{"id":48658,"date":"2020-10-02T14:52:03","date_gmt":"2020-10-02T13:52:03","guid":{"rendered":"https:\/\/blogs.bmj.com\/bmj\/?p=48658"},"modified":"2020-10-09T17:03:26","modified_gmt":"2020-10-09T16:03:26","slug":"breaks-at-breaking-point-doctors-need-to-take-time-out-in-a-pandemic","status":"publish","type":"post","link":"https:\/\/blogs.bmj.com\/bmj\/2020\/10\/02\/breaks-at-breaking-point-doctors-need-to-take-time-out-in-a-pandemic\/","title":{"rendered":"Breaks at breaking point\u2014doctors need to take time out in a pandemic"},"content":{"rendered":"<p><span style=\"font-weight: 400\">Work during the covid-19 pandemic\u2014<\/span><span style=\"font-weight: 400\">whether it is frontline care, administration, management or pastoral support\u2014<\/span><span style=\"font-weight: 400\">has two hallmarks which makes it problematic for fatigue and performance: it is relentless and it is unpredictable.<\/span><\/p>\n<p><span style=\"font-weight: 400\">The demands on healthcare workers are high, shifts are long, and urgent work is carried out in the shadow of a rapidly accumulating backlog of \u201cnormal\u201d work. <\/span><a href=\"https:\/\/blogs.bmj.com\/bmj\/2020\/05\/04\/covid-fatigue-is-taking-an-enormous-toll-on-healthcare-workers\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Commentaries<\/span><\/a><span style=\"font-weight: 400\"> have pointed out an increasing \u201ccovid fatigue\u201d among healthcare workers, a state compounded by the disappearance of normal periods of respite\u2014<\/span><span style=\"font-weight: 400\">fewer chances to grab a coffee, no time to catch up with colleagues, less informal chat in the corridors.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Spending time continuously \u201con task\u201d has clear negative consequences for performance and wellbeing. More than a century ago, Japanese student <\/span><a href=\"https:\/\/www.apadivisions.org\/division-35\/about\/heritage\/tsuruko-haraguchi-biography\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Tsuro Arai<\/span><\/a><span style=\"font-weight: 400\"> set herself the masochistic task of solving complex multiplication problems continuously over four 12-hour days. By forcing herself to work continuously without rest and meticulously documenting the results, Arai provided the first firm evidence that (predictably) humans cannot maintain cognitive performance over 12 hours without a break. Instead, performance becomes slower and less accurate as \u201ctime on task\u201d increases, only returning to normal after periods of respite. Fast forward more than a century, and healthcare workers are regularly working lengthy periods without a break, trying to deal with a pandemic that has been ongoing for many months, in an already over-stretched health service.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Even in times of normal service, health-relevant decisions and behaviours change as time on task increases. <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamainternalmedicine\/fullarticle\/1910546\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">GPs are more likely to prescribe unnecessary antibiotics towards the end of clinics<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19631284\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">gastroenterologists are less likely to detect polyps in afternoon than morning colonoscopies<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25365728\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">compliance with hand hygiene protocols declines from the start to the end of shifts<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1002\/hec.3933\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">surgeons are less likely to decide patients require surgery towards the end of shifts<\/span><\/a><span style=\"font-weight: 400\">, and <\/span><a href=\"https:\/\/psycnet.apa.org\/record\/2019-15022-005\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">telephone helpline nurses become progressively more likely to refer callers to other healthcare professionals<\/span><\/a><span style=\"font-weight: 400\"> as time since last break increases. The latter example raises a key point\u2014<\/span><span style=\"font-weight: 400\">changes in nurses\u2019 decision making were related specifically to the time elapsed <\/span><i><span style=\"font-weight: 400\">since last break<\/span><\/i><span style=\"font-weight: 400\"> and not to workload or time on shift in general. In other words, it is the timing and presence (or absence) of breaks within the work period, rather than overall workload which is critical.<\/span><\/p>\n<p><span style=\"font-weight: 400\">In addition to being demanding, work in a pandemic is novel and unpredictable. Workers must adapt rapidly to an evolving situation, thinking flexibly, solving problems, staying focused and remaining vigilant to change. This requires <\/span><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4084861\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">a type of cognitive functioning that is effortful and draining<\/span><\/a><span style=\"font-weight: 400\">. Under normal circumstances, this kind of high level, intensive thought is less common\u2014<\/span><span style=\"font-weight: 400\">most situations are at least somewhat familiar and can be navigated with little conscious deliberation. In the pandemic, this balance has been reversed and human cognition is simply not designed to operate in its most effortful mode for protracted periods. Extended periods of high level thought quickly lead to fatigue and associated changes in decision making and behaviour.<\/span><\/p>\n<p><span style=\"font-weight: 400\">From a worker\u2019s point of view, fatigue can be thought of as an <\/span><a href=\"https:\/\/books.google.co.uk\/books?id=oQBeAwAAQBAJ&amp;dq=robert+hockey+mental+fatigue&amp;lr=\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">adaptive signal<\/span><\/a><span style=\"font-weight: 400\">\u2014<\/span><span style=\"font-weight: 400\">a tell-tale sign that we have spent too long \u201con task\u201d and should change task or take a break before performance suffers and mistakes or accidents occur. Recent <\/span><a href=\"https:\/\/www.bmj.com\/content\/364\/bmj.l481\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">campaigns<\/span><\/a><span style=\"font-weight: 400\"> and <\/span><a href=\"https:\/\/anaesthetists.org\/Fatigue\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">initiatives<\/span><\/a><span style=\"font-weight: 400\"> highlight the importance of healthcare staff responding to fatigue and taking adequate breaks. Breaks are important in any line of work, under any circumstances, but should be considered essential in the present situation where work is particularly mentally draining.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Enabling staff to take adequate breaks in a busy, over-stretched health service is easier said than done. One possible solution is the adoption (in addition to standard breaks) of regular \u201cmicrobreaks\u201d throughout busy or demanding work periods. <\/span><a href=\"https:\/\/www.bbc.com\/worklife\/article\/20190312-the-tiny-breaks-that-ease-your-body-and-reboot-your-brain\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">Microbreaks<\/span><\/a><span style=\"font-weight: 400\">, as the name implies, are short breaks, typically of a few minutes\u2019 duration which provide enough respite to mitigate some of the declines that would otherwise occur in wellbeing and performance over time. Studies in surgery for example, show that surgeons who take regular, brief microbreaks (of 90 seconds to 5 minutes) during operations, report; <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/28059962\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">improvements in physical and mental performance, reductions in pain and discomfort<\/span><\/a><span style=\"font-weight: 400\">, <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/20835716\/\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400\">reduced stress, and fewer intraoperative events<\/span><\/a><span style=\"font-weight: 400\">. Importantly, operations with microbreaks take no longer than standard operations, suggesting that the breaks &#8220;pay for themselves&#8221; by counteracting normal reductions in efficiency over time.<\/span><\/p>\n<p><span style=\"font-weight: 400\">As we move into the next phase of the pandemic with winter and a potential second wave on its way, it\u2019s more important than ever to prioritise breaks. Work demands remain high, and mass disruption to services ensures that change and unpredictability will continue for the foreseeable future. Healthcare staff have been operating beyond capacity for an extended period and are already fatigued.
