\"> to patient experience evidence alongside medical evidence. NICE and Health Education England make <\/span><a href=\"https:\/\/www.nice.org.uk\/About\/What-we-do\/Evidence-Services\/journals-and-databases\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">a series of medical research databases<\/span><\/a><span style=\"font-weight: 400\"> available to all NHS staff. They should make patient experience evidence available on the same basis.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Research prioritisation<\/span><span style=\"font-weight: 400\">. Bodies such as NICE <\/span><a href=\"https:\/\/www.nice.org.uk\/about\/what-we-do\/our-research-work\/methodological-research-areas\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">prioritise research<\/span><\/a><span style=\"font-weight: 400\"> by understanding which areas of clinical practice are well evidenced, and which merit further enquiry. We need a similar approach for patient experience evidence, with better targeting of research effort and funding.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Professional development<\/span><span style=\"font-weight: 400\">. Clinicians learn continuously via training, guidelines, and journals. People charged with the challenging responsibility of hearing from patients should also have a systematic, evidence-based learning infrastructure.<\/span><\/li>\n<li style=\"font-weight: 400\">Analytics<span style=\"font-weight: 400\">. Clinicians have diagnostic aids to help them make sense of complex symptoms and conditions. Patient experience staff should have their own analytical tools to help them make sense of complex evidence emerging from surveys, complaints, online feedback and more.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">We believe that the culture in medicine is already starting to shift. There are many, many health professionals who understand the value of patient feedback\u2014not just as \u201cstories\u201d or \u201ccomplaints\u201d, but as evidence. Sooner or later, the NHS\u2014at the institutional and system level\u2014will begin to <\/span><span style=\"font-weight: 400\">catch up.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In the meantime, we are propelled by comments like this, from the Cumberlege review:<\/span><\/p>\n<p><span style=\"font-weight: 400\">\u201cPatients often know when something has gone wrong with their treatment. All too often they are the first to know.
